IFB 26-040 Summary: Interoperability Platform for New York State Office of Mental Health

This Invitation for Bid (IFB) document represents a procurement by the New York State Office of Mental Health (OMH) for an interoperability platform modernization project. OMH seeks to establish standardized HL7 FHIR protocols and deploy the PSYCKES clinical data system onto a new interoperability platform with integrated data exchange capabilities. The scope includes implementing a custom HL7 FHIR API to expose PSYCKES clinical summary data using custom profiles based on HL7 U.S. Core and USCDI standards; ingesting FHIR and non-FHIR data from external providers through multiple formats (FHIR resources, CCDA documents, NDJSON files, HL7 V2 messages, and CSV files); implementing patient matching using a Master Data Management solution; exchanging CDA resources; implementing Payer-to-Payer clinical record exchange; deploying Clinical Decision Support services; implementing EasyShare SMART Health Links specifications; creating an administrative website for SMART on FHIR application access management; and enabling secure OAuth 2.0 client registration. The Notice of Intent to Bid deadline is August 7, 2026, at 3:00 PM EST; questions must be submitted by August 14, 2026; and proposals are due August 28, 2026, at 3:00 PM EST. Contract award is anticipated for September 15, 2026, with a three-year term running through September 14, 2029. The current production environment serves data for over 20 million clients across 1,700+ facilities, processes approximately 5,000 CDS Hooks requests per minute at sub-one-second response times, and handles batch loads exceeding 1TB of data, requiring any successful bidder to maintain this scale and performance level without disruption to existing operations.

The contract will be awarded to the responsible and responsive bidder offering the lowest total bid amount while meeting all mandatory requirements. Bidders must submit pricing for three years of interoperability costs, standard support (225 hours annually at specified hourly rates), premium support (150 hours annually), and implementation costs for reaching full production readiness if not currently available at required scale. No escalation is permitted; all prices are fixed for the contract term. There is no reimbursement for travel expenses. The incumbent vendor is Smile CDR, Inc. MWBE participation goals are established at 0% for MBE participation and 0% for WBE participation based on current availability of qualified certified businesses; however, contractors must demonstrate good faith efforts toward achieving these goals. SDVOB participation goals are also 0%, though bidders are encouraged to make good faith efforts to utilize certified Service-Disabled Veteran-Owned Businesses. Mandatory bid submission requirements include proof of at least three years in business performing required services, New York State Workers' Compensation and Disability Benefits Insurance certification, evidence of vendor registration, completed tax certifications (Form ST-220-CA and ST-220-TD), non-collusive bidding certification, business references, vendor responsibility questionnaires, EEO staffing plans, MWBE utilization plans, and various state-required certifications regarding procurement lobbying compliance, sexual harassment prevention, gender-based violence policies, and Iran Divestment Act compliance. Electronic bids must be submitted to bids@omh.ny.gov and will not be accepted by facsimile or hard copy. OMH reserves the right to evaluate and reject all bids, negotiate costs with the selected bidder, and conduct further contract negotiations with the next responsible bidder if initial negotiations are unsuccessful.

1. Scope of Work:

1.1. Purpose

New York State Office of Mental Health (OMH) aims to improve service delivery in New York by making data more accessible, generating new data, and replacing outdated applications with integrated, user-friendly, and accessible systems. To support this, OMH will establish standardized HL7 protocols, including deploying PSYCKES data onto a new interoperability platform, connecting PSYCKES with interoperability solutions, and implementing a FHIR® API for clinical summaries.

Specific clinical data sections will be converted to FHIR®, such as demographic info, flags, care coordination, medications, diagnoses, procedures, encounters, lab services, questionnaire responses, and summaries. Additional steps include developing data ingestion processes from external providers, deploying patient matching, facilitating secure client application registration via OAuth 2.0, ensuring FHIR® standards compliance, and validating data efficiently. These initiatives aim to modernize OMH’s data collection, sharing, and interoperability, enabling prompt responses and efficient data management, and transforming the output into HL7 FHIR® data to support clinical decision making, care coordination, and quality improvement throughout New York State’s mental health system.

1.2. Requirement Specification

I. Implement an HL7 FHIR API to expose the PSYCKES Clinical Summary data using custom profiles. This would result in a custom Implementation Guide with HL7 U.S. Core and USCDI as the basis to support the PSYCKES use case. The current clinical summary resides in an Oracle database and lacks HL7 compliance features to promote interoperability.

Below are the sections in the PSYCKES Clinical Summary to be transformed to FHIR:

Patient Resource (Demographic & General Section)

Flag Resource (Active Quality Flag section, General Insurance summary)

Basic Observation Resource (Current Care Coordination, Notification, Active Medicaid Restrictions, Alerts & Incidents)

Condition Resource (Diagnosis)

Procedure Resource (Procedure)

Questionnaire Response Resource (Safety Plans)

Composition Resource (PSYCKES Clinical Summary)

II. Implement a process to ingest FHIR and non-FHIR data from external providers

a. FHIR resources from external partners; CCDA document, FHIR data bulk import;

NDJSON files

b. HL7 V2 message (optional)

c. .CSV files and other structured non-FHIR data formats

III. Implement the patient matching process using an MDM solution

IV. Ability to exchange CDA resources and Ingest HL7 v2.x messages using the same software

V. Implement the Payer-to-Payer (P2P) exchange solution that requires the exchange of clinical records at a member's request.

VI. Implement Clinical Decision Support service and Live Bundle

VII. Implements the EasyShare SMART Health Links (SHL) specifications

VIII. Implement an internal website for platform administrators to approve, suspend or revoke access to SMART on FHIR applications.

IX. Allow secure registration of the client application with the FHIR system

a. Allow end systems to register their API client with the OAuth 2.0 Authorization Server following the standards defined in the SMART Backend Service Launch specification, SMART on FHIR Provider and Patient Launch specification, SMART CDS Hooks specification

X. Enable data exchange using FHIR-R4 and future standards of the FHIR Specification

XI. Enable the PSYCKES-Interop FHIR Server to validate all input requests and the output HL7 FHIR data against HL7 specification using a terminology server and must support advanced validation, translation, and mapping capabilities

16. Consultant Disclosure Legislation:

In 2006 the NYS State Finance Law was amended to require State contractors who provide consulting services to disclose, by employment category, the number of persons employed to provide services under a contract for consulting services, the number of hours worked and the amount paid to the contractor by the State as compensation for work performed by these employees. This will include information on any persons working under any subcontracts with the State contractor. Under this law consulting services contracts have been defined as any contract entered into by a State Agency for analysis, evaluation, research, training, data processing, computer programming, engineering, environmental health, and mental health services, accounting, auditing, paralegal, legal, or similar services.

In order to comply with this law the Bidder must complete the attached Attachment I: Consulting Form A.

A. Consulting Form A – If applicable, this form will need to be submitted by all Bidders of this procurement and shall be included in the bid packet. The purpose this form is to capture the necessary planned employment information prospectively from the start date of the contract through the end of the contract term.

17. Bid Confidentiality:

All bids submitted for OMH’s consideration will be held in confidence. However, the resulting contract is subject to New York State Freedom of Information Law (FOIL). Therefore, if a Bidder believes that any information in its bid constitutes a trade secret or should otherwise be treated as confidential and wishes such information not to be disclosed if requested, pursuant to FOIL. (Article 6 of the Public Officer’s Law), the Bidder shall submit with its bid a separate letter specifically identifying the page number(s), line(s), or other appropriate designation(s) containing such information, explaining in detail why such information is a trade secret and formally requesting that such information be kept confidential. Failure by a Bidder to submit such a letter with its bid identifying trade secrets will constitute a waiver by the Bidder of any rights it may have under Section 89(5) of the Public Officers’ Law relating to protection of trade secrets. The proprietary nature of the information designated confidential by the Bidder may be subject to disclosure if ordered by a court of competent jurisdiction. A request that an entire bid be kept confidential is not advisable since a bid cannot reasonably consist of all data subject to FOIL proprietary status.

18. Contract Payment:

All contractors are required to participate in the Electronic Payment program offered by the NYS Office of the State Comptroller (OSC). Payment for invoices submitted by contractors will only be rendered electronically unless payment by paper check has been expressly authorized by OMH’s Office of Financial Management, at OMH’s sole discretion, due to extenuating circumstances. Electronic payments will be made in accordance with ordinary State procedures and practices. Contractors shall comply with the State Comptroller’s procedures to authorize electronic payments. For additional information and to apply for Electronic Payments, the CONTRACTOR is directed to the following web site:

http://www.osc.state.ny.us/epay/index.htm.

19. Contract Specifications

The OMH contract boilerplate is included with this IFB and contains additional specifications. All terms contained therein apply to this solicitation. The Bidder selected for award under this solicitation is expected to enter into a contract substantially in accordance with the form, terms, and conditions set forth in the attached boilerplate.

20. Extraneous Terms

Bids shall conform to the terms set forth in this solicitation and associated OMH contract boilerplate.

Material deviations may render the bid nonresponsive and may result in the rejection of the bid. Extraneous terms proposed by a Bidder for consideration shall be submitted using the format and process set forth in this solicitation. Any Bidder submissions on standard, pre-printed forms, such as, but not limited to, product literature, order forms, license agreements, contracts, or other documents that are attached or referenced with submissions shall not be considered part of the bid or resulting Contract but shall be deemed included for informational or promotional purposes only. Only extraneous terms accepted by the OMH, in writing, shall be expressly incorporated into the Contract. Acceptance and/or processing of a Bid shall not constitute acceptance of extraneous terms. OMH cannot entertain any exceptions to Appendix A, Standard Clauses for New York State Contracts.

21. Debriefing:

A debriefing is available to any entity that submitted a proposal or bid in response to a solicitation. A Bidder will be accorded fair and equal treatment with respect to its opportunity for debriefing. A Debriefing must be requested in writing by the entity within fifteen (15) calendar days of OMH notifying of Bidder Selection/Non-Selection. A Bidder’s written request for a debriefing shall be submitted to the designated contact indicated in Section 2. The debriefing shall occur within ten (10) days of OMH’s receipt of this request or as soon after that time as practicable under the circumstances.

22. Protest Procedure:

A Bidder wishing to challenge the selection of a firm for contract award must send a Notice of Protest on business letterhead, within seven (7) business days of notice of a contract being awarded, to the OMH designated contact in Section 2 of this IFB. OMH may summarily deny a protest that fails to contain specific factual or legal allegations. Upon receipt of OMH’s Protest Determination, the protesting party may file an appeal with the Office of the State Comptroller (OSC). The process for filing such an appeal is set forth at:

23. Iran Divestment Act:

As a result of the Iran Divestment Act of 2012 (Act), Chapter 1 of the 2012 Laws of New York, a new provision has been added to the State Finance Law (SFL), § 165-a, effective April 12, 2012. By submitting a bid in response to this solicitation or by assuming the responsibility of a Contract awarded hereunder, Bidder/Contractor (or any assignee) certifies that it is not on the "Entities Determined To Be Non- Responsive Bidders/Offerors Pursuant to The New York State Iran Divestment Act of 2012" list ("Prohibited Entities List") posted on the OGS website at: http://www.ogs.ny.gov/about/regs/docs/ListofEntities.pdf and further certifies that it will not utilize on such Contract any subcontractor that is identified on the Prohibited Entities List.

Additionally, Bidder/Contractor is advised that should it seek to renew or extend a Contract awarded in response to the solicitation, it must provide the same certification at the time the Contract is renewed or extended. By submitting a bid in response to this solicitation or by assuming the responsibility of a Contract awarded hereunder, Bidder/Contractor (or any assignee) certifies that once the prohibited entities list is posted on the OGS website, it will not utilize on such Contract any subcontractor that is identified on the prohibited entities list.

During the term of the Contract, should OMH receive information that a person (as defined in State Finance Law §165-a) is in violation of the above-referenced certifications, OMH will review such information and offer the person an opportunity to respond. If the person fails to demonstrate that it has ceased its engagement in the investment activity which is in violation of the Act within 90 days after the determination of such violation, then OMH shall take such action as may be appropriate and provided for by law, rule, or contract, including, but not limited to, seeking compliance, recovering damages, or declaring the Contractor in default.

OMH reserves the right to reject any bid, request for assignment, renewal or extension for an entity that appears on the Prohibited Entities List prior to the award, assignment, renewal or extension of a contract, and to pursue a responsibility review with respect to any entity that is awarded a contract and appears on the Prohibited Entities list after contract award.

CONTRACTOR REQUIREMENTS AND PROCEDURES FOR BUSINESS PARTICIPATION OPPORTUNITIES FOR

VENDORS CERTIFIED PURSUANT TO ARTICLE 15-A AND 17-E

NEW YORK STATE LAW

Pursuant to New York State Executive Law Article 15-A and 5 NYCRR 140-145 OMH recognizes its obligation under the law to promote opportunities for maximum feasible participation of certified minority-and women-owned business enterprises and the employment of minority group members and women in the performance of OMH contracts.

In 2006, the State of New York commissioned a disparity study to evaluate whether minority and women-owned business enterprises had a full and fair opportunity to participate in state contracting. The findings of the study were published on April 29, 2010, under the title "The State of Minority and Women-Owned Business Enterprises:

Evidence from New York" (“Disparity Study”). The report found evidence of statistically significant disparities between the level of participation of minority-and women-owned business enterprises in state procurement contracting versus the number of minority-and women-owned business enterprises that were ready, willing and able to participate in state procurements.
