About this file

This is an Employer Certification document for the Wage Theft Ordinance Responsibility Criteria issued by the City of Austin, Texas, in connection with a contract opportunity for Maintenance and Repair of Autoclave Lab Equipment. The document must be signed by an authorized person on behalf of the firm, with the signatory's printed name, title, location, and date of certification.
