About this file
This document is an Inspection and Maintenance Services Report for two aircraft fire training facilities (AFTF and STU) located at Columbus AFB, Mississippi, dated 2022-11-28. The inspection identified numerous compliance deficiencies and equipment failures requiring corrective action across both trainers and their associated support systems.
The AFTF requires approximately $76,309 in repairs across multiple critical systems. Major deficiencies include a non-compliant propane emergency shut-off system that does not function and exceeds safe operating parameters, with hydrostatic pressure relief valves beyond their 10-year service life (17 valves requiring replacement at $1,275). The Remtron remote emergency stop system is inoperable as the base station fails to isolate power when activated. Fuel distribution control systems have persistent leaks in coax valves at multiple training points, and the fuel control emergency stop system cannot isolate fuel in emergencies. Structural hazards include open doorways lacking fall protection (four doorways requiring stainless chain installation at $400 each), compromised troop ramp access with trip/fall hazards and large deformations creating entrapment risks, and damaged igniter shielding. The facility also requires a new control tower ship's ladder replacement ($12,500), lower weir drain valve and french drain system ($1,130), and fuel manifold system overhaul with pneumatic actuators and new stainless ball valves ($18,250). Additional repairs include oxidation mitigation on metal flooring ($5,415), maranite panel replacements ($5,200), and perferated stainless steel igniter protection screens ($450).
The Structural Trainer (STU) requires fewer repairs but is partially non-compliant with NFPA 1403 standards due to compromised burn cell thermal liners in two cells. Deficiencies include three damaged maranite panels in Burn Cell 1 and one panel in Burn Cell 2 requiring replacement ($5,200 for eight panels with benchstock), a broken exhaust fan belt requiring replacement ($60), and general oxidation on metal flooring. Both facilities require immediate attention to safety-critical systems including emergency stop functionality, propane system compliance, and fall protection mechanisms. The inspectors note that while these issues do not warrant facility lockout, prioritized repairs are necessary for full compliance with USAF Technical Order 35E-1-2-13-1, NFPA 58 (2024), NFPA 1403 (2018), NEC 70 (2023), and OSHA 29 CFR 1910 standards for firefighter live fire and recurring training.

Service
(2025-Q3) NON-COMPLIANT / HAZARDOUS due to multiple points of potential firefighter injury from falling (opening on main fuselage) or stepping into holes at the top of the Troop Ramp, and trip hazard at the bottom of the Troop Ramp coupled with Remote E-Stop System IN/OP, broken fuel system Emergency Stop assembly, and leaking Coax valves within the burn pit
SPECIAL NOTES:
I have reviewed the report and pictures in support of the Inspection report furnished, e-mail correspondence and conversations regarding the fire training facility construction, condition, and needed compliance repairs. Based on the information furnished, we have performed in a manner consistent with that degree of care and skill ordinarily exercised by members of the same profession currently practicing under similar circumstances at the same.
To the best of my knowledge, information and belief, the inspection revealed general conformance and safety issues which are subject to applicable standards. In my professional opinion the minor conformance and safety issues noted are NOT cause for a facility lockout. However I do believe the issues addressed in this report should be prioritized and repairs or upgrade be made as soon as possible for complete compliance.
ROOF (2025-Q4) Serviceable (2025-Q4) Serviceable (2025-Q4) Serviceable - Burn Cell 1 exhaust fan IN/OP due to fan belt damage
