About this file
This is a Past Performance Questionnaire (PPQ) template used by the Defense Health Agency (DHA) Western Markets Contracting Division to evaluate contractor past performance on U.S. Government contracts. The form is designed to be completed by customers, clients, or government agencies who have worked with an offeror on a previous contract. Completed questionnaires should be submitted to isaac.a.donwillies.civ@health.mil no later than October 23, 2025, either directly by the evaluator or through the offeror. The government reserves the right to verify all information submitted.
The questionnaire requires evaluators to rate contractor performance across thirteen detailed categories using a standardized five-point scale: "E" (Exceptional), "VG" (Very Good), "S" (Satisfactory), "M" (Marginal), and "U" (Unacceptable), plus "NA" (Not Applicable). Rating categories include client relationship, subcontractor management, employee hiring and retention, corporate management and integrity, quality of work and quality control, schedule performance, cost control, labor standards compliance, safety standards compliance, and timely documentation submission. The form also includes yes/no questions regarding cure notices, contract performance problem resolution, willingness to award future contracts, customer satisfaction, and contractor opportunity to respond to negative feedback. An overall firm rating must be provided, and evaluators must sign and date the completed form. The template accommodates space for offeror information, project details, evaluator credentials, and additional remarks.
