Performance Work Statement (PWS) for the Hospitalist/Physician
| 1.0 | General |
| 1.1 | Introduction |
The mission of the Indian Health Service (IHS) is to raise the physical, mental, social, and spiritual health of American Indians and Alaska Natives to the highest level. The mission of the Pine Ridge Service Unit (PRSU), Great Plains Area, Indian Health Service, is to provide quality inpatient, outpatient, emergency, and related health care services to the Oglala Lakota Nation and other eligible beneficiaries.
This acquisition is for contracted Hospitalist/Physician services at Pine Ridge Hospital and associated Pine Ridge Service Unit facilities. These services are needed to support continuity of care, maintain inpatient and acute medical coverage, address high patient demand, supplement limited permanent staffing, and support critical access requirements. The purpose of this acquisition is to ensure timely, safe, responsive, and effective health care services for patients served by PRSU.
1.2 Objective
The objective of this contract is to obtain qualified, licensed, credentialed, and privileged Hospitalist/Physician services to support PRSU's ability to provide safe, timely, continuous, and high-quality inpatient and acute care services to eligible beneficiaries.
The Contractor shall furnish Hospitalist/Physician personnel capable of providing direct patient care, managing inpatient admissions, conducting daily rounds, completing required medical documentation, supporting referrals and transfers, and coordinating care with PRSU clinical staff. These services are intended to maintain continuous hospitalist coverage, address high patient demand, supplement limited permanent staffing, and support the delivery of acute medical care in a rural health care environment.
This requirement supports the mission of IHS and PRSU by helping ensure that American Indian and Alaska Native patients served by PRSU receive accessible, responsive, and clinically appropriate health care services. Contractor performance shall contribute to continuity of care, patient safety, timely treatment, proper documentation, and effective coordination of inpatient, emergency, outpatient, specialty, and referred care services.
1.3 Scope of Work
The scope of this requirement is to provide qualified Hospitalist/Physician services for the Pine Ridge Service Unit, with the primary place of performance at Pine Ridge Hospital in Pine Ridge, South Dakota, and support to associated PRSU facilities as needed. The requirement includes continuous hospitalist coverage to support inpatient acute care, patient admissions, daily medical management, clinical coordination, documentation, referrals, transfers, and consultation support within the provider's credentials and privileges.
The services are limited to non-personal, contracted Hospitalist/Physician services and are intended to supplement, not replace, Government clinical oversight and permanent staffing functions. Contractor personnel shall provide care consistent with PRSU policies, medical staff bylaws, credentialing and privileging requirements, applicable federal regulations, and accepted standards of medical practice.
This work supports PRSU's ability to maintain safe and effective acute care services in a rural health care environment with high patient demand, limited permanent staffing, and limited access to higher levels of care. The overall scope is focused on ensuring continuity of care, patient safety, timely clinical response, and appropriate coordination of services for eligible beneficiaries served by the Pine Ridge Service Unit.
1.4 Background
The Pine Ridge Service Unit (PRSU), Great Plains Area, Indian Health Service (IHS), provides inpatient, outpatient, and emergency care to the Oglala Lakota Nation. Due to high patient demand, limited permanent staffing, and critical access requirements, PRSU requires contracted Hospitalist/Physician services to ensure continuity of care and compliance with IHS mission standards.
Hospitalist/Physician services are extensively utilized throughout the Pine Ridge Reservation for nearly all aspects of acute health care. The Pine Ridge Reservation is located in the southwestern corner of South Dakota, approximately 100 miles from Rapid City. Surrounded by rolling prairie, the Badlands, and the Black Hills, the Pine Ridge Reservation is home to the Oglala Lakota Nation.
The people of the Pine Ridge Reservation experience significant health disparities, including low life expectancy and high infant mortality rates. Oglala Lakota County has consistently ranked among the poorest counties in the United States, and unemployment remains high in this population. Access to higher levels of care is limited. The closest higher level of care is approximately 35 minutes away by life flight and approximately 1 hour and 30 minutes by ground ambulance. Weather conditions can be challenging and may preclude air or ground transfer.
PRSU has historically relied on a combination of permanent Government providers and contracted medical professionals to meet patient care needs. Due to ongoing recruitment and retention challenges, rural location, and the need for continuous inpatient and acute care coverage, contracted Hospitalist/Physician services remain necessary to support safe and effective hospital operations. The future vision of this requirement is to maintain consistent, high-quality Hospitalist/Physician coverage while supporting continuity of care, patient safety, timely treatment, and compliance with PRSU, IHS, accreditation, and federal requirements.
1.5 Type of Contract Contemplated
The Government contemplates award of a firm-fixed-price, performance-based, non-personal services contract for Hospitalist/Physician services.
Under this contract type, the Contractor shall provide qualified, licensed, credentialed, and privileged Hospitalist/Physician personnel to perform the required services in accordance with the terms, conditions, performance standards, and schedule established in the contract. The Contractor is responsible for managing its personnel and ensuring adequate coverage to meet PRSU requirements.
Although the contract is firm-fixed-price, the requirement includes defined labor categories, shift coverage, and minimum staffing levels necessary to ensure continuity of patient care. The Contractor may provide additional personnel as needed to meet workload demands; however, staffing shall not fall below the minimum coverage requirements established in the contract.
All services shall be performed as non-personal services. Contractor personnel are not Government employees and shall not be supervised as Government employees. The Contractor retains responsibility for employee management, performance, conduct, timekeeping, leave coverage, and replacement of personnel when required.
1.6 Period of Performance
The period of performance for this contract shall consist of a base 3 month period.
The Government reserves the right to terminate the contract for convenience if PRSU hires a sufficient number of permanent Government providers to fulfill this requirement. In such cases, the Contracting Officer shall provide the Contractor with a minimum of sixty calendar days' written notice prior to termination.
1.7 Place of Performance
The place of performance for this contract shall be the Pine Ridge Service Unit (PRSU), Great Plains Area, Indian Health Service, located in Pine Ridge, South Dakota.
The Contractor's primary duty station shall be Pine Ridge Hospital, 607 Indian Health Road, P.O. Box 1201, Pine Ridge, SD 57770.
Services shall be performed on site at Pine Ridge Hospital and, as required, at associated PRSU facilities, including outlying clinics and mobile unit operations. The Pine Ridge Service Unit consists of one hospital, four outlying clinics, and one mobile unit.
Because the requirement involves direct patient care, inpatient acute care coverage, clinical documentation, coordination with PRSU staff, and use of PRSU health information systems, performance shall primarily occur at Government facilities. Contractor personnel shall report for duty as scheduled unless the facility is closed by authorized Government leadership.
Pine Ridge, South Dakota, experiences a wide range of weather conditions, including extremely cold and windy winters. Contractors shall be prepared to travel and perform duties under adverse weather conditions. The authority to close Pine Ridge Hospital or its outlying clinics rests solely with the Chief Executive Officer. If the facility remains open, Contractor personnel shall report for duty as scheduled. The Contractor is responsible for maintaining reliable transportation and associated costs necessary to ensure timely reporting for duty in accordance with this contract.
Contractor personnel shall comply with all PRSU access, identification, credentialing, privileging, security, employee health, infection control, and facility policies while performing services at Government locations.
1.8 Applicable Documents
The Contractor shall comply with the most current versions of the following documents, regulations, standards, and policies, as applicable to the performance of Hospitalist/Physician services under this contract. These documents are applicable only to the extent they directly relate to the services required by this PWS.
Applicable documents include the Federal Acquisition Regulation (FAR), Health and Human Services Acquisition Regulation (HHSAR), Public Health Service Acquisition Regulation (PHSAR), Indian Health Manual (IHM), Centers for Medicare & Medicaid Services Conditions of Participation for Hospitals, applicable The Joint Commission standards, PRSU Medical Staff Bylaws, PRSU credentialing and privileging policies, PRSU Health Information Management and Electronic Health Record policies, PRSU infection control and employee health policies, PRSU occupational safety and personal protective equipment policies, PRSU Pharmacy and Therapeutics Committee policies and formulary, PRSU Purchased/Referred Care policies and procedures, the Privacy Act, HIPAA, HSPD-12 requirements, and applicable federal background investigation and Personal Identity Verification requirements.
In the event of a conflict between applicable documents, federal law and regulation shall take precedence, followed by HHS/IHS policy, PRSU policies and procedures, medical staff bylaws, and local operating procedures, unless otherwise directed in writing by the Contracting Officer.
Only documents and policies directly related to the Contractor's performance of Hospitalist/Physician services, patient care, credentialing, security, documentation, privacy, and facility access shall be considered mandatory for contract compliance.

The Government may provide necessary clinical and task-related coordination to Contractor personnel to ensure patient safety and operational efficiency. Such coordination does not alter the non-personal services nature of this contract.
The Contractor is fully responsible for the performance and conduct of its personnel. This includes completion and accuracy of timekeeping; approving and providing coverage for leave requests; addressing quality of work issues and performance concerns; maintaining motivation, morale, and responsiveness of staff; and ensuring continuity of services in the absence of on-site contractor supervisors.

The Contractor shall monitor its own performance and take timely corrective action when deficiencies are identified. The Contractor's quality assurance activities do not replace the Government's right to inspect, review, monitor, or accept services in accordance with the contract.

The Contractor shall ensure all Contractor personnel meet licensing, credentialing, privileging, security, employee health, and training requirements before performing services. The Contractor shall maintain continuous hospitalist coverage in accordance with the contract schedule and staffing requirements.
