About this file
This document is a communication from the Massachusetts Rehabilitation Commission (MRC) regarding the required verification process for a Criminal Offender Record Information (CORI) Form. The CORI Form must be completed and verified by an MRC staff member, who will review the applicant's government-issued photo ID and attach a copy to the form. The completed and verified CORI Form should then be submitted to the MRC Contracts Department.
The document is related to a state and local contract opportunity issued by the MRC for Vehicle Modification Services. The MRC is seeking responses to procure modifications to vehicles for eligible individuals with severe disabilities who require motor vehicle modifications to drive independently or be safely transported as passengers. The contract opportunity is specific to the state of Massachusetts.
