Durable Medical Equipment

Questions and Answer

1. On Attachment 2 requesting assembled kit price on each CLIN – Will all of these line items be added up (w/ delivery cost) to make the total price for 1 kit? Do we need to show a total kit price on this spreadsheet? I have updated the Pricing CLIN Sheet and you will only have to complete the New Pricing Sheet

2. On Attachment 3, I noticed there is no line item for delivery cost. Are all the A La Carte prices going to be destination price? Please disregard Attachment 3 and complete Attachment 2 only.

3. If we provide delivery costs, are they going to be the same across the board to any of the 7 DCs?

Delivery Cost is to be TBD for each location.

4. Is FEMA going to use consolidation points if going outside the CONUS 48? For example, if you need supplies going to Guam, PR, or Hawaii, can we assume we are only responsible for shipping inside the lower 48? See answer above

5. We are estimating that it will take 3-4 skids per push pack and it states that it needs 10 layers of shipping plastic wrap to protect each skid. Do we need to use the large boxes pictured at the bottom of the SOW, or can we wrap all skids without the boxes? You must you the boxes that are required in the SOW.

6. Are you going to need any product information beyond just part # and manufacturer? For example spec sheets, pictures etc. To ensure proper identification of the commodities yes.

7. Is Attachment 4 Past Performance – this looks like it needs to be filled out by a past contractor we worked with on a similar project(s). Is this correct? I can’t make that determination only you would know if a past contractor worked with similar project (s).

8. It appears that the quantities and items in the attached will be the amount for each kit, but we want to confirm that there are no different configurations of the items within separate kits.

No there are not any different configurations

9. WILL THE KITTING BE DONE BY THE SELLER BEFORE SHIPMENT, OR WILL IT BE DONE BY THE END USER AFTER DELIVERY? All kitting should be done prior to delivering to the Distribution Centers.

10. WILL ALL PRODUCTS IN “ATTACHMENT 2” BE IN 1 KIT, OR WILL THERE BE MULTIPLE KIT CONFIGURATIONS? All commodities will not fit into one box, so multiple boxes will be needed.

11. DO THE KITS NEED TO BE STERILE OR NON STERILE? If your speaking of the boxes being sterile “no”, the commodities that are required should be already package.

12. IS THIS SOMETHING THAT WOULD BE IN A WOODEN CRATE, CARDBOARD BOX, OR STEEL

CONTAINER? See attachment

13. HOW MANY DIFFERENT CRATES WOULD BE DESIRED? The boxes are depended upon how you kit them.

14. HOW MANY OF THE CREATES OR KITS WOULD BE DELIVERED, AND HOW MANY WOULD

TRILLAMED NEED TO STORE? See question above

15. DO THE KITS NEED TO BE SEALED AND WATERPROOF/WEATHERPROOF? Yes
