Statement of Work Summary
This Statement of Work (SOW) details the ADA Door Opener Projects for Kayenta Health Center in Kayenta, Arizona, dated December 10, 2025, under the Kayenta Service Unit. The project involves installation and modification of twenty-seven ADA door opener locations throughout the health facility, which serves approximately 20,000 people in a remote area and provides ~200 patient visits daily with 24/7 emergency room services.
The contractor must prepare a comprehensive proposal with an accurate project timeline and perform all work to completion, including modifications and replacements while maintaining fire/smoke door ratings (NFPA compliance), furnishing UL-certified hardware, removing existing door closures, preparing doors for new ADA openers, ensuring existing electronic access systems remain functional, and installing new ADA door openers with two push buttons per location. Installation locations include single and double doors across multiple departments (G-Wing, Physical Therapy, ED, Radiology, Pharmacy, Patient Registration, Outpatient, Conference, Administration, Human Resources, and Inpatient areas). The contractor must maintain existing door ratings and specifications per FGI (Federal Guideline Institute) standards, use well-known reliable door opener brands, test and certify all openers with electronic access control systems, and dispose of debris at EPA-approved locations. Work must comply with NFPA criteria, The Joint Commission standards, and safety protocols, conducted Monday through Friday, 8:00 AM to 5:00 PM, excluding federal holidays, with coordination through facility management and security. The contractor provides a one-year workmanship warranty and manufacturer warranties with finish certificates. KSU Maintenance will provide 120-volt power installation, and the contractor must obtain facility ID badges, minimize disruption to patient care operations, maintain a clean work area, and submit equipment submittals prior to installation.
