b. Guidelines for the National Institute of Occupational Safety and Health (NIOSH) approved testing may be reviewed at American College of Occupational and Environmental Medicine website: Guidance Statements | ACOEM
c. Testing to determine forced vital capacity (FVC) and forced expiratory volume at one second (FEV1) must be conducted according to the American Thoracic Society standards. This requires a volume-time curve showing a minimum of three acceptable FVC maneuvers. Measurements must be made of the FVC and the FEV1. Calculate and express the FEV1/FVC ratio as a percentage.
Record the largest FVC and the largest FEV1.
d. Spirometry shall be conducted by an appropriate trained staff using a spirometer that meet the American Thoracic Society’s specifications. Spirometers must be cleaned and maintained according to the manufacturer’s specifications and calibrated as recommended by the American Thoracic Society.
4. EXERCISE TOLERENCE TEST (If Applicable)
a. The National Guidelines recommend subject be able to reach a minimum of 10 METS on exercise testing without developing ischemia or cardiac arrhythmia.
b. It is expected that standard Bruce Protocol or equivalent will be performed and 10 METS will be reached late in the third stage.
c. It is the responsibility of the Medical Surveillance Program director to determine if treadmill testing is indicated for a particular employee.
5. STRESS ECHOCARDIOGRAM (If Applicable)
a. Patients who have positive or questionable exercise tolerance test results may be asked to undergo a stress echocardiogram. The stress echocardiogram is a non-invasive procedure in which the subject exercises while undergoing an electrocardiogram. At the end of the exercise period the subject also receives an echocardiogram, which provides an image of the heart using techniques similar to sonar used in submarines.
6. LABORATORY TESTS:
Note: Laboratory testing is obtained to provide baseline information for comparison purposes in case the subject has a subsequent exposure to chemicals. The laboratory testing focuses on indicators of function for the kidneys, liver, and bone-marrow (where blood cells are formed). These are chosen because chemical exposures are likely to affect these organs. If a subject has chemical exposure, it is reassuring to see that these laboratory tests remain in the normal range. If they are abnormal following exposure, it is helpful to see if they were normal at baseline (which implies that the abnormality is new and may be due to the exposure) or if they were abnormal already at baseline (implying that the patient may have apre-existing abnormality not related to the exposure.

MD CONSULTATION SERVICES:
Hourly consultation from a physician for questions and projects in environmental health and safety, industrial hygiene, and toxicology.
Additional Services (If Applicable Due to Exposure Risk)
8. Blood Lead Test Note that all tested employees have blood lead levels below the applicable industrial standard, as described in CCR Title 8, Section 5216. Based on these levels, they may work in areas with lead containing materials. However, please note that medical removal may be required in cases where employees are at special increased risk from lead (such as pregnancy, anemia, past history of lead poisoning, or kidney disease), even though the employee’s blood lead level is below industrial standards.
Also note that the most important aspect in preventing occupational lead poisoning is to maintain low lead levels in the working environment. This may be achieved by having the company industrial hygienist regularly review areas and processes where lead is used and monitor exposure levels.
9. Cholinesterase On the basis of our evaluation, the following measures should be taken:
• Group I levels are normal, approved to work around pesticides
• Group II levels are decreased from baseline. Repeat testing is needed.
• Group III levels are decreased from baseline. Repeat testing is needed and employees may not work around pesticides until levels have normalized.
10. Hepatitis B Surface Ab levels A HBsAb (hepatitis B surface antibody) level of > or = 10 indicates adequate immunity to hepatitis B. If HBsAb level is <10 (inadequate), the 3 vaccine series should be given and the antibody level checked six weeks after the third vaccine.
11. Rabies Antibody Level A rabies antibody titer of > 0.1 IU/mL (0.1 IU/mL = 1:5 dilution) is considered an adequate level of protection. An antibody level of < 0.1 IU/mL indicates inadequate protection against rabies. Test antibody levels at six weeks after third vaccine. If the three vaccine series has been completed and the antibody level is <
0.1 IU/mL, one booster is required. Test six weeks after the booster vaccine. If the series was interrupted, repeat the series of three and test six weeks after the third vaccine.
12. TB Screening A negative TB test indicates that no TB infection is present. A positive TB test may indicate current or past TB infection. A chest x-ray is required for any positive TB skin test. A test with an indeterminate result should be repeated.
Our program follows the Cal-OSHA Protection standard, Title 8, California Code of Regulations (CCR), Section 5144, and the Department of Pesticide Regulation
Respiratory Protection Standard, Title 3, CCR, Section 6739 and must be compliant with the regulations.
