About this file
This is a Financial Proposal attachment for RFP HSCRC-25-0305 issued by the Maryland Health Services Cost Review Commission (HSCRC) for a Financial Policy Review contract. The HSCRC seeks a contractor to assist in adjudicating full-rate reviews, analyzing health systems' cost structures, and assessing proposed changes to methodologies under Maryland Health-General Article 19-201 et seq. The contract is structured as a time and materials project with an indefinite quantity (IDIQ) component. Contractors must complete the financial proposal using a standardized 1000-hour hypothetical project model with fixed hour allocations across 13 rate tiers representing different staff roles, including Subject Matter Experts, Project Leaders, Project Consultants, and Project Administrators. The contract is proposed for a three-year term with an anticipated award value of $4.5 million, though the actual contract award is not guaranteed for the duration of the contract.
Financial proposals will be evaluated based on the total cost of the hypothetical 1000-hour project. Contractors must populate only green-highlighted sections of the attachment and are prohibited from altering the form, adding or deleting rows, or making assumptions, as such actions will result in disqualification. Hourly rates must be entered for all 14 rows and cannot be left blank; rates will be fixed for the three-year contract term and cannot be increased. Contractors must ensure that senior-level roles are appropriately priced relative to junior roles and that cost distribution aligns logically with the designated rate tiers, as bids that artificially deflate total costs through inappropriate tier placement will be disqualified. Contractors must also provide contractor-specific role titles, individual names where applicable, firm information, and affiant details to complete the submission.

Total Cost 1000 Hour Hypothetical Project 1000 $0
Financial Proposal Affidavit Instructions:
DO NOT ALTER THE FORM, OR GIVE ASSUMPTIONS. ANY SUCH INFORMATION WILL BE CONSIDERED A CONDITIONAL OFFER.
1. The contract award will be $4.5 M. Financial Proposals will be evaluated based on the based total cost for the 1000 hour hypothetical project described by the hours shown above. This contract is a time and materials project and has an indefinite quantity (IDIQ) component. The contract award in not garranteed for the term of the contract.
2. Contractors should populate the green highlighted sections only. Do not add or delete rows.
3. Complete your Firm's name at the top left of this sheet. Complete the Affiant information at the bottom of the hourly rate chart. Submit this completed sheet as your Firm's Financial Proposal.
Financial Proposal Affidavit Notes:
a. Rate Tiers indicate the type of individual the HSCRC expects to use these hours. Contractor can place their specific role in Column C, however there should be a general alignment between contractor role, hourly and the rate tier. Generally we would expect the highest cost individuals to be listed in the first 6 rows. It is not permissible to deflate the total cost by entering high cost indidivudals in inappropriately low weighted cells. Such bids will be disqualified.
b. The contractor should enter their job role title in this column. Multiple columns are provided to allow contractor some flexiblity to balance different rates. All rows must be populated, however it is not required that they be unqiue. For exmaple, if the contractor is proposing only one level of project leader simply enter the same category in column C and the same rate in column E for both Project Leader and Project Leader 2. It is also permissible to enter a job category covering mutliple skill sets who will bill at the same rate, e.g. "Statisician/Health Economist", asuming they are all appropriate to the Rate Tier
c. Enter the names of all individual referecened in the contract in this column in the row under which they will bill. Multiple names may be listed in the same cell. This is not a commitment that these indiviuals will work on any specific project but is intended to allow the HSCRC to understand how the individuals in the contract map to the rates. It is not necessary to have named individuals in every tier but is desirable at the more senior levels.
d. Enter the hourly rate at which individuals in this tier will bill, do not leave any row at 0, this will invalidate your bid. The rate will apply for 3 years, it will not be permissable to raise rates during the term of the contract.
e. These columns represents the weight that individual is given in calculating the total cost of the contractor's proposal across the hypothetical 1000 hours. Changing these quantities will result in disqualification. While these cells are representative of the kind of mix of skills the HSCRC expects they do not represent commitments, actual workloads will be managed with the Contractor based on the need of the contract subject to the total spend limitation.
