About this file
This is a Statement of Work (SOW) for the replacement and recommissioning of a Digital Master Controller (DMC) Programmable Logic Controller (PLC) system at Kayenta Health Care Center in Kayenta, Arizona. The period of performance extends from the award date through July 31, 2026.
The contractor must replace the existing faulty DMC PLC that is experiencing an HMI/PLC communication failure affecting critical power generation and control systems. The scope of work includes: coordinating project scheduling with facility representatives and OEM support (Cummins or approved equivalent); performing pre-installation site walkdown and documentation review; safely de-energizing the system using lockout/tagout procedures; removing the existing PLC; providing and installing an OEM-approved replacement PLC compatible with existing systems; verifying power supply, grounding, and I/O terminations; providing all required OEM firmware, software, programming tools, and licenses (contractor-furnished unless explicitly stated otherwise); configuring communication parameters; integrating the PLC with existing HMIs, alarms, sensors, and field devices; performing point-to-point I/O verification; conducting functional testing and system commissioning including alarm verification and communication loss/recovery testing; and coordinating required system outages with minimum 10 business days advance notice and written Government approval.
The contractor must comply with manufacturer requirements, NFPA 70, NFPA 110, OSHA 29 CFR 1910, and facility safety and access requirements. All work shall be performed by qualified technicians experienced in generator control systems and PLC programming. Deliverables include PLC configuration files in native OEM format, commissioning and functional test reports documenting procedures and results, and updated system documentation. Acceptance is based on successful documented functional testing witnessed by the Contracting Officer's Representative (COR), restored HMI/PLC communications, verified alarm functionality, and fault-free operation for a minimum continuous run period of 2 hours. The contractor provides one year workmanship warranty and equipment warranty per manufacturer standards, and guarantees proper system functionality and restored communications upon final acceptance.

STATEMENT OF WORK
TO: DMC PLC REPLACEMENT AND RECOMMISSIONING
AT: KAYENTA HEALTH CARE CENTER KAYENTA, ARIZONA
DATE: DECEMBER 30, 2025
PERIOD OF PERFORMANCE: AWARD DATE – JULY 31, 2026
INTRODUCTION
Kayenta Health Care Center (KHC) operates critical power generation and control systems necessary to support continuous healthcare operations. The Digital Master Controller (DMC) system is a critical component used to monitor and control generator operations. The DMC system is currently experiencing an HMI/PLC Communication Failure, resulting in loss of reliable system monitoring and control. This Statement of Work (SOW) defines the requirements for replacement of the existing PLC and recommissioning of the DMC system to restore full operational functionality.
BACKGROUND
Kayenta Health Care Center provides comprehensive inpatient, outpatient, and emergency healthcare services. Reliable power generation and control systems are essential to patient safety and facility operations. The existing DMC PLC has experienced a system fault that cannot be resolved through troubleshooting alone and requires replacement. The work described herein shall be performed by qualified technicians experienced in generator control systems and OEM-supported PLC platforms.
OBJECTIVE
The objective of this project is to replace the existing DMC PLC, restore HMI/PLC communications, and return the DMC system to normal, reliable operation in accordance with manufacturer and facility requirements. Successful completion shall be based on documented functional testing and COR acceptance as defined herein.
PERFORMANCE DESCRIPTION
The Contractor shall furnish all labor, materials, equipment, tools, supervision, transportation, and coordination necessary to replace the DMC PLC and recommission the system. All work shall be coordinated with Facility representatives and OEM support (Cummins or approved equivalent) to ensure proper programming, validation, and operational integrity of the DMC system. Work shall be performed in a manner that minimizes impact to facility operations and maintains system safety.
SCOPE OF WORK
1. Coordinate project scheduling with the Facility Representative and OEM support.
2. Perform a pre-installation site walk-down and review existing DMC drawings and documentation.
3. Safely de-energize and place the DMC system in a controlled condition prior to work in accordance with approved lockout/tagout procedures.
4. Remove the existing faulty PLC and associated components as required.
5. Provide and install a new OEM-approved PLC compatible with the existing DMC system.
6. Verify proper power supply, grounding, and I/O terminations.
7. Provide all OEM-required firmware, software, programming tools, and licenses necessary to complete PLC programming and commissioning. No OEM software or licenses shall be assumed to be Government-furnished unless explicitly stated.
8. Load OEM-approved firmware and application programming.
9. Configure communication parameters to restore HMI/PLC communications.
10. Integrate the PLC with existing HMIs, alarms, sensors, and field devices.
11. Perform point-to-point I/O verification.
12. Conduct functional testing and system commissioning, including alarm verification and communication loss/recovery testing.
13. Coordinate all required system outages with the Facility Representative a minimum of 10 business days in advance. No outages impacting critical power systems shall occur without written Government approval.
14. Verify alarm functionality and confirm resolution of all communication faults.
COMPLIANCE REQUIREMENTS
All work shall comply with the following, as applicable:
• Manufacturer/OEM requirements (Cummins DMC systems)
• NFPA 70 (National Electrical Code)
• NFPA 110 – Emergency and Standby Power Systems
• OSHA 29 CFR 1910 – General Industry Standards
• Facility safety, infection control, and access requirements
CONTRACTOR QUALIFICATIONS
