About this file
The document is a Price Appendix FAQ for a Department of Defense Pharmacy Uniform Formulary Blanket Purchase Agreement/Uniform Formulary Additional Discount Program solicitation. The FAQ provides detailed instructions for vendors submitting pricing information for two pricing sections: BPA (Blanket Purchase Agreement) and ADP (Additional Discount Program). For both sections, vendors are instructed to carefully read formulary notes, fill in yellow cells in a spreadsheet, and follow specific auto-population and calculation rules for pricing data.
Key guidance includes: filling out tables for up to 10 NDCs (National Drug Codes), with options to request additional tables if needed; auto-calculation of certain pricing fields; specific indicators for not submitting quotes (such as red highlighting); and contact instructions for questions or issues. The document is specifically related to pharmaceutical pricing for insulin and metabolic dysfunction agent classes, including rapid-acting insulin and injectable diabetic/weight loss agents. Vendors must follow precise spreadsheet instructions and are advised to contact the points of contact on the RFQ document for any clarifications or additional requirements.
