Quality Assurance Surveillance Plan ALC Calibration Contract
Updated 4/27/2012
1.0 Introduction
This Quality Assurance Surveillance Plan (QASP) is written pursuant to the requirements listed in the Statement of Work (SOW) entitled "Calibration Test and Repair Services of Test, Measurement, and Diagnostic Equipment". This QASP sets forth the procedures and guidelines that the Aviation Logistics Center (ALC) and (ECAS) will use in evaluating the technical performance of the Calibration service contractor.
1.1 Purpose
The purpose of this QASP is to describe the systematic methods used to measure performance and to identify the reports required and the resources to be employed. The QASP provides a means for evaluating whether the contractor is meeting performance standards identified in the SOW. This QASP is designed to define roles and responsibilities, identify the performance objectives, define the methodologies used to monitor and evaluate the contractor's performance, describe quality assurance reporting, and describe the analysis of quality assurance monitoring results.
1 .2 Performance Management Strategy
1.2.1 The contractor's Quality Control Plan (QCP) will set forth the staffing and procedures for self-inspecting the quality, timeliness, responsiveness, and other requirements in the SOW. The contractor must develop and implement a performance management system with processes to assess and report their performance to the COR.
1.2.2 The COR will monitor performance and determine how the contractor is performing based on the requirements of the SOW. The Government will make decisions based on performance data and notify the contractor of those decisions. The contractor will be responsible for making required changes in their processes and practices to ensure performance is effectively managed.
2.0 Government Roles and Responsibilities
The following personnel must oversee and coordinate surveillance activities.
2.1 Contracting Officer
The KO must ensure performance of all necessary actions for effective contracting, ensure compliance with the contract terms, and must safeguard the interests of the United States in the contractual relationship. The KO must also ensure that the contractor receives impartial, fair, and equitable treatment under this contract. The KO is ultimately responsible for the final determination of the adequacy of the contractor's performance.

2.2 Contracting Officer's Representative (COR)
The COR is responsible for technical administration of the contract and must assure proper Government surveillance of the contractor's performance. The COR must keep a quality assurance file. The COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government's behalf.
3.0 Methods of Surveillance
The below listed methods of surveillance must be used in the administration of this QASP. In addition to specific instructions that may be mentioned, the appropriate and standardized form that is to be used for documentation for QA surveillance is the surveillance activity checklist.
3.1 Review of Documentation and Reports
This level of inspection must be accomplished by verifying spreadsheets, calibration certifications, and status reports meet requirements. Each month, the COR will review the generated documentation and enter summary results into the surveillance activity checklist.
3.2 Compliance to Order of Precedence for Calibration Procedures
This level of inspection must be accomplished by monitoring the contractor's AFCAV software version. Each month, the COR will review the generated documentation and enter summary results into the surveillance activity checklist.
3.3 Review of Contractor Documentation and Reports
The COR will review the contractor's system backup documentation, network security report, and property disposal records to ensure actions have been completed as specified in the SOW.
3.4 Observations of Performance
During each month, the COR will randomly observe calibration operations to verify compliance to authorized calibration procedures. During each month the COR will observe contractor calibration activities and quality assurance inspections on-base and at contractor facility randomly selected by the COR. International Organization for Standardization (ISO) auditors may conduct audits and/or process reviews as deemed necessary by the Government. The COR and/or ACOR will be present during any ISO reviews.
4.0 Performance Standards
The Statement of Work (SOW) and this QASP include performance criteria. The Government must use these to determine contractor performance.
5.0 Documentation
The COR will, in addition to providing documentation to the contracting officer, maintain a complete quality assurance file. The file will contain copies of all reports, evaluations, recommendations, and any actions related to the government's performance of the quality assurance function, including the originals of all surveillance activity checklists. All such records will be retained for the life of this contract. Discrepancies will be documented on DD FORM 2772(series). The COR must forward these records to the contracting officer at termination or completion of the contract.
