STATEMENT OF WORK

INSTALL FIRE DEPARTMENT EXTRACTOR, B11

JKSE 192408

23 April 2020

GENERAL:

This statement of work (SOW) defines the services to be performed in the fulfillment of this contract. The work consists of furnishing all materials, labor, equipment and supervision as identified in this SOW or within the Contract Drawing. Changes and/or modifications must have prior approval from the Contracting Officer. The Contractor is responsible for ensuring proper documentation is received for personnel and vehicle passes obtained in time to avoid any delay in project start of performance. Please ensure personnel documentation is Real ID Act compliant. This includes personal attainment of contractor badges and proper advance notification to an Air Force Civil Engineer squadron representative for scheduling of escort support, see Section 01010 at the end of this document. See Section 01000 specifications at the end of this document for Contract specifics such as payment, work hours, job sites access, O&M Manuals, and Electronic AutoCad As-Builts.

MEASUREMENT AND PAYMENT

The work described here after will be paid thru the following Contract Line Item Numbers:

CLIN 1: Provide all necessary equipment, labor, materials, and incidentals to Install Fire Department Extractor, B11 as per the Statement of Work and Contract Drawings.

CLIN 2: CONTRACTOR MANPOWER REPORTING (CMRA): The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the contractor will report ALL contractor manpower (including subcontractor manpower) required for performance of this contract. The contractor is required to completely fill in all the information in the format using the following web address https://contractormanpower.army.pentagon.mil or https://cmra.army.mil. See Attachment 2 for more information.

PROJECT DESCRIPTION and SCOPE:

1. Demolition:

a. Demo items has outlined on Contract Drawings.

b. Contractor is responsible for disposal of all demolished items.

c. Contractor is to protect areas adjacent to demolition from damage.

d. Patch and repair of finishes due to demolition will be the responsibility of the Contractor.

e. Provide a waste disposal reports, monthly, annotating weights of recycled materials- metal and concrete; and weighs of rubbish disposed of in the landfill.

2. Concrete:

a. Demolition per Contract Drawings.

b. Install reinforcing steel per Contract Drawings.

c. Install 6” of compacted fill; compact to 95 percent theoretical maximum density.

d. Install a durable, lightweight, closed-cell foam expansion joint w/ “tear-off” strip the full depth of the existing concrete, or approved equal, per Contract Documents

e. Install extractor utility slab-on-grade per Contract Drawings.

f. Finish concrete to match existing slab-on-grade.

g. Seal all expansion joints with an approved self-leveling sealant

3. Plumbing:

a. Demolition as per drawings.

b. Owner supplied extractor to be installed under a separate contract.

c. Install Contract Drawings’ specified water heater, or approved equal, in accordance with manufacturer’s installation instructions.

d. Install combustion air intake and flue outlet according to manufacturer’s installation instructions.

i. Maintain required clearances between flue outlet and combustion air intake, and from parapets, air intakes or other obstructions on roof structure.

ii. Flue vent shall be Schedule 40 CPVC or approved Polypropylene pipe

iii. Combustion air intake may be Schedule 40 PVC, Schedule 40 CPVC or approved Polypropylene pipe

e. Install a premanufactured pipe flashing boot for roof penetrations.

i. See Contract Drawings for further requirements

f. Connect to existing domestic water and natural gas lines per Contract Drawings with shut of valves that allow isolation of water heater from utility connections.

g. Install water hammer arrestor per Contract Drawings

h. Install ¾” hose bibb with integral vacuum breaker per Contract Drawings

i. Install a vent free gas regulator to reduce and maintain the inlet pressure to the water heater as recommended by the manufacturer.

j. Install new 4” drain under existing concrete floor per Contract Drawings to existing trench drain.

k. Pressure test all installed piping per industry standards and provide test/inspection report to Base Civil Engineer for review.

l. Insulate all domestic hot and cold water supply with Owens Corning SSL 11 with ASJ Max Preformed Pipe Insulation: fiberglass pipe insulation, factory applied all-service jacket with self-seal lap. The insulation shall have a minimum thickness of 1-1/2 inch. Apply insulation continuously over fittings, valves, and specialties with insulation inserts and PVC fitting covers.

m. Label all new and relocated hot water supply and return, domestic cold and hot water, vent, and sewer piping with self-adhesive pipe markers and flow indication arrows showing direction of flow. Labels shall be installed near each valve and control device, at branches, near penetrations, and at maximum distance of 50 feet along each run.

4. Electrical:

a. Demo all existing electrical outlets, lights and surface mounted conduit as required for proper installation water heater and extractor per Contract Drawings

b. Install electrical components per Contract Documents and manufacturer’s recommendations.

HOURS OF OPERATION: Normal Duty Hours: Mon – Fri 0700 – 1630 Hrs.

RSD Weekends: Sat – Sun 0600 – 1530 Hrs.

LOCATION: 120th AW, Great Falls, Montana.

SECTION 010000 - REQUIREMENTS AND INFORMATION PART 1 - GENERAL

FUNCTION AND AUTHORITY OF U.S. GOVERNMENT PERSONNEL

Contracting Officer. Contracting Officers are agents of the Government and are the only officials authorized to enter into any contract or agreement that binds the Government. Only the Contracting Officer may authorize the performance of additional work or make changes not specified in the contract.

Technical Representatives. The Contracting Officer will assign a representative (Contracting Officer’s Representative, COR) to act as the technical representative for the project. The duties of the COR are addressed at the time of contract award.

SCOPE

Project provides for furnishing all plant, labor, materials, equipment and supervision to perform Project No. JKSE 192408 at the Montana Air National Guard Great Falls, MT, herein and shown on the construction drawings.

The Contractor will be held responsible for all requirements described in the contract documents. Failure to familiarize itself with their requirements will not relieve the contractor of its responsibility to comply.

The organization of the specifications into divisions, sections, and articles and the arrangement of the drawings, shall not control the Contractor in dividing the work among subcontractors or in establishing the extent of the work to be performed by any trade.

DESCRIPTION OF WORK

The Work consists of furnishing all supervision, plant, labor, equipment, appliances, materials and incidentals performing all Work in strict accordance with the Specifications and Drawings.

ENTRY TO THE MONTANA AIR NATIONAL GUARD

The Montana Air National Guard (MANG) is a closed Base, pursuant to Sec 21, Internal Security Act of 1950, 50 U.S.C. 797 and, as such, only those persons granted permission may enter. It is, therefore, required that control be exercised over Contractor personnel while working on the installation. To maintain this control a list of all Contractor personnel and vehicles which will be working under the contract must be submitted to the Contracting Officer one week prior to the start of work under the contract. The employee list shall include the contract number, project number and title, employee name, gender, date of birth, social security number, and the estimated starting and ending date of each employee. A subsequent list of all additions or deletions will be submitted as employees are hired or released. The vehicle list shall include the contract number, project number and title, each vehicles description and license plate number.

The Contracting Officer will forward the completed employee list to Security Forces for the required Criminal Justice Information Network (CJIN) checks. These checks will be conducted prior to any individual names being put on an Entry Authority List (EAL). If information from the CJIN verifies no outstanding warrants or criminal history, that individual will be granted access to the installation for the term of the contract, or unless access is terminated by proper authority. If the information from CJIN indicates that the person has an outstanding warrant that person will be denied access to the installation and Security Forces will immediately notify local law enforcement. If the information from CJIN indicates that the person has a criminal history that is determined to be detrimental to the installation, the person will be denied access (examples include sabotage conviction/anti-government group affiliation, etc…).

All contractors with a contract longer than 30 days must report to Pass & Registration office, Bldg. #63 prior to their contract start date. Contractor passes will be issued that allow them access during the contract period. The individual names must be listed on the EAL prior to any pass being issued. Any individual driving on the installation must possess a Real ID Act compliant ID, valid drivers license, and comply with all of the vehicle registration requirements established by the State of Montana and the MANG.

WORK SCHEDULING

Work hours for the Contractor will normally be between the hours of 7:00 AM through 5:00 PM, excluding Saturdays, Sundays, and Federal Holidays. If the Contractor desires to work during periods other than above, notification must be provided to the Contracting Officer three (3) working days in advance of its intention to work during other periods to allow assignment of additional inspection personnel. When the Contracting Officer determines that Government inspection personnel are reasonably available, the Contractor may be authorized to perform in excess of their normal duty hours/days. However, if Government inspection personnel are required to perform in excess of their normal duty hours/days due to scheduling by the Contractor, the actual cost of the inspection, at overtime rates, will be charged to the Contractor and will be deducted from the Contract amount.

DIVISION OF WORK

The various divisions of the Specifications shall not be considered as negotiations of the material and labor involved. The arrangement and order of these divisions have been made for convenience only. It is not the intent, nor shall it be so construed, that a particular trade or subcontractor must perform the work in any one division.

Any item mentioned, under any division heading, must be supplied even though it is not specified under the heading for the respective work, but is shown on the Drawings. No claims for extras arising out of real or alleged error in such arrangement or order of the various divisions will be given consideration.

LIST OF SUBCONTRACTORS

A. The Contractor shall identify all subcontractors on the Standard Form (SF) 1413, “Statement of Acknowledgment”, who will be employed to work in the contract. Federal labor standards apply to all subcontractors as well as the prime contractor.

CORRELATION OF DRAWINGS, SPECIFICATIONS AND CONTRACTS

The Specifications, Contract and the accompanying Drawings are intended to describe and provide for complete and usable facilities. They are intended to be cooperative, and what is called for by one shall be as binding as if called for by all. The Contractor will understand that the work herein described shall be complete in every detail, notwithstanding every item necessarily involved is not particularly mentioned. The Contractor shall be held to provide all labor and material necessary for the entire completion of the work intended to be described, and shall not avail itself of any manifestly unintentional error or omission, should any exist.

Should any error or inconsistency appear in the Drawings or Specifications, the Contractor, before proceeding with the work, shall make mention of same to the Contracting Officer for proper adjustment, and in no case shall it proceed with the work in uncertainty.

Any proposed changes to these specifications by the Contractor must be submitted in writing to the Contracting Officer for approval prior to implementation.

REPORT OF ERROR AND DISCREPANCIES

The Contractor shall be responsible for any and all discrepancies in work due to failure to obtain dimensions and investigate conditions at the site before fabrication and installation.

The Contractor shall bear all cost in replacing all materials and labor due to not observing the above paragraph and such replaced materials shall meet the approval of the Contracting Officer.

The Contractor shall promptly notify the Contracting Officer in writing of any discrepancies.

SUBMITTALS

Shop Drawings and Product Data. The Contractor shall furnish submittals in the form of manufacture’s brochures, pamphlets, or written specifications on all items to be installed unless specifically directed otherwise by these specifications or by the Contracting Officer.

Approval of Materials. Prior to the purchase of material, the Contractor shall submit to the Contracting Officer, for material approval/disapproval, brochures and technical literature covering, in detail, the materials it proposes to supply. This shall include the specific catalog and model specification number designations. Submittals shall demonstrate that the item conforms to all of the requirements. No unapproved or disapproved materials shall be used. Submittals shall be made for the items listed on the Schedule of Material Submittals. The submittals shall be accomplished on, and in accordance with the instructions in the Air Force (AF) Form 3000, “Material Approval Submittal”. Unless there are objections or changes to the required submittal dates, the Contractor accepts the Schedule of Material Submittals. The number of submittals for each item and procedure for submission will be determined at the pre-construction conference. Following review coordination, one (1) set each of the submittal shall be maintained by the Contractor, the A/E, the COR, and the Contracting Officer. When the A/E has Type C Services and when reviewed by the A/E under type C Services, the A/E will accept and review up to three separate submittals of shop drawings for each item indicated on the Contractor’s accepted schedule of submittals. Additional reviews in excess of three submittals for the same item will be provided by the A/E; however, the Contractor shall reimburse the A/E and the A/E’s consultants for evaluating and reviewing each shop drawing submittal in excess of three submittals for the same item. The Contractor shall directly reimburse the A/E for both the A/E and A/E’s consultants at the rate of $107.36 per man-hour.

Schedule of Material Submittals is provided at the end of this document

MEANING OF APPROVED, DIRECTED ETC.

A. “Approved”, “Directed”, “Required”, “Applicable”, or words of like or similar effect, when used in the specifications shall be interpreted to mean “Approved By”, “Directed By”, etc., the Contracting Officer unless otherwise specifically stipulated.

GOVERNMENT FURNISHED PROPERTY

The Government furnished property for this project consists of the following: No Government furnished property or equipment.

COORDINATION MEETINGS

Project coordination meetings may be conducted at regularly scheduled times convenient for all parties involved. Project coordination meetings are in addition to specific meetings held for other purposes, such as regular progress meetings and special pre- installation meetings.

Representation at each meeting shall be as required by the Contracting Officer for the construction activities involved.

PROGRESS MEETINGS

Progress meetings at the Project site shall be as directed by the Contracting Officer.

Attendees. In addition to the Contracting Officer, each subcontractor, supplier or other entity concerned with current progress or involved in planning, coordination or performance of future activities shall be represented at these meetings by persons familiar with the Project and authorized to include matters relating to progress.

Agenda. Review and correct or approve minutes of the previous meeting. Review other items of significance that could affect progress. Include topics for discussion as appropriate to the current status of the Project. The Agenda will be provided by the Architect and mailed to each attendee prior to the meeting.

The Architect is responsible for conducting all meetings and for documentation and distribution of all meeting minutes.

Schedule Updating. Revise the Contract Progress Schedule (AF Form 3064) when directed by the Contracting Officer, where revisions to the schedule have been made or recognized.

PERFORMANCE OF THE WORK

Scheduling. All work under the Contract shall be arranged and carried out in such a manner as to complete the work efficiently. A Contract Progress Schedule (AF Form 3064) shall be submitted at the pre-construction conference for act. This chart shall show how the work is to progress from start to completion of the Contract as required by Contract Clause 52.236-15, Schedules for Construction Contracts. A Contract Progress Report (AF Form 3065) showing (in percentages) how the shall be submitted at the pre-construction conference for the work under work has been completed for each element of work listed in the Progress Schedule, shall be submitted to the Contracting Officer monthly.

Contractor use of the premises. Contractor shall conduct all work in such a manner as to minimize the inconvenience and disruption of the National Guard’s daily schedule. Confine operations at the site to the area permitted under the contract. Portions of the site beyond areas on which work is indicated are not to be disturbed. Conform to site rules and regulations affecting the work while engaged in project construction.

Keep existing driveways and entrances serving the premises clear and available to National Guard personnel at all times. Do not use these areas for parking or storage of materials. Lock automotive type vehicles, such as passenger cars and trucks and other mechanized or motorized construction equipment, when parked and unattended, so as to prevent unauthorized use.

Advise the COR whenever work is expected to be hazardous to National Guard personnel and/or operations.

STANDARDS OF MANUFACTURE

For the purpose of establishing the standard of construction and the requirements to be met in the work of all divisions, the Drawings and these Specifications are based on the use of products hereinafter specified, adapted to the installation as required to meet the condition.

Where brand names are shown, these names are intended to describe a quality of product, and in no way are intended to limit products of equal quality. Therefore, products of other manufacturers may be employed for this work provided they are equivalent materials and construction, and equality adaptable to the conditions as approved by the Contracting Officer. All recognized regulatory/code standards shall be the latest published edition.

COMPLIANCE WITH CODES AND REGULATIONS

All work shall be done in accordance with the applicable codes and/or ordinances in force at the time of construction. It is the Contractor’s responsibility to ensure that where Environmental Protection Agency (EPA), Department of National Resources (DNR) or other such regulations control the removal, handling, installation or disposal of materials, they shall be strictly adhered to whether or not specifically referenced in the construction documents.

To ensure compliance with OSHA and EPA, the Contractor shall data sheets available on site for materials used in the performance of the project.

INSPECTIONS AND TESTS

ALL testing costs associated with this project are the sole responsibility of the Contractor.

Inspections and tests are for the sole benefit of the Government and shall not relieve the Contractor of the responsibility of providing quality control measures to ensure that the work strictly complies with the Contract requirements. No inspection or test by the Government shall be construed as constituting or implying acceptance. All items to be concealed shall be Government inspected prior to concealment.

APPLICATION FOR PAYMENT

Payments may be made monthly based on the Government percent of work completed as listed on the AF Form 3065, “Contract Progress Report”. Requests shall be submitted to the Contracting Officer and shall contain the information required by Federal Acquisition Regulation Clauses 52.232-5, Payments Under Fixed-Price Construction Contracts, and 52.232.27, Prompt Payment for Construction Contracts. (These clauses will be provided in full text to the successful contractor.)

The amounts requested are only for performance in accordance with the specifications, terms, and conditions of the contract.

Payments to subcontractors and suppliers have been made from previous payments received under the contract, and timely payments will be made from the proceeds of the payment covered by this certification, in accordance with subcontract agreements and the requirements of chapter 39 of title 31, United States Code; and

This request for progress payments does not include any amounts that the prime contractor intends to withhold or retain from a subcontractor or supplier in accordance with the terms and conditions of the subcontract.

INSURANCE

A. The Contractor shall procure and maintain insurance as required in the Contract. The Contractor shall require and ensure compliance for insurance by subcontractors.

SAFETY

All work shall be performed in accordance with the Occupational Safety and Health Act (OSHA) and in accordance with U.S. Army Corps of Engineers Safety and Health Requirements Manual EM385-1-1. The more stringent of the two shall apply.

Any Contractor’s equipment that causes or generates electro-magnetic disturbances or interference shall be removed from service until properly repaired. The Contracting Officer may also require re-positioning or removal of the equipment from the Facility.

CUTTING AND REPAIRING

Unless otherwise specified hereinafter, the Contractor shall do all necessary cutting, drilling, fitting and patching of work and corresponding work that may be required to make several parts come together and fit it to receive or be received by work of other trades shown upon or reasonably implied by the Drawings and Specifications for the completed project.

The Contractor shall be held responsible for all cutting, replacement, and repairing of work that is due to faulty workmanship and which is not specifically covered by specifications for trades which are affected. The Contractor will also be held responsible for providing, without extra cost to the Government, any small incidental items which are not specifically mentioned in trade specifications, but which are necessary to complete the work in accordance with the drawings.

All cutting, drilling, fitting, patching, and repairing shall be finished to the quality established in the specifications and Contract documents

TEMPORARY FIELD OFFICES AND SHEDS

As soon as practical after award of Contract, and until final acceptance of the work, the Contractor shall provide, maintain and later remove, a suitable temporary office(s) for its own use. All field offices shall be maintained in good repair, and shall also be maintained in a clean and sanitary condition.

The Contractor may construct temporary sheds as it may require for the use of its workmen and as required for tool cribs and storage of all work under this Contract. Temporary sheds shall be confined to the space assigned by the Contracting Officer. Sheds shall be of approved construction.

The Contractor shall provide a facility on the job site to accommodate 5-10 personnel for regularly scheduled meetings. The facility shall have electricity, heat and air conditioning, and necessary seating and table.

TEMPORARY TOILET FACILITIES

The Contractor shall provide, at its expense, all temporary toilet facilities. All temporary toilet facilities shall meet the requirements of health authorities having jurisdiction and shall be pumped at regularly established intervals, kept clean, and in a sanitary condition at all times.

STORAGE

A. The Government will provide no secure storage space. The Government will not be responsible for property belonging to, or under the present control of the Contractor. The Contractor is to protect its materials. The Contracting Officer will designate an unsecured, open area for storage of construction equipment and materials during the period covered by this contract.

MISPLACED MATERIALS

A. Any material that is deposited elsewhere than areas designated as approved by the Contracting Officer shall be moved and deposited where directed. No payment will be made for handling such material. The Contracting Officer will notify the Contractor of any noncompliance with the foregoing provisions.

TELEPHONE

A. If the Contractor desires to provide telephone services at its field office for its own use and for the use of subcontractors, it shall make all arrangements with the telephone company and shall pay all charges for installation, services and removal

UTILITIES

The Contractor will be allowed to utilize Government provided utilities of water and electricity on the construction site. Any expense incurred to gain access to these utilities (temporary tap-ins, etc.) shall be the responsibility of the Contractor and all utilities shall be returned to their original configurations at the end of the contract. No alterations to existing utilities shall be accomplished without the written permission of the Contracting Officer.

All utility shutdowns require the prior approval of the COR. Request for utility shutdown shall be made in writing at least three (3) days prior to the expected date of implementation. Every effort shall be made to make all shutdowns as brief as possible, and as limited in extent as possible.

COMPRESSED AIR

A. The Contractor shall provide all compressed air used for work under this contract including temporary lines and connections. Remove all temporary lines, etc., at the completion of the work.

BLOCKING OF STREETS/BUILDING SECONDARY ENTRANCE DRIVES

A. At least three (3) working day prior to the blocking of any street, the Contractor shall advise the COR of its intentions, identifying the location and the estimated time of closure.

WEATHER PROTECTION AND TEMPORARY HEATING

A. The Contractor shall provide and maintain weather protection as may be required to properly protect all parts of the structure from damage during construction. The Contractor shall be responsible for repairs and maintenance to the heating system or units during the period of building construction and shall deliver same to the Government, at termination of such use, in perfect condition, cleaning out all air ducts and replacing all filters. Any temporary heating shall be at the expense of the Contractor. Any materials or equipment damaged by weather conditions shall be replaced at no cost to the Government.

This project has no salvageable items.

DEBRIS

The Contractor shall maintain the construction site in as clean and orderly condition as possible. All refuse and/or salvage material shall be gathered and disposed of daily to maintain the site in this condition. Any debris that blows off the site into surrounding areas must be removed daily.

All refuse, debris and construction waste shall be legally disposed of OFF Federal Property at the Contractor’s expense. The use of Government waste receptacles is strictly prohibited.

Provide monthly waste disposal reports annotating weights of recycled materials- metal and concrete; and weighs of rubbish disposed of in the landfill.

SALVAGE

A. All property identified as Government Salvage on the drawings shall be returned to the Government at a place on Post designated by the Contracting Officer Representative. The Contractor shall properly dispose of all other materials, not re-installed, OFF SITE.

DAMAGES AND REPAIRS

All damages by the Contractor’s operations shall be repaired, or replaced, at the Contractor’s expense, as directed by the Contracting Officer. Any Government property All existing sidewalks, curbs, and pavement disturbed, broken, or removed or otherwise damaged by the Contractor during performance of the work under this contract shall be replaced by the contractor at its own expense. Replaced sidewalks, curbs, and pavements shall be smooth, shall blend into the existing work, and shall not present depressions or humps.

HAUL ROUTES

The Contractor shall establish, prior to commencement of work and as approved by the Contracting Officer, entrance and exit routes at the construction site. When materials are transported in execution of the work, vehicles shall not be loaded beyond the loading limit established by Federal, State or Local law or regulation. The Contractor shall provide protection against damage when it is necessary to cross curbing or sidewalks. The Contractor should be aware of existing bridge limitations and obtain approvals from the appropriate jurisdictions for haul routes.

Maintenance of Traffic. It is understood and agreed that the Contractor shall provide for the free and unobstructed movement of vehicles on all areas with respect to its own operations and the operations of all its subcontractors. It is further understood and agreed that the contractor shall provide for the uninterrupted operation of visual and electronic signals (including power supplies thereto) used in the guidance of vehicles while operating to, from, and upon the site.

With respect to its own operations and the operation of all its subcontractors, the Contractor shall provide marking, lighting, and other acceptable means of identifying personnel, equipment, vehicles, storage areas, and any work or condition that may be hazardous to the operation of military vehicles, fire-rescue equipment, or maintenance vehicles at the construction area.

The Contractor shall furnish, erect, and maintain weighted barricades, warning signs, and other traffic control devices as required to maintain traffic and insure the safety of military vehicles and the Contractor’s equipment. The Contractor shall make its own estimate of all labor, materials, equipment, and incidentals necessary for maintaining efficient vehicular traffic patterns.

FINAL CLEANING AND ACCEPTANCE OF THE WORK

Prior to the acceptance of the facility and at such times as directed by the Contracting Officer, the Contractor shall thoroughly clean all exposed surfaces of the building where work under this contract was completed.

All protective coatings, except lacquers, shall be removed from finish surfaces and the finish surfaces shall be washed and cleaned. Contractor shall be held responsible for all damaged materials, and at completion, shall replace, at its own expense, all such damaged materials.

The Government shall conduct pre-final and final inspections of the work under this contract prior to contract completion. The Contractor shall allow time to schedule and conduct the inspections within the contract performance period, allowing 10 calendar days notice to the Contracting Officer of each inspection.

The red-line drawings shall be complete and available at the final inspection.

The Contractor shall guarantee all materials and workmanship free from defects or failure for a period of one (1) year from date of contract completion. The Contractor further agrees that he will, at his own expense, repair and replace all such defective materials and all other material damaged thereby that becomes defective during the term warranty. All warranty periods shall run from the date of final contract acceptance. Manufacturer’s material warranty shall remain en effect for length of material warranty specified, from the date of final contract acceptance.

MANUALS, PARTS, LISTS, ETC

A. Prior to final payment, the Contractor shall provide the Contracting Officer with three (3) bound sets containing complete maintenance, repair and operating instructions and parts lists for each piece of installed equipment.

AS-BUILT DRAWINGS

The Contractor shall keep at the work site, an accurate record of all deviations, indicated in RED, from the approved design drawings and specifications which may occur in the work as actually constructed. The Contractor shall submit to the Contracting Officer, within 30 calendar days after pre-final inspection, complete information including descriptions, drawings, dimensions, marked prints, etc., as required for correction of the tracings to the as-built conditions. Final payment will not be made until as-builts are submitted and approved.

The RED LINED drawings shall be complete with any deviation in actual construction.

Within 30 days of work completion the Contractor shall submit 2- CD Copies of the project as-builts. Each CD shall contain one set of electronic as-builts in an AutoCAD 2014 .dwg format and pdf format.

INSTALLED EQUIPMENT (REAL PROPERTY)

The Contractor shall furnish, upon completion of the project, a typewritten list, in triplicate, of all equipment installed in the facility, and the installed cost of each item. Furthermore, the listing shall include the location of each item and nameplate data.

Typically, the listing shall include: air conditioners, air handling units, condensers, fans, pumps, air compressors, transformers, unit heaters, regulators, direct current power supplies, latrine fixtures, motors, engines, motor or engine-driven equipment, cranes, drinking fountains, sinks, water coolers, generators, space heaters, water heaters, refrigerators, freezers, coolers, meters, gas detectors, humidifiers dehumidifier, air purifier, ovens, power units, fuel tanks, water tanks, elevators, welders, recorders, reels, scales, hydrants, intrusion detection equipment, fire detector and alarm equipment, emergency light sets, emergency eye wash deluge showers, washers, dryers, dishwashers, bridge cranes, and like items of equipment.

Final payment will not be made to Contractor until the Contracting Officer has received and approved the listing. The Contractor shall use the DD Form 1354 provided by the Civil Engineering Real Property Records Branch at the pre-construction meeting.

OTHER CONTRACTS

A. The Contractor shall be responsible for the coordination of its work with any other Contractor in the area, making them aware of proposed work that may affect the work of their particular trade in process of performance.

CONTRACTOR QUALITY ASSURANCE AND WORKMANSHIP

The contractor will monitor quality control over suppliers, manufacturers, products, services, site conditions, and workmanship, to produce work of specified quality. Comply with specified standards as a minimum quality for the work except when more stringent tolerances, codes, or specified requirements indicate higher standards or more precise workmanship. Where a quality standard is not specified, the contractor shall provide a level of workmanship that meets a recognized industry standard. The Contracting Officer shall resolve conflicts in interpretations of quality of workmanship.

Comply fully with manufacturers' instructions, including each step in sequence. Should manufacturer's instructions conflict with Contract Documents, request clarification from the Contracting Officer before proceeding.

Perform work by persons qualified in the specified trade or specialty to ensure quality workmanship.

Secure products in place with positive anchorage devices designed and sized to withstand stresses, vibration, physical distortion or disfigurement.

BRAND NAME DISCLAIMER

The use of Brand Name is to set a standard of quality and is not intended to restrict the Contractor from offering an item from other sources of supply, as approved by the Contracting Officer. Pre-approval of products is “NOT” authorized

HAZARDOUS MATERIALS

A. The Contractor shall establish a Hazardous Material (HM) storage and distribution system when HM is to be used. All HM required to support the contract shall be reported to the Hazardous Material Pharmacy (HMP) using the Contractor HM Identification Form. The Contractor HM Identification Form will be provided to the Contractor at or prior to the Pre-Construction meeting. Additional HM needed by the Contractor shall be identified to the Contracting Officer’s Representative (COR) for approval by the HMP. Reference Attachment 18A.

B. The Contractor planning to use HM for the work shall register with the base HMP prior to start of work in order to support the installation’s compliance with Executive Order 12856, Federal Compliance with Right-to-Know Laws and Pollution Prevention Requirements.

C. The Contractor shall maintain Contractor HM Identification Form for HM on the job site for inspection/verification.

D. The COR will verify that the HM identified to HMP in use on the job site.

E. The Contractor shall be responsible for the following items: Provide a list of each material and quantity of material for all proposed Hazardous Material (HM). HM shall be construed to mean any item that is:

1. A health hazard or physical hazard as defined in 29 CFR, 1910.1200(c).

2. Regulated in its disposal by EPA under 40 CFR.

3. Hazardous as defined by DOT regulations under 49 CFR.

4. Hazardous as defined by the Dangerous Goods Regulation of the International Air Trans-port Association.

5. Provide a material safety data sheet (MSDS) for each item on the HM list.

F. The Contractor shall establish a construction specific HM storage and issue location that fully complies with Federal, State and Local environmental regulations. Materials issued shall be tracked for quantities used. Unused materials shall be inventoried and removed from the ANG installation prior to close out of the contract or expiration date of the HM. Reports of materials delivered, used and removed from the installation shall be submitted to the COR monthly and prior to contract close-out.

G. The Contractor shall comply with all Federal, State and Local environmental standards.

H. The Contractor shall accompany the Bio-environmental Engineering Representative (BEE) and the (COR) on project close-out inspection to ensure all used/unused HM is removed from the installation. This requirement shall not be a punch list item and must be accomplished prior to the Government accepting beneficial occupancy of the facility or construction item. Reference Attachment 18B.

SECTION 010010 - SECURITY

PART 1 - GENERAL

1.1. IDENTIFICATION OF EMPLOYEES:

A. The Contractor shall be responsible for obtaining identification for all employees. Employees without a badge and REAL ID ACT compliant identification in their possession will be denied access to the base and work areas. AF Form 75 will be issued to Contractors working for thirty (30) days or less. Contractors working for thirty (30) days to one (1) year will be issued an Identicard 2000 Contractor ID Card upon presentation of a properly completed AF Form 355 from the Contracting Officer or designated representative. Contractors working for more than one (1) year will be issued a base decal upon presentation of a properly completed AF Form 355 from the Contracting Officer or designated representative.

PART 2 - PRODUCTS

PART 3 - EXECUTION

3.1. CONDUCT OF WORK:

A. Coordination and Access to Site: Coordination with using agencies shall be made through the Contracting Officer or designated representative to assist the Contractor in completing the work with a minimum of interference and inconvenience.

B. General Area Requirements: Security requirements and procedures shall be coordinated with the Contracting Officer. Activities of the Contractor and the Contractor's employees and subcontractors and their employees, while on the base, will be in accordance with base regulations, including those of the Fire Marshall, as well as security directives. This includes, but is not limited to, giving way to all emergency vehicles that are in a response mode. All personnel performing duties as an escort official(s) for contractors within a restricted area are briefed thoroughly on control methods and actions to take to enforce control. Contractors will be briefed on security procedures and their responsibilities prior to the start of a contract. Contractors will be notified, as necessary, to changes in security procedures.

(1) Identification Credentials:

Starting October 10, 2015, visitors seeking access to military bases and almost all Federal facilities using their state-issued driver's licenses or identification cards must present proper identification issued by REAL ID compliant states or a state that has received an extension. When planning a visit to a Federal facility or military base, visitors should contact the facility to determine what identification will be accepted. State Enhanced Driver's Licenses (EDLs) designated as acceptable border-crossing documents by DHS under the Western Hemisphere Travel Initiative are acceptable for official federal purposes such as accessing a Federal facility or boarding a commercial aircraft. Individual agency policies may still apply.

Below is a list of alternate ID's that are acceptable.

a) U.S. Passport

b) U.S Passport Card

c) Permanent Resident Card/Alien Registration Receipt Card (Form I-551).

d) A foreign passport with a temporary (I-551) stamp or temporary (I-551) printed notation on a machine-readable immigrant visa.

e) An employment authorization document that contains a photograph (Form I- 766).

f) Identification card issued by Federal, State, or local government agencies, provided it contains a photograph and biographic information such as name, date of birth, gender, height, eye color, and address.

g) U.S. Coast Guard Merchant Mariner Cards/Credentials.

h) PIV or Federally-Issued PIV-1 Cards (Personal Identification Verification) issued by the Federal Government.

i) PIV-I card (Personal identification verification-Interoperable Issued by Non-Federal Government entities).

j) DHS "Trusted Traveler Cards" (Global entry, NEXUS, SENTRI, FAST).

k) Merchant Mariner card issued by DHS/ United States Coast Guard (USCG).

l) Border Crossing Card (Form DSP-150).

m) U.S. Certificate of Naturalization or Certificate of Citizenship (Form

n) N-550) and U.S. Permanent Resident Card (Form I-551).

o) U.S. Refugee travel document or other travel document or evidence of immigration status issued by DHS containing a photograph.

p) A Foreign Government Issued Passport.

If Montana does not receive another extension from Department of Homeland Security (DHS), starting January 1, 2020, a person with a Montana driver license or identification card who is visiting a military base or almost all federal facilities will be required to have an alternative form of acceptable identification to access the federal facility

(2) Commercial or company vehicles will be allowed access to the base provided company emblems are permanently attached to the sides of the vehicles. Magnetic signs are not acceptable. Personnel still require a badge or pass as identified in paragraph 1.

(3) Entry Authority List (EAL): EAL’s will be prepared by the unit or agency responsible for monitoring or administrating the contract. The EAL(s) must be signed by the preparers unit commander or agency chief, and forwarded to the Installation Commander or designated representative for final approval. Contractor vehicles must be listed on the EAL(s) including year, make, model, VIN and license number as stated on registration. The Contractor shall submit a list of personnel, including subcontractors, who will work within or adjacent to the restricted area(s). This list, which shall contain the name (Last, First and MI), address, Civilian Agency Affiliation, clearance information (if applicable), and social security number of each employee, no later than one (1) week prior to initial start of work. This list shall be submitted on stationary with the company's letterhead and be signed by a responsible member of that company. After the list has been submitted, it will be the Contractor’s responsibility to keep the list current. In addition to the 72-hour notification as noted below, not less than every two (2) weeks, the Contractor shall submit a complete up-to-date list of persons desiring access to restricted areas to include any additions or deletions made to the list during the preceding two (2) weeks. The information listed above must be provided for new employees whose names were not listed on the initial list at least 72-hours prior to the time said new employees need access to the restricted area. Employees who have terminated employment or who have been dismissed must be identified and removed as soon as possible from the entry authority list but no later than 24-hours after termination. Only those employees with base entry badges whose names are on the list will be permitted entry into the restricted area.

(4) Entry Procedures: Contractor personnel will be subject to personal search and/or frisks upon entering and leaving the Base.

(5) Contractor shall not stockpile equipment within 30 feet of interior/exterior of a perimeter fence.

(6) Existing lighting for security purposes must be functional at all times during the hours of darkness. Deficiencies in security lighting or power shall be repaired and replaced prior to the end of the workday.

(7) Motorized Equipment:

(a) Unattended vehicles shall be rendered immobile by removing the keys or by other suitable means.

(8) Fire Extinguishers: Motorized equipment operated shall be equipped with fire extinguishers as follows:

(a) Pickup truck or other light passenger vehicles: One extinguisher per vehicle, rating 5 BC.

(b) All other trucks and heavy motorized equipment: Two extinguishers per vehicle, rating 10 BC.
