BID CHECKLIST FOR ALL CONTRACTORS- HELPFUL TIPS:

Upon receipt of bids and after conducting the bid opening we are required by FAR 14 to review each bid for MINOR INFORMALITIES and MISTAKES. A Minor informality is one that is merely a matter of form, not of substance, and must be corrected. Award cannot be made unless these minor informalities are corrected prior to final award review, thus review of your bid before it is submitted is beneficial to all parties.

Please review your bid/documents carefully to verify conformance. I am providing a list of the most common informalities that contractors are asked to correct before final determination can be made for contract award:

ALL SUBMISSION DOCUMENTS MUST CONTAIN THE EXACT NAME OF YOUR COMPANY AND ANY “DBA” NAME REGISTERED WITH YOUR STATE IF YOU’VE INCLUDED IT ON THE SAM. Please do not shorten the title of your company or alter it in anyway on your submitted documents. The following documents must all match:

1. SF-1442 – Page 2

2. System for Award Management print out (obtained by me)

3. Bid Bond

I will access and print your SAM immediately upon opening your bid. If the company names and DBA’s do not match you will be contacted to correct ALL OF THEM to reflect the information, thus delaying the award.

1. SF-1442 – Solicitation/Offer/Award & Solicitation Document:

Page 2 - Ensure you enter your total price in block 17, and acknowledge all amendments. If you have a DBA listed on SAM, ensure it’s listed with the name of your company. Submit signed SF-30's (AMENDMENTS) with your bid.

Page 4 - Ensure you enter your total price in both Unit Price and Amount.

Page 26- Do not complete anything in Section (b)(2)- neither needs to be checked, as the provision it references IS already included in the solicitation. DO NOT MARK THIS BLOCK.

Page 28 – Make sure you COMPLETE AND SUBMIT.

2. System for Award Management (SAM):

***MAKE SURE YOU ARE REGISTERED IN SAM, AND THAT ALL INFORMATION IS CURRENT!***

Your firm must be registered in the System for Award Management (SAM) and have completed the Online Representations and Certifications in the SAM to receive a contract award from the Federal Government. Ensure your firm has the North American Industry Classification System (NAICS) Code listed that is appropriate for this procurement. The NAICS code for this solicitation is 238220.

- For SAM, ensure you have 238220 listed and ensure, as this procurement is a small business set aside, that you are certified as a small business

3. BID BOND:

YOU WILL BE REQUIRED TO OBTAIN A CORRECTED BID BOND IF THE DOCUMENT is NOT CORRECT. Your Bid Bond Address for your Surety MUST MATCH the Treasury Circular 570 found at http://www.fms.treas.gov/c570/c570_a-z.html. Please do not submit your bond with the local office that is directly responsible for the contractor submitting the bid. Your bond will be required to be re-issued if the addresses in both the Surety Box at the top of the form, and the Surety A Block at the bottom of the form do not match.

Verify before sending your bond - it MUST contain:

- the correct bid opening dated

- the Surety Corporation Liability Limit

- the raised seal for the Principal and the Bonding Company

- the solicitation number

- the title of the solicitation

- the penal sum of the bond (minimum of 20%)

- the required powers of attorney http://www.fms.treas.gov/c570/c570_a-z.html http://www.fms.treas.gov/c570/c570_a-z.html
