About this file

The form is a self-certification document required for solicitation 75H70526Q00021 titled "UNIPOC POINT OF CARE MANAGEMENT SYSTEM," dated 07/28/2026.

The form establishes that offerors must certify whether they meet the definition of an "Indian Economic Enterprise" with Indian ownership under the authority of the Buy Indian Act (25 U.S.C. 47). Offerors must maintain eligibility status at three critical junctures: at the time an offer is submitted, at the time of contract award, and throughout the full term of contract performance. Any contractor that no longer meets eligibility requirements during the contract period must provide immediate written notification to the Contracting Officer. The form requires offerors to provide the name of the federally recognized tribal entity, the 51% owner of the IEE firm, business name, UEI number, and a certifying signature. Successful offerors must also register with the System of Award Management (SAM). The form warns that false or misleading information constitutes a violation under 18 U.S.C. 1001, and false claims during contract performance are subject to penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287. Individual Contracting Officers may require documentation of eligibility before making awards, which are subject to protest if the offeror's eligibility is questioned.
