About this file
This document is a Request for Quote (RFQ) issued by the Illinois Department of Veterans Affairs (IDVA) for the procurement of influenza vaccinations for three of its Veterans Homes located in Manteno, LaSalle, and Quincy, Illinois. The RFQ specifies the required products, which include Fluad Trivalent Influenza High Dose 0.5ml Prefilled Syringes and Fluzone Influenza High Dose 0.5ml Prefilled Syringes, and the quantities needed for each location. The vaccinations must be delivered between August 1, 2019, and September 20, 2019, for the fiscal year 2020. The RFQ states that the award will be made per location.
The RFQ outlines the pricing structure, requiring vendors to provide unit prices and extended prices for the requested quantities at each location, as well as any applicable freight charges. The RFQ also includes several terms and conditions, such as a requirement for a minimum 1-year warranty on all products, a 180-day validity period for quoted prices, and the agency's right to reject any or all offers. There are no specific set-aside requirements mentioned, and the RFQ does not provide information about incumbents or funding sources.
