INSTRUCTIONS TO OFFERORS
The Department of Health and Human Services, the Nashville Area Office Indian Health Service (NAOIHS), is soliciting quotes to provide Contractor shall provide maintenance of the grounds surrounding the clinic. Contractor is required to furnish all labor, equipment, materials, supplies, and supervision for all work detailed in this contract. Contractor shall provide contact information for business hours and after hours for all seasons
QUESTIONS
If you have questions regarding this requirement, please submit your inquiries immediately via e-mail to michael.thorne @ihs.gov but no later than April 22, 2024 5:00 PM EST. Questions with the Government’s responses will be posted to SAM.gov. Please be advised that the Government reserves the right to transmit those questions and answers of a common interest to all prospective Offerors.
PROPOSAL SUBMISSION DOCUMENTATION
The Offeror shall complete the following in order to submit a complete proposal.
(a) Complete and return Standard Form 1449, including your DUNS/Unique Entity Identifier (UEI) number. Include contact information for your Principal (the person authorized to represent you and negotiate in this matter). ***NOTE:
Sign in Block 30a, Name and Title of Signer in Block 30b, Date in Block 30c.
Include the Offeror’s DUNS/ Unique Entity Identifier (UEI) number and email address for correspondence in Block 17a.
(b) Complete and return any Amendments (SF30) that has been issued, complete Block 8, 15A and 15B before submitting.
(c) Complete Pricing Schedule (pages 1 and 2) Complete a and b to include the Offerors information.
(d) Technical Approach & Relevant Experience (5-page max, MS Word, or Adobe Acrobat PDF) – Provide a narrative that details your company’s technical approach towards successfully fulfilling the SOW requirements. Offeror narrative will be evaluated to determine if it demonstrates proven methods and techniques, and the extent to which the offeror’s methods and approach to performing all aspects of the scope of work, specifically the SOW tasks, are expected to result in successful completion of the proposed tasks and technical requirements within.
Your quote should convey the story about how the work will be successfully completed and any possible insights on how your company would mitigate possible unforeseen challenges. Your narrative must reflect your company’s understanding of the tasks involved to complete the required work. List of Key Personnel and description of their technical abilities, training, and experience.
(e) Past Performance (5-page max, MS Word, or Adobe Acrobat PDF) - Offeror must, at a minimum provide past experience on similar type projects within the last five (5) years. The past experience should indicate successes of the contractor in completing similar type work projects with the government. The offeror will be evaluated with respect to its past performance of projects similar in size and scope to the requirements in this solicitation. If performance ratings are not available in Contractor Performance Assessment Reporting System (CPARS), then the government shall use the information received from the offeror.
It is presumed that offerors have previously had an opportunity to respond to adverse past performance for ratings that were entered in CPARS; therefore, offerors will not be provided another opportunity to address adverse past performance if their CPARS ratings are used as part of the evaluation.
The evaluation of past performance is a subjective assessment based on consideration of all relevant facts and circumstances.
The evaluation may consider information from any source(s) deemed necessary by the Contracting Officer. The Government will seek to determine whether the offeror has consistently demonstrated a commitment to customer satisfaction and timely delivery of quality goods and services at fair and reasonable prices.
(f) Price/Cost -(3-page max, MS Word, or Adobe Acrobat PDF) The agency will evaluate price based on competitive pricing, government estimate, and/or market prices to determine the proposed price is fair and reasonable. Price shall include a detailed breakdown of costs to support the Firm-Fixed Price amount proposed for the Pricing Schedule.
The award will be made to the schedule contractor that represents the best value (see FAR 8.404(d)). This request does not commit the Government to pay any costs incurred in the submission of any quote, nor to contract for services.
Note that full, accurate, and complete information is required by this request in accordance with 18 U.S.C. § 1001 which also prescribes the penalties for making false statements.
Proposals must be submitted electronically via email to Contract Specialist: Michael Thorne email: Michael.thorne@ihs.gov on or before the RFQ closing date of May 3, 2024 5:00 PM EST. Offerors shall quote on all required items. The government will not split this requirement.
This solicitation is 100% set-aside for Native Owned small business.
