About this file
This is an Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form for federal contract opportunities.
The form is a self-certification document that offerors must complete to demonstrate eligibility for contract awards under the Buy Indian Act (25 U.S.C. 47). The solicitation applies to the Western Oregon POD2 Server Room Upgrades project under solicitation number 75H70126Q00023. Offerors must certify their status as an "Indian Economic Enterprise" with "Indian Ownership" as defined in HHSAR 326.601 at three critical time periods: when the offer is submitted, at the time of contract award, and throughout the full contract performance period. Any changes in eligibility status during these periods require immediate written notification to the Contracting Officer. The form requires offerors to provide the name of the Federally Recognized Tribal Entity, the name and signature of the 51% owner of the Indian Economic Enterprise firm, business name, and DUNS number. Contracting Officers may request documentation of eligibility before award, and successful offerors must be registered with the System of Award Management (SAM). The form emphasizes that false or misleading information submitted during the offer or contract performance phases constitutes violations of federal law subject to penalties under 18 U.S.C. 1001, 1004, and 287, and 31 U.S.C. 3729-3731.
