About this file
This is an Invitation for Bid (IFB) offer form for the State of Illinois. The document serves as the standard bidding template that vendors must complete and submit when responding to state solicitations. The form requires bidders to provide their offer in full compliance with the subject solicitation requirements outlined in the referenced solicitation document section A.1. Bidders are instructed to review all required documents and understand that failure to meet all requirements constitutes grounds for disqualification. The form includes checkboxes and text fields for bidders to complete their submission details and requires an authorized representative signature and date to validate the bid submission.
The form addresses preferences available under the Illinois Procurement Code, allowing bidders to claim applicable preferences such as the Illinois Business Preference or Domestic Product Preference, with corresponding documentation requirements for each claim type. Additionally, the form permits bidders to take exceptions to standard contract terms and conditions by submitting a redline version for state review and determination of acceptance or rejection. The Standard Terms and Conditions referenced in the solicitation are available on the Illinois Chief Procurement Office website. All bidders must certify their understanding that the standard terms and conditions will become part of any resulting contract, and submission of the completed form constitutes a binding bid offer to the State of Illinois.
