About this file

This is an Insurance Checklist for Standard City Contracts issued by the City of West Sacramento, California, establishing mandatory insurance requirements for all contractors and vendors engaged in City contracts. The checklist specifies comprehensive insurance coverage requirements that all vendors must maintain and document through a Certificate of Insurance (COI) submitted to the City of West Sacramento at 1110 West Capitol Avenue, West Sacramento, CA 95691. Required coverage includes Commercial General Liability at $1,000,000 per occurrence/$2,000,000 aggregate, Commercial Automobile Liability at $1,000,000 per accident, Workers' Compensation and Employers' Liability per statutory requirements, and Professional Liability (Errors & Omissions) at $1,000,000 per claim if applicable to the contract scope.

The insurance requirements mandate that the City be named as an additional insured on all policies except Workers' Compensation and Professional Liability, with copies of Additional Insured Endorsements (equivalent to CG 2010 0714) required as proof of endorsement. All policies must include Primary and Noncontributory Endorsements and Separation of Insureds clauses. Workers' Compensation policies must include a Waiver of Subrogation Endorsement in favor of the City, or a Workers' Comp Waiver for self-employed contractors with no employees. All certificates must provide thirty (30) days' written notice to the Certificate Holder prior to cancellation or material change. Contractors and vendors are instructed to forward this checklist to their insurance agents or brokers to ensure compliance, and certificates that lack the required Additional Insured Endorsements will be considered incomplete.
