About this file

This document is a Human Trafficking Affidavit required in compliance with Section 787.06(13) of Florida Statutes, intended for a nongovernmental entity executing, renewing, or extending a contract with a city government. The affidavit requires an authorized representative of the nongovernmental entity to attest under penalty of perjury that the company and its subsidiaries do not use coercion for labor or services as defined in the referenced statute.

The affidavit mandates that if the nongovernmental entity ever uses coercion for labor or services in the future, they will immediately notify the governmental entity, and no further contracts may be executed, renewed, or extended between the parties. The document includes spaces for company details, an authorized signature, notarization, and requires the signing representative to confirm personal knowledge of the matters stated and that the facts are true. The affidavit serves as a legal commitment to prevent human trafficking and ensure ethical labor practices in government contract relationships.
