About this file
This document is an Indian Health Service (IHS) Buy Indian Act Indian Economic Enterprise Representation Form. The form requires offerors to self-certify whether they meet the definition of an "Indian Economic Enterprise" with Indian Ownership when responding to a solicitation, sources sought notice, or request for information. The enterprise must maintain this status at the time of offer, contract award, and throughout the entire contract performance period.
Key requirements include: immediate written notification to the Contracting Officer if eligibility changes, potential documentation requirements before award, mandatory registration in the System of Award Management (SAM), and awareness that false or misleading information is punishable under 18 U.S.C. 1001 and 31 U.S.C. 3729-3731. The form provides spaces for the offeror to indicate their status, list the name of the 51% owner, certifying signature, federally recognized tribal entity, business name, and DUNS number.
