In the Instructions to Offerors, on Page 46 under the heading “Evaluation”
The RFP refers to sub-factors that are not elsewhere described in the Instructions to Offerors. The RFP says “The Government, after reviewing the sub-factors Organization and Technical Approach; Personnel Qualifications; and Quality Control…” However there is no “Organization and Technical Approach,” Personnel Qualifications,” or “Quality Control” sub-factors. Please clarify where these sub-factors can be found or if they should be replaced with the technical approach sub-factors “Management Approach, Communications Plan, and Clinic Service Provider Network” identified at the bottom of page 44.
See Amended Instructions to Offerors dated April 13, 2017.

We assume that the phrase “Psychological screening should be treated as a component of the Pre-Employment medical examination. Any Pre-employment psychological consultation would be a paper records review only.” means that during the physical examination process the physician would review whatever documentation the patient presents and obtain a mental health history. Is this assumption correct?
See Amended PWS dated April 13, 2017.

PWS Page 12 Section 2.0 Para 6
We assume that the phrase “A “virtual face-to-face” is not specific and would not be appropriate for either a Pre-Employment or FFDE.” means that during the FFDE the patient would require a “Face-to-face” evaluation (not virtual) by a psychologist or psychiatrist. Is this assumption correct?
Correct.

Attachment 2, Pricing Schedule
The quantity for the 66 month total in column AA is incorrect for the following CLINs: 0002AM1, 0002AM3, 0002AM6, and 0002AM8. The quantity shows 99 instead of a calculation which would result in the quantity of 6.5 and the Total amount column (spreadsheet column AB) does not contain a formula to sum the totals from each period. Request revision to correct these entries.
See Amended Pricing Schedule dated April 13, 2017.

SF1449, Block 20 top of page 2 and Section C – E, FAR 52.212-1, page 43 and FAR 52.212-4, page 3
The SF1449 states that “Firm Fixed Price (FFP) and Time and Material (T&M) task orders may be issued.” The RFP states that “It is anticipated that this will be a single award IDIQ and that individual orders will be issued as hybrid task orders.” Please clarify what services will be ordered on a T&M basis? Does this only relate to the seven (7) CLINs which are billed at an hourly rate?
Only the 7 CLINs that state “BILLED AT AN HOURLY RATE” will be billed on a T&M basis and well as the travel CLIN.
