About this file

This file is a HUD-53239 Leverage Excel template for the Choice Neighborhoods Implementation Grants, designed to capture leveraged resources across multiple development categories. The template requires applicants to list funds and resources for Neighborhood Development, Housing Development, and People (Supportive Services) components, with each entry needing a corresponding commitment document that meets standards outlined in the Notice of Funding Opportunity (NOFO). The document is part of the application process for the Choice Neighborhoods Implementation Grants, administered by the Department of Housing and Urban Development, which supports comprehensive community revitalization efforts focused on transforming severely distressed public and HUD-assisted housing.

The template includes sections for listing resources with specific requirements: documenting the source of resources, their dollar value, and the page number of supporting commitment documents. There are strict guidelines about consistency, with amounts needing to match other application attachments like Sources & Uses. The form is OMB-approved (No. 2501-0044, expiring 2/28/2027) and includes a public reporting burden estimate of 8 hours per response. Applicants must provide detailed information about leveraged resources, with a limit of 20 documents for the People Leverage Rating Factor, ultimately supporting HUD's goal of holistic neighborhood transformation and improved quality of life for residents.
