About this file
This is a Notice of Intent to Sole Source a federal supply contract. The Defense Health Agency Contracting Activity (DHACA), Healthcare Contracting Division – San Diego, intends to award a firm-fixed-price purchase order to Source Medical Systems, INC. (UEI: C73UQD1KU9M9, CAGE: 8V8K5) located in Castle Pines, Colorado for a Shoulder Innovations System to support an emergency requirement at Madigan Army Medical Center in Tacoma, Washington. The procurement is authorized under Federal Acquisition Regulation (FAR) 12.102(a) and the Revolutionary FAR Overhaul (RFO). The NAICS code is 339112 (Surgical and Medical Instruments Manufacturing) with a small business size standard of 1,000 employees; the Product Service Code is 6515 (Surgical and Medical Instruments and Supply). The required delivery date is 27 October 2026.
This acquisition is being pursued as a sole-source procurement because the patient requires reverse shoulder arthroplasty with a lower profile glenoid baseplate and surgical planning mechanism to address a proximal humerus fracture, small glenoid, and moderate superior inclination; these proprietary components are distributed exclusively by the original equipment manufacturer, creating no reasonable expectation of receiving quotes from two or more small businesses. The notice is not a request for competitive proposals, and interested vendors may submit capability statements not exceeding six pages (8.5" x 11", 10-point font minimum) including management, technical, and cost data with convincing evidence of capability to provide the equipment and preventive maintenance. The closing date for challenge submissions is 1000 Pacific Daylight Time, Wednesday, 28 October 2026. Capability statements must be submitted via email as Microsoft Word or Adobe PDF attachments to Joshua Thompson (joshua.t.thompson.ctr@health.mil) or Tracy Robinson (tracy.m.robinson18.civ@health.mil). Telephone requests or offers will not be accepted.

THIS IS A NOTICE OF INTENT TO AWARD A SOLE SOURCE SUPPLY CONTRACT
AND IS NOT A REQUEST FOR COMPETITIVE QUOTES.
The Defense Health Agency Contracting Activity (DHACA), Healthcare Contracting Division – San Diego (HCD-SD), intends to award a firm-fixed-price purchase order for Shoulder Innovations System to support an emergency requirement for MADIGAN ARMY MEDICAL CENTER, WA in accordance with Federal Acquisition Regulation (FAR) 12.102(a) under the Revolutionary FAR Overhaul (RFO).
Source Medical Systems, INC.
1038 Titanite PL Castle Pines, Colorado
UEI: C73UQD1KU9M9
CAGE:8V8K5
The North American Industry Classification System (NAICS) code for this requirement is 339112 (Surgical and Medical Instruments Manufacturing) with a small business size standard of 1000 employees. The associated Product Service Code (PSC) is 6515 (Surgical and Medical Instruments and Supply).
The Department of Orthopedic Surgery requests to procure the shoulder innovation system in order to treat an emergency procedure. This patient requires this implant system due to the patients anatomy and injury pattern. The patient has a proximal humerus fracture and is indicated for reverse shoulder arthroplasty. The patient has a very small glenoid as well as moderate superior inclination. This system offers a lower profile glenoid baseplate as well as providing a mechanism for surgical planning and correction of the deformity. Because these are proprietary components distributed exclusively by the OEM, there is no reasonable expectation of receiving quotes from two (2) or more small businesses. Accordingly, the proposed acquisition strategy is to execute this procurement on a sole-source basis under the authority of Other Than Full and Open Competition.
Delivery Date: 27 October 2026 Delivery Location:
Madigan Army Medical Center 9040 Fitzsimmons Drive Tacoma, WA 98431
