The following have been modified:

CLAUSE(S) 52.212-1 - ADDENDUM TO 52.212-1

Replace 52.212-1 paragraphs (c) and (e), including all subparagraphs, as follows:

(c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 120 calendar days from the date specified for receipt of offers.

(e) Single Offer. Offerors shall submit one offer presenting terms and conditions for commercial items/services satisfying the requirements of this solicitation. In the event an offeror submits more than one offer, only the last complete offer submitted prior to the due date and time for offers will be evaluated.

Append the following paragraphs to 52.212-1:

(m) Offer Submission Instructions. This solicitation uses commercial terms and conditions in commercial format at FAR Part 12 and competitive procedures at FAR Subpart 15.3. As the Government intends to award without discussions, the initial offer should reflect a competitive position in terms of price and technical approach. Only one contract award is anticipated to result from this Request for Proposal (RFP); however, prior to award, the Government reserves the right to withdraw, modify, or eliminate various aspects of the requirement determined to be too costly or impractical. By submitting an offer, the offeror indicates intent to comply with all requirements of the solicitation and perform all work within the scope of an anticipated contract award. Offers that do not comply with RFP instructions may be rejected without consideration or evaluation.

(1) Questions. Questions shall be submitted to via email to the Points of Contact (POC) identified below, no later than March 17th, 2025, at 4:00 p.m. Eastern Time. Follow-up/additional questions will not be accepted after this date and time. Answers to submitted questions will be posted to Business Opportunities along with any resulting amendment to the solicitation.

(2) Electronic Submittal: Offerors shall electronically submit offers via e-mail to the Points of Contact (POC) identified below. The offeror is responsible for ensuring receipt by the POC. The firewall utilized by the Government contracting office does not allow receipt of zip files. Emails exceeding 20 MB may not deliver.

Offers may be submitted by multiple emails.

(3) Points of Contact. Ms. Cherish Young, Contracting Officer, cherish.d.young2.civ@health.mil, and Mr. John Kelley, Senior Contract Analyst (Contractor), john.j.kelley36.ctr@health.mil.

(n) Offer Content and Format. An offer shall consist of a cover letter and five volumes: Volume 1 for material to be reviewed under the Program Management and Quality Control factors, Volume 2 for material to be reviewed under the Experience factor, Volume 3 for material to be reviewed under the Past Performance factor, Volume 4 for all Price information, and Volume 5 for all other matters of responsibility required for compliance with the solicitation.

A page is 8.5 inches by 11 inches with 1-inch margins using single lines in Times New Roman font of no less than 12-point size font. Tables, graphics, and call-outs may use 10-point font. Narrative shall be submitted in MS Word or searchable PDF Format. Detailed pricing information shall be submitted in tabular format compatible with MS Excel. The offeror’s company name, the date, solicitation number, and volume number shall be included on each page of the offer and may display in a header/footer. All pages of the offer volumes shall be numbered, using a uniform numbering system. Page numbers, headers and footers may be outside the page margins and are not bound by the font and point size requirement. For any segments of an offer with a specified page limit, pages exceeding the limit will be removed and not forwarded for evaluation. Any offer containing proprietary information shall be clearly marked. All pricing information shall be addressed ONLY in Volume 4. Each volume shall contain a glossary of all abbreviations and acronyms used, with an explanation for each. Cover pages, Tables of Content, and Glossaries do not count against the page limitations for their respective volumes.

(1) Cover Letter Instructions. The cover letter has no page limit. The cover letter shall reference the solicitation number and identify all enclosures being transmitted as part of the offer. The cover letter shall include the Company Name, Address, Contract and Government Entity (CAGE) code, and Unique Entity Identifier (UEI).

The cover letter shall identify the name, address (if different from the company address), telephone number and e-mail address of the individual authorized to provide clarifications and exchange communications with the Government. Cover letters shall show offer validity. Cover letters shall indicate the time specified for receipt of offers and shall provide a statement specifying the extent of agreement with terms, conditions, and provisions included in the solicitation. Cover letters shall indicate receipt of all solicitation amendments by attachment of one copy of each Standard Form 30 or by listing each solicitation amendment. Contents of the cover letter will not be evaluated against the evaluation criteria specified in 52.212-2.

(2) Volume 1 Corporate and Management Capabilities. Volume 1 shall include the Program Management narrative and Quality Control Plan.

(i) Section 1, Program Management. This section will be reviewed under evaluation Factor 1, Program Management. This section shall not exceed 10 pages. This section shall describe how the offeror intends to organize, plan, report, prioritize, schedule work and exercise project control. At a minimum, this description should address the following elements:

(A) Internal and external lines of authority and communication;

(B) Approach to provide a sufficient pool of qualified personnel to perform this effort; and

(C) Reporting systems to include system/process for capturing performance metrics.

(ii) Section 2, Quality Control Plan. This section will be reviewed under evaluation Factor 3, Quality Control Plan. The offer shall include the draft Quality Control Plan described at PWS paragraph 1.10.1.

The draft plan has no page limit. The draft plan shall address the following:

(A) Sampling and measuring for each PRS (PWS, Attachment 1) measure. This approach will ensure attainment of the Acceptable Performance Level for each PRS measure. This approach must be agreeable to both the Government and Contractor and will use one of the following methods described for each PRS measure: Quality of Service, Access to Services and Cost Containment. The plan must detail the procedures to be utilized to ensure inspection will be taken from the appropriate phases of workflow. If the Offeror proposes any changes or modifications to the PRS, these should also be included.

(B) Appointment of an official who shall be responsible for the operation of the quality control system/department and for investigating and ascertaining the causes of deficiencies.

(C) Describe the safeguarding and protection of privacy information.

(3) Volume 2, Experience. This volume will be reviewed under evaluation Factor 3, Experience. Volume 2 shall not exceed 5 pages. Volume 2 shall describe the offeror’s previous experience in performing the following tasks:

(ii) Mentor clinic personnel

(iii) Technical writing

(iv) Provide operations support in maintaining repositories, including SharePoint and shared drives

(v) Provide coordination of activities to include use of trackers, calendar invites, email communications

(4) Volume 3, Past Performance Instructions. This volume will be reviewed under evaluation Factor 4, Past Performance. Volume 3 shall identify the offerors performance history and references for past performance questionnaires. Performance history shall not exceed one page for each identified contract.

(i) Performance History. The offer shall identify Government contracts relevant to the requirement of this solicitation, performed within the past three years from the date of the solicitation, and performed by the offeror or performed by a proposed subcontractor, partner, or affiliate substantively contributing to the proposed effort. The offer shall identify the contract number, dates of performance, and contact information for an individual knowledgeable of the offeror’s performance. The offer shall describe the extent to which each contract is relevant to the offered effort. Relevant work is similar in size, scope, and complexity to the work described in the PWS and demonstrates the offeror’s ability to perform the primary and vital requirements of that work.

(ii) References. The offeror may seek up to five past performance references for contracts identified in the performance history using the Past Performance Questionnaire at Attachment B of the solicitation. At least one reference must be a contract performed by the prime offeror. The offeror should complete the first page of the questionnaire and provide the file to a contact who is knowledgeable of the offeror’s performance for the reference organization. The contact must be an employee or authorized agent of the reference organization. The contact should complete the remainder of the form and forward directly to the Government points of contact for this solicitation. The offer shall include the first page of each questionnaire issued by the offeror.

(iii) Information From Other Sources: The Government may also seek and consider past performance information obtained through other sources, including but not limited to the Past Performance Information Retrieval System.

(iv) Subcontractor Consent to Discuss Past Performance Information. For each proposed subcontractor, the offeror shall submit the Subcontractor Past Performance Consent form at Attachment E.

(5) Volume 4, Price Instructions. The Price Volume has no page limit. The Government anticipates award of a Firm Fixed Price contract. The offeror shall price all services required by the Performance Work Statement under the Firm Fixed Price CLINs. The offerer may include the cost of cellular telephone services, plus applicable burdens, in the price of each CLIN. The offeror’s price shall not include assumptions that change the terms or provisions of this solicitation.

(i) Price Narrative. The offeror may include explanation of pricing in narrative format.

(ii) Pricing Sheet. The offer shall include a completed pricing in the “CLINS TOTAL” tab of Attachment A, Pricing Sheet, in a Microsoft Excel format. The attached pricing sheet is set to calculate the CLIN prices from the “CLIN BREAKDOWN” tabs. Offerors making alterations or additions to the pricing sheet are responsible for ensuring that CLIN unit prices and total prices calculate correctly. All fixed price line items must include a unit price, rounded and extended to two decimal places. The evaluated price of extension of service at FAR 52.217-8 will be calculated from the unit values of CLIN 4001. Line items offered at no price shall be marked “NSP” or zero dollars. Failure to submit a price on any CLIN shall be a material deficiency of the offer and result in it being rejected without evaluation. The spreadsheets must be formatted such that when printed, the pages will be readable on 8.5 x 11 inch paper.

(iii) Supplemental Pricing Information. The offer shall include supplemental pricing information that provides a breakdown of the price for each fixed-price CLIN. The offeror shall show the fully burdened rate. This breakdown shall identify each item (e.g. labor category), and the number of units of the item (e.g., labor hours). The offer shall demonstrate calculation of the CLIN unit price, rounded to the nearest cent, from the item price information. The offeror shall complete a “CLIN BREAKDOWN” tab of Attachment A, Pricing Sheet, for each services CLIN. A “CLIN BREAKDOWN” tab is not required for extension of services at 52.217-8. The offeror shall complete all cells where an input is required.

Supplemental pricing information shall identify item prices for services to be performed by the offeror, similarly situated subcontractors, and subcontractors who are not similarly situated. Where a similarly situated subcontract is assigned a NAICS code different from the NAICS code at the SF 1449, Block 10, the offeror shall provide a narrative justification for the subcontract NAICS code. The offer shall demonstrate calculation of the percentage of award value to be performed by the offeror and any similarly situated subcontractors for each performance period, except extension of services at 52.217-8.

(iv) Compliance with Service Contract Labor Standards. The Government’s estimated level of effort anticipates labor generally exempt from Service Contract Labor Standards. Some PWS tasks, however, could be performed by service employees under the definition at FAR 52.222-41. For offered performance by such service employees, the offeror shall identify the equivalent standard occupational code and county and state of performance. For such service employees, the offeror shall identify the direct hourly labor rate;

hourly, weekly or monthly health and welfare rate; and number of days of annual paid time off. Offerors may make a written request to the points of contact on this solicitation for a wage determination prior to the due date and time for proposals.

(v) Estimated Level of Effort. The Government estimates that this order will require an approximate level of effort, as outlined below, to accomplish the tasks identified in the PWS. The offeror is not constrained by the information provided and may propose alternate labor categories and/or hours as the offeror determines are appropriate to accomplish the tasks in the PWS. The Government estimate is based on 1,912 productive labor hours per person per year. The offeror’s accounting of productive labor hours per person per year may differ. The offered level of effort should be consistent with the offeror’s total submission.

Labor Category Full Time Equivalent

Expected PWS Task

Psychologist 5.0 5.2 Operations Specialist 1.0 5.3

(vi) Additional Pricing Information: The contracting officer may request additional pricing information in accordance with the provision at 252.215-7010, as incorporated into this solicitation. The contracting officer has determined that the services to be procured meet the definition of commercial services, so offerors are exempt from the requirements at 10 U.S.C. 3702 and FAR 15.403-3 for certified cost and pricing data.

(6) Volume 5, Other Matters of Responsibility. Contents of this volume will not be evaluated against the evaluation criteria specified in 52.212-2. This volume has no page limits.

(i) Representations and Certifications. The offeror shall have current FAR and DFARS representations in System for Award Management (SAM). For representations made only for the offer submitted in response to this solicitation and different from those in SAM, the offeror shall complete appropriate sections of the provision at 52.212-3 (Alternate I), as incorporated into this solicitation. If the offeror’s representations at

252.204-7016 in SAM is marked “does” provide, the offeror shall complete the representation at 252.204- 7017. If the offeror’s representation at 52.204-26 or 52.212-3(v) in SAM is marked “does use,” the offeror shall complete the representation 52.204-24, as incorporated in this solicitation. The offeror shall complete the representations at 52.229-11.

(ii) Contract History OCI Review. Using instructions at DHA.L.1, Preventing Organizational Conflicts of Interest, paragraph (c)(2), offerors shall provide an OCI disclosure statement and list all contracts and task orders (whether awarded by DHA or another contracting activity) under which the offeror and any anticipated subcontractors performed work for DHA as either prime contractors or subcontractors within the past 5 years, including efforts currently being performed. This includes actions issued under contracting office codes HT0011, HT0014, HT0015, HT0038, HT0090, HT9402, HT9406, HT9407, HT9408, HT9409, HT9410, HT9411 or HT9425, and actions issued by other contracting activities using funding office codes at Attachment D. The offer shall prepare the disclosure of contracts in a format substantially the same as the worksheet at Attachment C. Disclosure of other relevant facts concerning any past, present, or currently planned interests shall be made in narrative format. Disclosures for proposed subcontractors and team members must be submitted with the offer and may not be submitted separately.

(iii) Organizational Conflict of Interest Mitigation Plan. The offer shall include an OCI Mitigation plan as described in paragraph DHA.L.1, Preventing Organizational Conflicts of Interest, paragraph (e)(1). ALL OFFERORS ARE REQUIRED TO SUBMIT AN OCI MITIGATION PLAN with the offer, even if the offeror has NEVER performed contracts for DHA. Failure to submit an OCI Mitigation Plan shall be a material deficiency of the offer and result in it being rejected without evaluation. In addition to mitigation of actual or potential OCI identified in the disclosures for OCI Review, if any, the OCI plan shall address the offeror’s controls and employee training for identifying OCI which may emerge in the future performance of a contract as a result of the awardee’s position of influence and potential access to competitively useful information. The offer shall include document number and date for the plan to complete the fill-in at instruction DHA.H.2 – Mitigation Of Organizational Conflicts Of Interest.

(iv) Proof of Insurance. The offeror shall provide proof of Worker’s Compensation insurance for all places of performance locations as well as other proof of insurance (general liability, vehicle, etc.) appropriate to the offeror’s approach to services.

(v) Quality Assurance Surveillance Plan. The offer shall provide for a name, phone number and e-mail to complete QASP paragraph 3.4, Contractor Representatives, should the offer result in an award.

(vi) Other Disclosures. The offeror shall review all other provisions incorporated at paragraph (r) of this Addendum to 52.212-1 and make any required disclosures.

(8) Page Limit Summary.

(i) Cover Letter: No page limit

(ii) Volume 1:

(A) Section 1: Not to exceed 10 pages excluding Cover pages, Tables of Content, and Glossaries.

(B) Section 2: No page limit

(iii) Volume 2: Not to exceed 5 pages, excluding Cover pages, Tables of Content, and Glossaries.

(v) Volume 3:

(A) Performance History: Not to exceed one page per contract.

(B) References: First page of each questionnaire, not to exceed five questionnaires.

(C) Subcontractor Consent to Discuss Past Performance Information: One form per subcontractor.

(vi) Volume 4: No page limit

(vii) Volume 5: No page limit

(o) Solicitation Amendment After Receipt of Proposals. If the contracting establishes a competitive range and enters into discussions, any instructions for Final Proposal Revisions will be issued by amendment to this solicitation distributed to those offerors remaining in the competitive range.

(p) Changes to Term and Price of the Base Period. By submission of an offer, the offeror agrees that the Government may award at a later start date than the solicited effort, but no later than the end date through which the offer is valid. The offeror additionally agrees that the Government may reduce the term of the base period, at the proposed monthly price, as a result of delays in the award decision or contract clearance process, or due to limitation of funds. With any such reduction in base period term, the Government will recalculate the term and price of the option periods so that the resulting award total price and term are the same as the proposed total value and term of the successful offer. For example, if the agency were to reduce the base term by 6 months, the recalculated Option 1 monthly price would be the average of 6 months at the proposed base price plus 6 months at the proposed Option 1 price. The agency would similarly recalculate the Option 2, 3, and 4 prices and add a 6-month Option 5 at the proposed Option 4 monthly price.

(q) Non-Government Advisors. Contract support personnel from Amyx, Inc., will be providing administrative support to the solicitation and contract administration support functions related to this contract effort. Contractor personnel have executed Non-Disclosure Agreements, and their contracts include clauses prohibiting Organizational Conflicts of Interest.

(r) The non-FAR Part 12 discretionary FAR, DFARS, and local provisions included herein are incorporated into this solicitation either by reference or in full text. If incorporated by reference, see provision 52.252-1 for locations where full text can be obtained.

CLAUSE(S) 52.212-2 - ADDENDUM TO 52.212-2

Replace 52.212-2 paragraphs (b) as follows:

(b) Options. The Government will evaluate proposals for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that proposal is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). As part of the price evaluation, the Government will evaluate its option to extend services (see FAR Clause 52.217-8) by adding half of the amount of the proposal’s second option period prices to the proposal’s total price. Thus, the total price for the purpose of evaluation will include all CLINs in the phase-in period, all CLINs in the base period, all CLINs in the first and second option periods, one-half of CLINs in the second option period, and all CLINs in the phase-out period.

Append the following paragraphs to 52.212-2:

(d) Basis for Award. Award of a contract will be made on a competitive basis, using “best value subjective tradeoff” among price and factors other than price as defined in the DoD Source Selection Procedures found at https://www.acq.osd.mil/dpap/policy/policyvault/USA004370-14-DPAP.pdf. The Government may to award to other than the lowest priced proposal, or other than the proposal rated highest in factors other than price. The contracting officer intends to award without discussions but may enter into discussions if it is in the Government’s interest to do so. If entering into discussions, award will be based on evaluation of the Final Proposal Revisions.

(1) After the date for receipt of offers, the Government will review each offer for compliance with the content and format requirements at the Addendum to 52.212-1, paragraph (n), as incorporated in this solicitation. For any segments of an offer with a page limit defined at the Addendum to 52.212-1, pages exceeding the specified limit will be removed before the quote is forwarded for evaluation. An offer failing to meet any other material requirement of content or format at the Addendum to 52.212-1 may be removed for noncompliance and returned to the offeror without evaluation.

(2) Offers forwarded for evaluation will be assigned a rating for each of the factors described in paragraphs (e), (f), (g) and (j), below. Price will be evaluated in accordance with paragraph (k), below. If the content of an offer is inadequate to complete an evaluation under a factor, the Government will assess a Deficiency for that factor.

For example, an offer which repeats the requirements of the Performance Work Statement without addressing the offeror’s particular approach or capabilities may lack adequate content for an evaluation.

(3) Following assessment of all ratings, the Government will determine the offer that represents the best value by performing trade-off analysis. Only offers rated Satisfactory Confidence or Neutral Confidence in the Past Performance factor; evaluated to be fair and reasonable in price; rated at least Acceptable in all other evaluation factors; and compliant with all other requirements of the solicitation will be considered for award and enter into tradeoff analysis. The selection authority will compare weighted evaluations ratings and comments which distinguish one offer from another to the difference in price of the offers. The better value of two offers will be compared to the next offer, and so on, until one offer is determined to be a better value of all other offerors.

This offer is the best value and will be selected for award. Trade-off analysis is not intended to produce a ranking of all offers. The Government reserves the right to make no award, depending on the quality of proposals received and the availability of funds.

(e) Factor 1 Program Management. The Government will review Volume 3, Sections 1, to determine the offeror’s ability to organize, plan, report, prioritize, schedule work and complete tasks. The evaluation will record comments characterized according to paragraph (i)(1). This factor will be assigned a combined Technical and Risk rating as described at paragraph (i)(2) of this Addendum to 52.212-2. Consideration will be given to the degree to which the proposed processes appear effective and flexible in planning, organizing, scheduling, reporting, and sufficient for completing tasks. Additional consideration will be given to the Offeror’s demonstrated ability to screen and hire qualified personnel to successfully accomplish the requirements. The evaluation will record a Significant Weakness for each performance period where percentage of contract value to be performed by the offeror and similarly situated subcontractors would not comply with 52.219-14 as incorporated in this solicitation. The evaluation will record a Significant Weakness where compensation of service employees, if any, is below the minimums of a relevant Wage Determination.

(f) Factor 2, Experience. The Government will review Volume 2, Section 1, narrative to assess the offeror’s experience in performing the tasks identified at Addendum to 52.212-1, paragraph (n)(3). The evaluation will record comments characterized according to paragraph (i)(1). This factor will be assigned a combined Technical and Risk rating as described at paragraph (i)(2) of this Addendum to 52.212-2. For any task not addressed, the evaluation will assume the offeror has no experience performing that task.

(g) Factor 3. Quality Control Plan. The Government will review Volume 1, Section 2, draft Quality Control Plan, to determine whether the approach to quality control identifies processes, procedures, and metrics which are likely to result in successful outcomes within the cost and on schedule. An “Acceptable” rating will be assigned if the plan addresses all the required elements at Addendum to 52.212-1, paragraph (n)(2)(ii), and the plan demonstrates a realistic and reasonable approach. Otherwise, an “Unacceptable” rating will be assigned.

(i) Technical and Risk Ratings. The Program Management factor and Experience factor will receive a rating based on evaluation comments applied to the criterial below.

(1) Evaluation Comments. Evaluation comments affecting factor ratings will be characterized as Strengths, Weaknesses, Significant Weaknesses, or Deficiencies as defined below.

(2) Technical and Risk Rating Scale.

Combined Technical and Risk Ratings Scale

Rating Definition and Criteria

Outstanding

Proposal indicates an exceptional approach and understanding of the requirements and contains multiple strengths. Impacts of any weaknesses have little potential to disrupt schedule, increase cost or degrade performance. Normal contractor effort and normal Government monitoring will likely be able to overcome any difficulties.

Good

Proposal indicates a thorough approach and understanding of the requirements and contains at least one strength. Impacts of a significant weakness or combination of weaknesses have little potential to disrupt schedule, increase cost or degrade performance; or may disrupt schedule, increase cost or degrade performance. Normal contractor effort and normal Government monitoring, up through special contractor emphasis and close Government monitoring, will likely overcome difficulties.

Acceptable Proposal meets requirements and indicates an adequate approach and understanding of the requirements. Impacts of a significant weakness or combination of weaknesses may disrupt schedule, increase cost or degrade performance. Special contractor emphasis and close Government monitoring will likely overcome difficulties.

Marginal Proposal has not demonstrated an adequate approach and understanding of the requirements;

or impacts of a significant weakness or combination of weaknesses would likely disrupt schedule, increase cost or degrade performance. Is unlikely to overcome any difficulties, even with special contractor emphasis and close Government monitoring.

Unacceptable Proposal does not meet requirements of the solicitation, and thus, contains one or more deficiencies, and/or risk of unsuccessful performance is unacceptable. Proposal is unawardable.

(j) Factor 4: Past Performance. The Government will review Volume 4, past performance questionnaires from references, and past performance information obtained from other sources to identify reports of recent and relevant past performance.

(1) Recent efforts are those performed within the last three years from release of this solicitation, including efforts currently being performed.

(2) Relevant past performance for the offeror is like service as stated in this solicitation’s Performance Work Statement in terms of similar scope and complexity. Relevant past performance for other team members is like service in terms of scope and complexity to the portion of the solicitation’s Performance Work Statement that they are proposed to perform. A past performance reference is either relevant, somewhat relevant, or not relevant.

(3) Recent and relevant past performance will be reviewed for favorable and adverse findings. Consideration will be given to quality of service, timeliness, effectiveness in managing, directing resources, coordinating, and reasonable and cooperative behavior in dealing with customers and the ability to control cost and manage schedules. The Government will consider the collective record of recent and relevant past performance in

Strength An aspect of a proposal that has merit or exceeds specified performance or capability requirements in a way that will be advantageous to the Government during contract performance.

Weakness A flaw in the proposal that increases the risk of unsuccessful contract performance.

Significant Weakness

A flaw in the proposal that appreciably increases the risk of unsuccessful contract performance.

Deficiency A material failure of a proposal to meet a Government requirement or a combination of weaknesses in a proposal that increases the risk of unsuccessful contract performance to an unacceptable level.

determining the Past Performance confidence rating. The Government will attempt to contact references identified in Volume 4 to confirm past and present performance data identified in proposals and may obtain additional past and present performance data from other sources.

Past Performance Confidence Assessments Standards

Satisfactory Confidence

Based on the recent/relevant performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort.

Neutral Confidence

No recent/relevant performance record is available or the offeror's performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned. The offeror may not be evaluated favorably or unfavorably on the factor of past performance.

Limited Confidence

Indicates that the offeror has historically failed to perform in a satisfactory manner in the fulfillment of like or similar requirements. This rating indicates an unacceptable level of performance risk.

(k) Price. Price will be evaluated for reasonableness using analysis techniques at FAR 15.404-1(b)(2).

(1) The total evaluated price will be calculated from the Volume 4 pricing sheet. Calculation of total evaluated price is described in paragraph (b) of this Addendum to 52.212-2.

(2) The evaluation will examine compensation of service employees, as defined in 52.222-41, to assess risk to compliance with Service Contract Labor Standards and provide this to the Program Management factor evaluation. The price evaluation will compare wages, health and welfare benefits, and paid time off for service employees, if any, to the relevant Wage Determinations for performance locations identified in Volume 4.

(3) The evaluation will examine calculation of the percentage of contract value to be performed by the offeror and similarly situated subcontractors by performance period and provide this to the Program Management factor evaluation. If the contracting officer determines that a subcontract NAICS code does not accurately reflect the services to be performed by the subcontractor and that the subcontractor would not be similarly situated under the solicitation NAICS, the evaluation will exclude that subcontract from the contract value to be performed by the offeror and similarly situated subcontractors.
