ATTACHMENT ONE

PERFORMANCE WORK STATEMENT

SAMPLE TASKS

Sample Task One

The contractor receives a five hour notification that a US&R Type One (heavy) task force needs to be moved from Ronald Reagan National Airport (DCA) in Washington, DC, to New Orleans, LA, airport (MSY). Return of the team will be arranged on a later date. A model US&R Type

One (heavy) task force consists of:

Seventy (70) personnel;

Four (4) canines;

73,574 lbs of equipment (1244 cubic feet);

a. Fifty-two (52) - 40" x 48" skids; or

b. Sixteen (16) - 463 L pallets; and

c. Hazmat information will be specified.

Four (4) vehicles (total vehicle weight: 35,800 lbs);

a. Two (2) SUVs (Suburban type); and

b. Two (2) Pick-ups (Ford F-2S0 type) w/utility trailer and two (2) ATVs.

Sample Task Two

The contractor receives a request, 24-hours in advance, that a plane for 8 passengers is required for travel from Ronald Reagan National Airport (DCA) in Washington, DC, to Devil’s Lake, ND

(DVL). The flight will depart DCA at 6 am ET. The return flight from ND will depart the same day at 2 pm CT with a 1 hour one stop over in Nashville, TN (BNA) to pick up an additional passenger.

Sample Task Three

The contractor receives a five-hour notification that the Incident Management Assistance Team

(IMAT) West in Sacramento, CA, needs to deploy to Guam International Airport (PGUM).

Return of the team will be arranged on a later date. A model IMAT is comprised of:

Twenty-four (24) personnel;

2,400 lbs of baggage;

8,516 lbs of equipment (623 cubic feet):

a. Nineteen (19) - 40" x 48" skids;

b. or two (2) - 463L pallets; and

c. Hazmat information will be specified.

Six (6) vehicles per team (total vehicle weight: 45,200 lbs):

a. A communications vehicle (Ford F350 crew cab with a custom body);

b. A communications support van (Ford E-350 standard length);

c. Four (4) GM extended vans which will hold all equipment and baggage;

d. An SUV (Suburban type) ; and

e. An 8' cargo trailer.
