52.212-1 Instructions to Offerors Commercial Items (JAN 2017) ADDENDUM

L-1 General Proposal Requirements In order to maximize efficiency and ensure fairness during the evaluation process, all Contractors must comply with the following instructions for the proposal format and content. All proposals must unconditionally accept all solicitation terms and conditions without any deviations, conditions, exceptions or red-lines. Proposals that contain exceptions, conditions, red-lines or other deviations from the RFP’s terms shall render the entire proposal unacceptable and ineligible for award.

L-2 Deadline for Receipt of Questions from Prospective Offerors Potential offerors may submit questions, in writing, regarding any aspect of this solicitation by email to Marcus.M.Fields.civ@mail.mil and Keisha.L.Simmons.civ@mail.mil. questions must be received, in writing, no later than 01:00 PM EST, on Friday, 13 July 2018. Only written responses will bind the Government. To alter or change the RFP’s terms, the Government’s responses must be incorporated in the RFP via written amendment to the RFP.

L-3 Proposal Content The Government reserves the right to not evaluate or consider any incomplete proposal. An incomplete proposal is defined as one that is lacking the information required below. An incomplete proposal may be considered unacceptable by the Government. Offerors shall assume that the Government has no prior knowledge of their facilities and experience and will base its evaluation solely on the information presented in the offeror’s proposal.

SITE VISIT:

a) The site visit is MANADATORY and a REQUIREMENT to this solicitation. NO EXCEPTIONS will be made for this solicitation. Please follow the instructions.

b) The Pentagon Reservation site visit/walk will be conducted on Wednesday; July, 11 , 2018 from

10:00AM – 12:00 Noon Eastern Time (EST). Offerors will tour locations associated with this solicitation.

c) Contractors shall submit the names of all attendees ( not to exceed 3 individuals) from each prospective contractor and the following information via email to:

• First name

• Last name

• Email address

• Date of birth (mm/dd/yyyy)

• SSN (if country of citizenship is United States), not required for non-U.S. citizens

• Birthplace

• Gender (m/f)

• Passport number (if applicable)

f) Attendees should bring two (2) ID’s, one of which must be photo identification. It is recommended that individuals traveling to the Pentagon, take the Metro or park at Pentagon City Mall.

g) No questions will be permitted during the site visit; all questions shall be submitted in writing after the site visit.

h) Responses to all questions will be in the form of an amendment and will be posted on FedBizOPPS

(FBO) https://www.fbo.gov/. No telephone inquiries or other oral requests will not be entertained. Only the Government’s written responses to timely written questions that are incorporated in a written amendment to the solicitation.

i) All prospective offerors are permitted to take notes. Photos are not permitted during the site visit.

j) Contractors are urged and expected to inspect the site where services are performed and to satisfy themselves regarding all general and local condition that may affect the cost of contract performance, to the extent that the information is reasonably obtainable.

k) Person(s) attending the Site visit are requested to arrive at the Pentagon and enter through the Visitors

Entrance no later than twenty (20) minutes prior to the start of the site visit in order to allow time to process through security.

l) After entering through the metal detectors/baggage check point, participants should meet the point of contract or his/her representative who will be standing in the rear of the visitor’s area with a sign reading “Pentagon Fire Protection Systems Maintenance Services Requirement”. After gathering, the prospective offeror’s will be processed through security and escorted into the Pentagon to the appropriate site(s). Person’s attending the site visit will be escorted back to the Visitor’s entrance at conclusion of the site visit. All visitors must follow the rules and regulations as well as direction of Government personnel.

Key Personnel

To be eligible for contract award, Offerors must submit resumes and commitment letters of their key personnel, in their proposal that, on the proposal due date, that the key personnel at least meets the minimum qualifications. There will be no exceptions or extensions to this requirement. This key personnel is a PASS/FAIL requirement to be verified by the Government no later than the proposal due date. Any Offeror who key personnel doesn’t meet the minimum qualifications, at proposal due date, shall be deemed ineligible for award and their proposal will not be further considered by the Government.

Volume I: Administrative The proposal shall be accompanied by a cover letter (letter of transmittal) prepared on the company’s letterhead stationery. The cover letter (letter of transmittal) shall identify all enclosures being transmitted and shall be used only to transmit the proposal and shall include no other information. In addition, it shall clearly state the length of proposal validity, which shall be at least 90 days.

The following are further descriptions of the information that shall be provided with the proposal.

Administrative – Cover Letter, Tax Identification Number, CAGE Code and DUNS Number Volume II: Technical Proposal

1) Proposal Contents. All proposals must clearly and convincingly demonstrate that the offeror has a thorough understanding of the requirements and associated risks, and is able, willing, and competent to devote the resources necessary to meet or exceed the requirements. The technical proposal must demonstrate an ability to comply with all requirements in the solicitation. General statements that the offeror can or will comply with the requirements, that standard procedures will be used, that well known techniques will be used, or paraphrases of the RFP’s Performance Work Statement (PWS), will not constitute compliance.

Offeror shall address their technical capability to adequately perform the requirements set forth in the PWS. Further, the proposal shall provide information supporting the offeror’s ability to meet contract requirements in the areas listed below.

2. Proposal Length. The Technical Proposal must not exceed 35 pages total for all three sub-factors, single-sided, (Excluding: blank pages, title pages, tab indices, and table of contents). Each page shall be 8 ½ x 11 inches, double-spaced, no smaller than 11 point font, with one (1) inch margin requirements.

Pages shall be numbered and paragraphs identified by commonly used and consistent system to assist in referencing specific areas of the proposal. Pages shall also have a header or footer that contains at a minimum, contractor name and solicitation number. Enclosures must be identified on all pages. Pages that exceed the page limit shall be removed from the proposal and not considered or evaluated.

Attachments or exhibits referenced in technical proposal shall be counted in the total page length.

Volume III: Past Performance Page Limit. There is no page limit for Attachment #3 J-L1, Client Authorization Letter. However, offerors shall submit no more than 2 pages for each past performance data submission, Attachment #4 J-L2, Past Performance Data Sheet.

1) Offeror shall submit a Past Performance Data Sheet, (Attachment #4 J-L2 Past Performance Data

Sheet) , for up to three (3) Government or commercial contracts for services directly related or similar to the scope of work described in the PWS. The prime contractor may submit one (1) past performance data sheet for each primary subcontractor provided the total references for the prime contractor and subcontractor do not exceed three.

2) The offeror shall complete Section 1 of Attachment #5 J-L3, Contractor Performance Evaluation Survey and send it to three (3) references/customers for the prime contractor or subcontractor identified in Attachment #4 above. As stated in Attachment #5, the reference will complete this form and return it directly to the Government by the solicitation proposal due date.

Volume IV: Price Proposal

1)There is no page limit for this volume 2)The price proposal shall consist of the following:

a) Completed and signed Attachment #2 J-B1 Pricing Submission Worksheet

b) Written acknowledgement of all amendments that may be associated with this solicitation

c) Complete the pricing sheet J-B1, Pricing Submission Worksheet. For each labor category, provide a fully loaded hourly rate & monthly rate for the base and all option periods. This information shall be incorporated into the successful offeror’s contract as an attachment and shall form the basis of pricing all work under the contract. Any discounts offered must be explicit.

Volume V: Small Business Participation Plan requirements and substantiating documentation to demonstrate how you will meet these requirements. This required information will be used to evaluate the extent of your proposed participation/commitment to use U.S. small businesses in the performance of this acquisition (as small business prime offerors or small business subcontractors) relative to Section M-3, Factor III:Small Business Participation. These SBPCD requirements will be incorporated into any resulting contract(s) as specified below.

The small business concerns (SBC) as defined in FAR Part 19 that are to be used in your proposed approach to satisfying the requirements of this solicitation for the entire performance period, including company name, business category(ies), description of products or services and NAICS code to be provided by each SBC and the nature of the commitment with the SBC (i.e., letter of commitment, joint ventures, mentor protégé agreements, others). Binding commitments shall become enforceable, contractual requirements. Use the table/format below in proposing this required information. (Note: Section M, Factor

L-5 Proposal Delivery Offerors must electronically deliver proposals by the required proposal due-date/time. Electronic documents must be separated into volumes as indicated in the instructions above. All electronic documents must be legible with proper signatures in PDF. There will be NO exceptions for failure to electronically deliver, no alternative documents will be accepted once an original electronical document is submitted. Timely submission of electronic documents are the offeror’s responsibility. WHS must receive the questions NLT Friday, July 13, 2018, at 01:00PM EST. Please use the Question sheet found in Section J Attachment #9 Questions- to submit questions. WHS will answer questions which may affect proposals in an amendment to the solicitation. WHS will not reference the source of the questions. To change or be included in the solicitation the answers to any questions must be incorporated in a written amendment to the solicitation.
