The plan shall:
Demonstrate a management commitment to employee safety and health;
Identify the application of the safety and health plan to subcontractors;
Identify the roles and responsibilities of the following individuals with respect to safety and VPP: Management, Supervisors, Employees, and Safety Coordinator;
Identify applicable safety rules and regulations;
Include a worksite hazard analysis to include base-line hazard identification and required control measures;
Include a job site analysis to include hazards of tasks required to control measures;
Identify employee safety and health training requirements and the documentation process;
Include a workplace inspection frequency, to include identifying the individual conducting the inspections;
Include employee hazard reporting procedures;
Identify individual(s) responsible for corrective action of hazards;
Identify first aid/injury procedures;
Identify procedures for accident investigation and reporting;
Identify emergency response procedures; and
Identify the process for tracking controlled hazards in contractor work areas.
3.5.4.3.2. The contractor shall submit their Total Case Incidence Rate (TCIR) and Days Away, Restricted, and or Transfer Case Incident Rate (DART) rates and OSHA Form 300A annually to the AFIT COR to be submitted to the local contracting office for consolidation and submission as part of the installation’s annual VPP Safety and Health Management report. TCIR and DART rates are due by the 15th of January of each year.
Rate Calculation:
Total Case Rate (TCIR): Sum of the number of recordable non-fatal injuries plus illnesses in a given time frame using the formula (N/EH x 200,000) where:
N = Sum of the number of recordable non-fatal injuries plus illnesses in a given time frame.
EH = total number of hours worked by all employees in a given time frame.
200,000 = Equivalent of 100 full time workers working 40 hours per week, 50 weeks per year.
Days Away, Restricted, or Transferred (DART) Rate: This includes cases involving days away from work, restricted work activity, and transfers to another job and is calculated based on (N/EH) x (200,000) where:
N = the number of cases involving days away and/or job transfer or restriction, EH = the total number of hours worked by all employees during the calendar year, and
200,000 = the base for 100 full-time equivalent employees.
3.5.4.3.3. The contractor’s Quality Control Plan shall identify the processes and procedures the contractor will use to track compliance with the Safety and Health Plan, and the process and procedures that will be used to correct deficiencies. Task Order Level Service Delivery Summary
Task Order specific criteria above and beyond the IDIQ level service delivery summary will be defined in the PWS for each task order.
4.2. IDIQ Management
The contractor will ensure that the overarching IDIQ and each task order will have a designated Contractor Point of Contact (POC). The IDIQ/task order POC shall be responsible for serving as the coordinating single point of contact between the contractor and Government. The POC shall be fully authorized to manage the activities of contractor and third-party contractors needed to meet the task order requirements, providing a corporate escalation path for issues that may develop on individual task orders.
4.3. Performance Reporting. The contractor’s TO performance will be monitored by the Government and reported in CPARS or a Customer Survey, depending on the dollar amount of the task order. Performance standards shall include the contractor’s ability to provide or satisfy the following:
• Provide satisfactory solutions to requirements with the necessary customer support
• Provide solutions and services that meet or exceed specified performance parameters
• Deliver timely and quality deliverables to include accurate reports and responsive proposals
• Ensure solutions to requirements are in compliance with applicable policy and regulation
4.4. Monthly Invoice. The contractor shall provide a monthly invoice per TOto be submitted simultaneously with the monthly status report. The monthly invoice shall be submitted 10 calendar days from the last business day of each month. The invoice shall include but not be limited to:
• Task order number
• Travel costs (if needed).
• Supporting documentation for travel costs (if needed). Invoices including travel costs shall include supporting documentation as required by the Federal Travel Regulation (FTR) (receipts for all costs $75.00 or greater). Invoice submissions including travel costs shall include completed travel expense sheets for each trip for each employee.
• The contractor shall comply with line item billing requests.
• The contractor shall include number of hours expended per labor category, the labor category, and the rate for the month being invoiced.
• The contractor shall include a total amount of labor hours and total dollars expended.
4.5. Onramp/Offramp (open season) Provisions. This program will be reviewed one year after IDIQ establishment (then annually thereafter) to determine whether it is in the best interest of the Government to “reissue” an RFP for purposes of adding additional IDIQ contract holders. The Government is under no obligation to solicit or establish additional IDIQ contracts and add additional contract holders. However, should such a determination to hold an open season be made, during this process the Government may add additional IDIQ contract holders. In the event that additional IDIQ contracts are established in accordance with these provisions, the period of performance of the “onramped” IDIQ holders will only be for that length of time remaining in the originally established five year IDIQ period. In very rare instances, unforeseen and/or urgent IDIQ contract holder additions outside of annual open season may be approved. Approval shall only be given by the IDIQ contracting officer.
During the reviews of the program, the Government will also determine whether it is in its best interest to off-ramp any contractors. Reasons for off-ramping may include (but are not limited to) poor performance. If the Government determines that off-ramping is necessary, the contractor’s next IDIQ contract option will not be exercised. In rare instances, the Government may terminate the contractor’s IDIQ contract prior to the next option period. This may result in the contractor not receiving the minimum guarantee.
4.6. Contractor Submission. Deliverables are to be transmitted with a cover letter, on the prime contractor’s letterhead, describing the contents, electronically through the General Services Administration’s (GSA) web-based procurement system, Information Technology Solutions Shop (ITSS), and to any other destination(s) as required per the Government’s request. The contractor shall provide hard copy deliverables as required upon the Government’s request. All deliverables shall be produced using recommended software tools/versions as approved by the Government. All reports shall be accomplished utilizing the MS Office Software Suite (to include MS Project as required) or Adobe .pdf.
4.7. Government Review. Government personnel will have 10 business days to review deliverables (to include resubmissions) and provide written acceptance/rejection. The AFIT COR will notify the contractor of deliverable acceptance or provide comments in writing. The contractor shall incorporate Government comments, or provide rationale for not doing so within 5 business days of receipt of comments. Government acceptance of the final deliverable will be based on resolution of Government comments or acceptance of rationale for non-inclusion. Additional changes volunteered by the contractor will be considered a resubmission of the deliverable.
4.8. Deliverables. Deliverables are identified in subsequent paragraphs. All deliverables shall meet professional standards and meet the requirements set forth in contractual documentation. Revisions to deliverables, once submitted, cannot be revised but can be appended. All documentation generated becomes Government property.
4.8.1. Deliverable Table. If a delivery date falls on a Federal holiday or Saturday or Sunday, the deliverable due date shall be the next scheduled federal workday. The deliverable due time shall be on or before COB (1700) the day the deliverable is due unless otherwise stated within this PWS.
