Instructions

Tabs A, AA,AB, AC & AD:
All Rates including benefits must be based on hourly rates and not annual figures
Compensation for health care providers (employees) shall not exceed $400,000 per year.
The offeror can add additional columns according to their benefits package
The offeror can label the columns according to their benefits package. The Government has provided a few examples that can be change accordingly to the offerors benefits package.
The offeror is responsible for doing the mathematical calculations, like adding the fringe benefits, calculating the percentage and the total salary, etc.
The offeror cannot change the order and name of the specialties (column A).
Productive Hrs.= total hours in a work year LESS vacation & holidays (e.g. 1,920 hours = 1 one full time contractor employee for one year = 8 hrs/day x 5 days/wk for 48 wks productive hrs.)
Another way to think of productive hours is that they are hours actually worked and billed to the Govt. contract.
Costs associated with vacations and holidays are normally included in the overhead/fringe benefit rate, therefore the vacation/holiday associated costs are to be input into columns J&K (Tabs A, AA, AB, AC & AD) for the purposes of this spreadsheet.
Tab B "Roll Up":
Populate the Fully Burdened Hourly Rates for the Base Period and Option Years.
Verify the math is correctly computed.
