STATEMENT OF WORK

DESCRIPTION OF REQUIREMENT AND REQUIRING ACTIVITY:

The requirement includes the periodic preventive maintenance (annual), inspection, calibration and spare parts for VISULAS TRION, Manufactured by Carl Zeiss, Equipment Control Number (ECN) 084900, Serial No. 1217195. The requiring activity is U.S. Naval Hospital, Yokosuka.

PLACE OF PERFORMANCE:

U.S. Naval Hospital, Bldg #E-1400 CFA Yokosuka, Japan

PERIOD OF PERFORMANCE:

Basic Year: 15Sep2025 through 14Sep2026 Option Year I: 15Sep2026 through 14Sep2027 Option Year II: 15Sep 2027 through 14Sep2028 Option Year III: 15Sep2028 through 14Sep2029

PREVENTIVE MAINTENANCE (PM) SERVICES:

The Contractor shall perform the preventive maintenance (PM) services in accordance with the manufacturer’s instructions. The services shall be performed on site bi-annually during the performance period with mutually agreed upon schedule between the Contractor and the requiring activity. Only Biomedical Repair personnel are authorized to coordinate and schedule PM services. Such services shall include but are not limited to, technical inspection, adjustment, cleaning, replacement parts and other service to keep the equipment in good condition. PM services don’t include:

Maintenance/repair for other equipment/system stated in section #1.

Overhaul/renewal of subject equipment Movement/Removal of equipment Repair caused by inappropriate operation/treatment by the Government Consumable items/consumption parts

ON CALL EMERGENCY SERVICES AND CORRECTIVE MAINTENANCE OR REPAIR

The Contractor shall provide on-call services which include all necessary labor, materials and transportation to fix inoperative equipment and keep Equipment works properly up to 12 times per year. On-call services include replacement parts when it’s required. The on-call emergency services shall be provided upon request on oral or written notification by the authorized Biomedical Repair personnel or designated representative. The Contractor shall respond to the on-call services and arrive at the equipment site within 48 hours (within business hours) after such a call. This contract includes phone call support services during the performance period described in Section #11.

When any repair and/or replacement parts are required out of scope of above section 4 and in this Section 5, requisition needs to be submitted/processed by separated contract.

CONTRACTOR EXPERIENCE/TRAINING:

The Contractor shall provide upon documentation that the service personnel have received training and experience in repairing and/or maintaining the equipment covered herein.

REPORTING TO BIOMEDICAL REPAIR BEFORE AND AFTER SERVICE:

Inspection and acceptance of the preventive maintenance and on-call repair services performed hereunder shall be made by the receiving officer of the requiring activity. The contractor shall report to the authorized Biomedical Repair personnel prior to and upon completion of each service. Upon completion of the service, the contractor shall provide a copy of the service report (with English Translation) signed by a Biomed Technician, certifying that the service has been performed.

SERVICE REPORTS:

All medical equipment service reports must contain the following minimal information:

MAINTENANCE COSTS:

Costs of full maintenance services include labor, parts, travel time, mileage, and all other related expenses necessary to maintain the equipment in good operation condition without any cost to the Government.

REPLACEMENT PARTS:

The Contractor shall furnish replacement parts required for PM at no additional cost to the Government.

Consumable items, accessories, and items not connected electrically to the equipment are excluded from such replacement parts.

SERVICES PERFORMANCE TIME:

The schedule PM and phone call support services shall be performed during the regular working hours from 0800 to 1600, Monday through Friday except, Saturday, Sunday and Japanese holidays.
