About this file

This document contains instructions for completing Schedules 1-4, which are required subcontracting and payment reporting forms for Palm Beach County, Florida government contracts. The schedules must be submitted by prime contractors and their subcontractors/subconsultants to comply with the county's Office of Small Business Development (OSBD) requirements whenever an Apprenticeship and Pre-Apprenticeship Initiative (API) goal is mandated. Schedule 1, submitted with the bid or proposal, requires the prime contractor to identify all subcontractors and subconsultants, designate whether each is a certified Small Business Enterprise (SBE), indicate the dollar amount or percentage of work assigned to each party, and calculate the total SBE participation percentage. Schedule 2 or 2(a) must be completed by all subcontractors and subconsultants, detailing the specific work to be performed or materials supplied with corresponding dollar amounts or work percentages. For any subcontractor that will further subcontract work to second or third-tier subs, separate Schedule 2 forms must be submitted showing this arrangement.

Schedules 3 and 3(a) are payment-related forms submitted by the prime contractor with each payment request to Palm Beach County. Schedule 3 tracks subcontractor participation, contracted amounts, approved change orders, payments made, and payment dates; Schedule 3(a) is specific to professional services contracts and requires the prime to report the percentage of work performed by their own workforce and work completion percentages by subcontractants. Schedule 4 is a fillable PDF form submitted with payment requests to document actual disbursements made to subcontractors and subconsultants, requiring the subcontractor's signature and notarization to verify payment receipt. The prime contractor is responsible for managing all schedules, and violations of the SBE Ordinance may be reported to the OSBD through the county's complaint process. Additional vendor information and resources are available through the OSBD website at https://pbc.gov/pbcvendors and https://discover.pbc.gov/HED/osbd.
