CLIENT AUTHORIZATION LETTER

Offerors should send with their list of references a letter similar to the following authorizing the reference to provide past performance information to the Government.

[Offeror’s Official Letterhead]

Dear “Client:”

We are responding to a US Customs & Border Protection (CBP) acquisition calling for vendors to provide forensic urinalysis in support of its Drug Free Federal Workplace Program, mandated under provisions of Executive Order 12564.

CBP is placing increased emphasis in its acquisitions on contractor Past Performance as a source selection factor and is requiring offerors responding to its solicitations to identify our past and current clients for which we have or are performing work similar to that required by CBP. We request that you participate by completing the attached Past Performance Questionnaire form relative to our performance, and submitting it directly to the individual listed below, for evaluation purposes. In the event that you are contacted by representatives of CBP for information on work that our firm performed, you are hereby authorized to respond to those inquiries.

Lloyd A. Burton

Contract Specialist US Customs & Border Protection lloyd.burton@dhs.gov Fax #: 817-545-1288

We have identified Mr./Ms._____________________________of your organization as the point of contact based on his/her knowledge of our work. Your cooperation is appreciated.
