PART 1
1.0 GENERAL INFORMATION
1.1 This is a non-personal services contract to provide Childbirth Education Classes.
1.2 Description of services/introduction: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to provide Childbirth Education Classes as defined in this Performance Work Statement (PWS) except for those items specified as government furnished property and services. The contractor shall perform to the standards in this performance work statement.
1.3 Background: Provide specialized pre-natal (before birth) care that is personalized, and includes a comprehensive educational program to prepare the mother and the family for the process of becoming parents. Obstetrics professionals use the newest education, technology, and equipment, such as 4-dimensional ultrasound and video teleconferencing and up-to-date patient education. Counseling services to families with a history of genetic (inherited) disorders, include preconception counseling, prenatal genetic counseling and diagnostic testing.
1.4 Objectives: Provide a patient-centered program that focuses on providing optimal childbirth education classes tailored to the military member and their families. These classes will provide preparation for pregnancy, labor, and birth.
1.5 Scope: The contractor will provide WRNMMC a total of 34 (Not to exceed 54) hours of prenatal classes per month. Classes may be attended by individuals and/or couples.
1.5.1 Services will include prenatal classes offered in five areas relating to pregnancy and parenting to include, Prepared childbirth, Prepared Childbirth w/breastfeeding, Introduction to Breastfeeding, Infant Care, and Educating Siblings to be.
1.5.2 The instructors shall be provided by the contractor who is also responsible for supervising and training the instructors, evaluation and revising curricula, researching and writing new curricula and handouts, and interfacing with the Department of OB/GYN Representative (contract sponsor) and Contract Representative (COR) at WRNMMC. The contractor will work closely with WRNMMC to choose and/or develop new standardized handouts for the new curricula and update curricula and/or handouts when a change in WRNMMC policies, practices or procedures occurs and/or if there are new, relevant research findings.
1.6 Period of Performance (PoP): 2 Jan 2019 to 01 Jan 2020, Base Year 2 Jan 2020 to 01 Jan 2021, Option 1 2 Jan 2021 to 01 Jan 2022, Option 2 2 Jan 2022 to 01 Jan 2023, Option 3 2 Jan 2023 to 01 Jan 2024, Option 4
1.6.1 Transition: Transition-in/transition-out period
1.7 Administrative specifications
1.7.1 Place of performance: The work shall be performed at Walter Reed National Militray Medical Center, Obstetrics and Gynecology Department.
1.7.2 Recognized Federal holidays. The following is a list of legal Federal holidays as referred to elsewhere in the contract. Contractor Employee may be required to work on legal holidays, as determined by the Department’s Point of Contact.

1.7.3 Hours of operation: The contractor is responsible for conducting business Monday thru Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. The Contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons.
1.7.4 Emergency Services: On occasion, services may be required to support an activation or exercise of contingency plans outside othe normal duty hours.
1.7.5 Conduct: Contractor personnel shall adhere to standards of conduct as established by the DHA Commander.
1.8 Contractor travel: The Contractor HCP shall be paid the contract hourly rate until the HCP leaves the MTF premise. Hours worked do not include travel to and from the MTF.
1.9 Other Direct Costs (ODC)
1.10 Quality
1.10.1 Quality Control (QC): The contractor shall develop and maintain an effective quality control program to ensure services are performed as specified in Part 7, Attachment 1 of the PWS. The contractor shall develop and implement procedures to identify, prevent, and ensure nonrecurrence of defective services. The contractor’s quality control program is the means by which the work complies with stated requirements. Within thirty calendar days after contract award, three copies of a comprehensive written Quality Control Plan (QCP) shall be submitted to the COR and within 5 working days when changes are made thereafter. After acceptance of the Quality Control Plan (QCP) the contractor shall receive the contracting officer’s (CO) acceptance in writing of any proposed change to his QC system. See Technical Exhibit 1.
1.10.2 Quality assurance: The government will evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan (QASP). This plan provides a systematic method for the Government to evaluate performance and to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s).
1.11 Contractor personnel
1.11.1 Common Access Card (CAC) requirements: The contractor shall complete or provide to the Government all information required per the DHA CAC request process, current version 2.1, January 2018, or more recent when updated. See process attached in Section Part 7.1 of the PWS. A CAC is the standard identification for eligible DoD contractor personnel.
1.11.2 Contractor training. The contractor shall complete all requirements, training, and forms per the DHA’s Onboarding Checklist for Contractor Employees, current edition February 2018 or more recent when updated. See the form at https://info.health.mil/sites/DOP/OnboardingCtr/Contractor_OnBoarding_Checklist.pdf.
1.11.3 Physical Security: The contractor shall be responsible for safeguarding all government equipment, information and property provided for contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured.
1.11.4 Reserved.
1.11.5 Lock combinations: The Contractor shall establish and implement methods of ensuring that all lock combinations are not revealed to unauthorized persons. The Contractor shall ensure that lock combinations are changed when personnel having access to the combinations no longer have a need to know such combinations. These procedures shall be included in the Contractor’s QCP.
1.12 Key personnel (Contractor): The contractor shall provide a contract representative who shall be responsible for the performance of the work. The name of this person and an alternate who shall act for the contractor when the manager is absent shall be designated in writing to the CO. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The contract manager or alternate shall be available between the hours, of 8:00 a.m. to 4:30p.m., Monday thru Friday except Federal holidays or when the government facility is closed for administrative reasons.

1.14.1 Reserved.
1.14.1.1 As part of its submission, the Contractor shall provide the estimated total cost (if any) incurred to comply with this reporting requirement. Reporting period shall be the period of performance not to exceed 12 months ending September 30 of each government fiscal year and must be reported by 31 October of each calendar year.
Contractors may use a direct Extensible Markup Language (XML) data transfer to the database server or fill in the fields on the website. The XML direct transfer is a format for transferring files from a Contractor’s system to the secure website without the need for separate data entries for each required data element at the website. The specific formats for the XML direct transfer may be downloaded from the website.
1.14.1.2 Service portfolio groups are further define in DFARS PGI 237.102-74 Taxonomy for the acquisition of service and supplies and equipment.
1.14.1.3 Contractors shall direct questions to the help desk at: http://www.ecmra.mil/.
1.14.2 Non-Disclosure Agreement (NDA): All DHA Government contractor personnel who will obtain access to proprietary, classified, or confidential information or any information release of which is protected or governed by law or regulation associated with DHA acquisitions shall be required to complete and sign a DHA Contractor NDA (DHA Form 49) prior to beginning work on the subject contract. The Contractor shall execute an NDA on behalf of the company and shall ensure that all staff assigned to, including all subcontractors and consultants, or other personnel performing on contract/Task order execute an NDA protecting the procurement sensitive information of the Government and the proprietary information of other contractors. The NDA shall be executed not later than first day of employment and to be renewed upon exercising a contract option period. Assignment of staff who has not executed this statement or failure to adhere to this statement shall constitute default on the part of the Contractor.
The contractor shall maintain originally signed NDAs of individual employees and provide copy to the COR.
1.14.3 Contracting Officer Representative (COR): The COR will be identified by separate letter. The COR monitors all technical aspects of the contract and assists in contract administration The COR is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract;
perform inspections necessary in connection with contract performance; maintain written and oral communications with the Contractor concerning technical aspects of the contract; issue written interpretations of technical requirements, including Government drawings, designs, specifications; monitor Contractor's performance and notifies both the CO and Contractor of any deficiencies; coordinate availability of government furnished property;
and provide site entry of Contractor personnel. A letter of designation issued to the COR, a copy of which is sent to the Contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting contract.
1.14.4 Post award conference/periodic progress meetings: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5. The CO, COR, and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the CO will apprise the contractor of how the government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues.
These meetings shall be at no additional cost to the government.

2.0 DEFINITIONS, ACRONYMS, AND APPLICABLE PUBLICATIONS

2.1.3 Contracting Officer (CO): A person with the authority to enter into, administer, and/or terminate contracts and make related determinations and findings.
2.1.4 Contracting Officer’s Representative (COR): An individual, including a contracting officer’s technical representative (COTR), designated and authorized in writing by the contracting officer to perform specific technical or administrative functions. This individual does NOT have authority to change the terms and conditions of the contract.
2.1.5 Nonpersonal services contract: a contract under which the personnel rendering the services are not subject, either by the contract’s terms or by the manner of its administration, to the supervision and control usually prevailing in relationships between the Government and its employees.
2.1.6 Quality Assurance Surveillance Plan (QASP): An organized written document specifying the surveillance methodology to be used for surveillance of contractor performance. The Government may either prepare the quality assurance surveillance plan or require the offerors to submit a proposed quality assurance surveillance plan for the Government’s consideration in development of the Government’s plan.

2.4 Applicable Publication: Publications and forms can be obtained from the MTF. The Contractor is required to follow all mandatory documents to the extent they apply to this contract. Any such changes to mandatory publications that cause a change in the scope of performance will not be implemented by the Contractor until the Contracting Officer issues a modification in writing.
PART 3
3.0 GOVERNMENT FURNISHED PROPERTY, EQUIPMENT, AND SERVICES
3.1 Services: Government Training/Orientation.
3.1.2 Staff Orientation - New Employee. Contractor Employee shall participate in all staff orientation and/or training. Contractor Employee will attend a Government-sponsored initial orientation to familiarize them with the policies and procedures of the MTF. Orientation attendance will be scheduled by the COR. Such orientation training may include instruction on automated processing, standard operating procedures, local in-services, quality improvement policies, communications, and occupational exposure to blood borne pathogens, safety programs, etc.
3.1.3. HIPAA Privacy and Security Training (HIPAA 101). Contractor Employee shall be required to complete the On-Line Web-based Training Modules within the first 30 days of employment.
3.1.4. Government Unique Training. The Government may elect to provide unique Government training to Contractor Employee who are performing services under this contract. If the Government elects to provide such training, the Government will provide such training at no additional expense to the Contractor or to the Contractor HCP. When directed by the Contracting Officer, Contractor Employee shall attend all such training in a paid status as part of the normal services required and billed under the contract. Such training shall require a performance commitment by the Contractor and the Contractor shall reimburse the Government (by means of a reduction in an invoice) if a Contractor HCP fails to satisfy the performance commitment after the Contractor HCP receives the unique Government training. The amount of the reimbursement shall be the prorated cost of training, calculated based on the total cost of the training and the number of months by which the contract employee fails to complete the performance commitment. The length of the performance commitment shall be 12 months or until the end of all performance under this contract, whichever occurs first.
3.1.5. Contractor Employee Who Require Access to Government Information Systems. All contract employees with access to a government information system must successfully complete the DOD Information Assurance Awareness training prior to access to the information systems and then annually thereafter.
3.1.6. Hours for attending any of the above shall be compensated at the regular hourly rate established in the contract.
3.2 Facilities: The Government will provide the necessary workspace for the contractor staff to provide patient education as outlined in the PWS to include desk space, telephones, computers, and other items necessary to maintain an office environment. Space used by Contractor Employee in performance of services may be used for other purposes during their absence. Items of clothing, personal effects, or equipment cannot be secured during their absence. The Government will not incur any liability for theft, damage to, or loss of such personal items.
3.2.1 Emergency Healthcare: The MTF will provide emergency healthcare for injuries occurring while on duty.
The Contractor shall reimburse the Government for such services.
3.3 Utilities: The Government will provide all utilities in the facility that will be available for the contractor’s use in performance of tasks outlined in this PWS. The Contractor shall instruct employees in utilities conservation practices.
3.4 Equipment: The Government will provide telephones, facsimile machines, copiers and computer equipment that are authorized for transaction of official Government business only and shall not be used for personal business.
Personal long distance calls are not authorized and the cost of all personal long distance calls made may be deducted from the Contractor’s invoice payments. Telephones, facsimile machines and computer equipment are subject to communications security monitoring at all times. Items issued will remain the property of the Government and will not be removed from the MTF, hospital, or clinic. They are to be used, turned in, or disposed of as directed.
3.5 Materials: The Government will provide the regulations, policies, and/or procedures upon request by the Contractor Employee.
PART 4
4.0 CONTRACTOR FURNISHED ITEMS AND SERVICES
4.1 General: The Contractor shall furnish all personnel, supplies, equipment, and services to comply with the requirements of this contract, except where specifically identified as Government-furnished supplies and services
4.2 Secret Facility Clearance: Not applicable.
4.3 Materials: The Contractor shall provide materials, supplies, and equipment necessary to meet the requirements under this PWS.
4.4 Equipment: The Contractor shall provide equipment to meet the requirements under this PWS.
PART 5
5.0 SPECIFIC TASKS
5.1 Task Headings: Childbirth Education Classes.
5.1.2 Special Qualifications: Education, certification, and experience.
5.1.3 Education: High School Diploma or GED certificate.
5.1.4 Certification: Cardiopulmonary Resuscitation (CPR) (AHA or ARC); Completion of a basic dysrhythmia course or equivalent United States program; Advanced Cardiac Life Support (ACLS) is desirable but not required.
Pass a Walter Reed National Military Medical Center-Bethesda Dysrhythmia Exam.
5.1.5 Experience: At least three years’ experience in obstetrics or certified in childbirth education.
5.1.6 Licensure: Childbirth Instructors will have certifications in the Childbirth Education or Obstetrics field from a recognized childbirth certifying organization (Lamaze International, the International Childbirth Education Association [ICEA], or the Childbirth and Postpartum Professional Association [CAPPA]), or a comparable organization or will have a minimum of 3 years’ experience in the field of childbirth education or obstetrics.
5.1.7 Breastfeeding instructors will have a certification from the International Lactation Consultant Association (ILCA) or a comparable organization, or will have a minimum of 3 years’ experience in the field of lactation education or obstetrics.
5.1.8 Other Qualifications: (1) Able to read, write, speak and understand English well enough to effectively communicate with all patients and other health care providers. (2) Knowledge of equipment and supply terminology and skill sufficient to identify and use necessary equipment and supplies properly. (4) Working knowledge in medical terminology is mandatory.
5.1.9 Knowledge of computer operations and proficiency in the use of basic word processing, data entry and automated medical records.
5.1.10 Be a U.S citizen.
5.2 Specific Duties: The contractor will provide a total of provide 6-8 classes per month to the WRNMMC OBGYN population. Classes attended by individuals and/or couples. Each instructor will distribute and collect patient satisfaction surveys at the end of every class to the individuals and couples for tabulation and analysis. Each individual or couple in a class shall be provided with one class survey form. The forms are devised by the WRNMMC staff. The instructor will turn in these surveys at the end of every class to a location determined by the designated clinic representative. The length and frequency of the classes may vary based on need but will not exceed 34 hours per month or 8 classes per month. The contractor and WRNMMC personnel will work together to determine the length and frequency of each class during that time.
5.2.2 The contractor will provide Prenatal classes offered in five areas relating to pregnancy and parenting:
Prepared Childbirth, Prepared Childbirth w/Breastfeeding, Introduction to Breastfeeding, Infant care, and Educating Siblings-to-be.
5.2.2.1 The contractor will provide Prepared Childbirth class will teach individuals and/or couples what to expect when having a baby at WRNMMC, comfort measures, false versus true labor, stages of labor, how support person(s) can help, pain control options, delivery methods, relaxation, breathing, cesareans, recovery and postpartum. This class will be six (6) hours long and will be offered twice a month for a total of twenty-four (24) classes per year.
5.2.2.2 The contractor will provide Prepared Childbirth w/Breastfeeding class will teach individuals and/or couples how and when to feed, how to correctly position the baby on the breasts (the latch), how to tell if baby is getting enough, hunger cues, breast care, breastfeeding’s benefits, mother care, special situations, common concerns, how support person(s) can assist and WRNMMC support. Participants practice feeding positions using dolls. Class is for both mom and her partner. This class will be eight (8) hours long and will be offered once a month for a total of twelve (12) classes per year.
5.2.2.3 The contractor will provide Introduction to Breastfeeding class will teach individuals and/or couples how and when to feed, how to correctly position the baby on the breasts (the latch), how to tell if baby is getting enough, hunger cues, breast care, breastfeeding’s benefits, mother care, special situations, common concerns, how support person(s) can assist and WRNMMC support. This class will be two (2) hours long and will be offered twice a month for a total of twenty-four (24) classes per year.
5.2.2.4 The contractor will provide Infant Care class will teach individuals and/or couples newborn procedures:
diapering, bathing, safety measures, when to call the doctor and coping with the demands of the newborn. This class will be two and a half (2.5) hours long and will be offered once a month for a total of twelve (12) classes per year.
5.2.2.5 The contractor will provide Educating Siblings to Be class will teach Teach children learn about babies.
They tour Delivery area, watch a film, hold a “baby,” do an activity. Parents learn WRNMMC policy regarding children in Labor and Delivery and how to incorporate another child into the family. This class will two (2) hours long and will be offered quarterly for a total of four (4) classes per year.
5.2.3 Operations: The contractor shall provide a sufficient number of instructors to teach all classes. Upon receiving the class roster, the contractor will contact each individual registered to remind them of the class and give them information on when and where to meet the instructor. In the event of inclement weather or class cancellation without a replacement instructor, the contractor will contact the patients to reschedule or notify them of class cancellation. Class cancellations will be re-arranged by the contractor and coordinated with the OB/GYN Representative for availability of resources. The contractor will ensure each member is called with the new scheduled class date and time. Documentation of the rescheduled class will be forwarded to the OB/GYN Representative and the COR for tracking purposes. If applicable, classes will be rescheduled within the same month to ensure the monthly 54 hrs. is maintained. If classes are not able to be rescheduled within the same month, the course will be rescheduled the following month with an explanation attached to the schedule for documentation purposes. All documentation will be forwarded to the Department of OB/GYN Representative and COR Representative for tracking purposes.
PART 6
6.0 INFORMATION TECHNOLOGY & SECURITY
6.1 All work under this contract is unclassified. If security requirement is Secret the security requirements are in accordance with the attached DD254
6.3 PII/PHI, and Federal information requirements (refer to Clause Section for DHA Procedures, Guidance and Information 224.90 if applicable).
6.4 Training.
6.4.1 Contractor employees performing cybersecurity / cyberspace functions shall comply with the following requirements:
6.4.1.1 Training: All contractor and associated subcontractor employees working Cybersecurity (Information Assurance (IA))/Cyberspace functions must comply with DoD training requirements in Department of Defense Directive (DoDD) 8140.01, and DoD 8570.01-M.
6.4.1.2 Certification: Per DoDD 8140.01, Defense Federal Acquisition Regulation Supplement (DFARS) 252.239- 7001, contractor employees supporting Cybersecurity (Information Assurance)/Cyberspace functions shall be appropriately certified upon contract/task order award. The baseline certification as stipulated in DoD 8570.01-M must be completed prior to the beginning of their contract support services. In addition, the contractor shall comply with Computing Environment (CE) certification requirements as specified in the contract. CE certifications shall be obtained within the timelines specified in DoD 8570.01-M.
6.4.2 Privileged user requirements: All contractor employees with privileged user status must comply with the requirements of DHA-Administrative Instruction (AI) 081, Employee use of Information Technology.
6.5 General cybersecurity requirements – Contractor employees who require access to Government Information Systems must successfully complete the DOD Cyber Awareness training prior to access to the information systems and then annually thereafter.

Department of Defense Activity Address Code (DoDAAC) is a six position code that uniquely identifies a unit, activity, or organization.

3 Use of Former DoD/Defense Health Agency (DHA) Employees and Uniformed Service Members in Proposal Preparation.
The involvement of a former DoD/DHA employee/member in an offeror’s proposal preparation may give rise to an unfair competitive advantage or the appearance thereof, if the former DoD/DHA employee/ member acquired non-public, competitively-useful information in his or her former position. Such knowledge could include proprietary information of competitor’s performance on past or current contracts with similar requirements or source selection sensitive information pertaining to this procurement. Consequently, the Offeror must notify the Contracting Officer prior to the involvement in the proposal preparation by a former DoD/DHA employee/member reasonably expected to have had access to such information. Based on the notification, the Contracting Officer will make a determination whether involvement of the former DoD/DHA employee/member in proposal preparation could create an unfair competitive advantage or appearance thereof. The Contracting Officer will further determine whether any mitigation measures taken or proposed by the offeror are adequate to alleviate this concern or whether the offeror will be disqualified from the competition.
