About this file
This is an instruction document issued by Palm Beach County, Florida, detailing the requirements for completing OSBD (Office of Small Business Development) Schedules 1 through 4 for state and local contract opportunities, specifically a Grounds Maintenance and WUD Easements term contract. The document specifies that all Prime Contractors and Subcontractors/Subconsultants must complete and submit Schedules 1, 2, and 2(a) with their bids or proposals to be considered responsive to solicitations when an API (Aspiring Professionals Initiative) goal is required. Schedule 1, completed by the Prime Contractor, must identify the company information, work responsibility percentages or dollar amounts, and all subcontractors/subconsultants designated as either SBE (Small Business Enterprise) or Non-SBE, with corresponding work percentages or amounts. Schedule 2 or 2(a) must detail the specific work to be performed or materials supplied for each subcontractor/subconsultant, with dollar amounts or work percentages and applicable line item numbers.
Schedules 3 and 3(a) are submitted by the Prime Contractor with each payment request to Palm Beach County and must track subcontracting information including contracted amounts, approved change orders, amounts drawn, and payment dates. Schedule 4, a fillable PDF form provided by the user department, documents actual payments made to subcontractors/subconsultants and must be signed and notarized by the recipient, with separate forms required for each subcontractor/subconsultant and each payment disbursement. All forms must be properly executed and signed by authorized individuals with their titles included. The document references the OSBD website at https://pbc.gov/pbcvendors for locating vendor codes and provides a link to file SBD Ordinance violation complaints at https://discover.pbc.gov/HED/osbd.
