STATEMENT OF WORK

FOR

Holloman AFB, HEAVY EQUIPMENT RENTAL

03 Dec 2020

Part I – General Contract Requirements

1. General Requirements.

1.1. The work to be accomplished shall include; delivery, limited field service (Reference section 3 of SOW), and pick-up of heavy equipment. Operators are not included.

1.2. Area for delivery shall be Holloman Air Force Base, New Mexico 88330 in a staging area within the 635 MMS Compound, to be further designated by Point of Contact

(POC) at the time of delivery.

1.3.

1.4. Delivery and Pick Up of heavy equipment shall be coordinated with the Contractor and the POC two (2) business days in advance between the hours of 7:00 am and 7:00 pm

Monday-Sunday. Meeting time between POC and the Contractor for delivery and pickup shall be agreed to seven (7) days in advance to minimize inconvenience to both parties. Equipment shall be delivered through the Commercial gate to be scanned.

1.5. Contractor shall deliver all equipment with a full tank of fuel and it will be returned by the Government with a full tank of fuel.

1.6. Initial delivery and a pick-up inspection shall be conducted at the delivery site and will include pictures and an inspection log detailing existing discrepancies as agreed upon by both parties. Acceptance for the delivery shall be made after proper documentation (i.e.

pictures, list of damages, and other paperwork required to clearly document the equipment’s general condition) is received by the POC.

1.7. All deliveries under this agreement shall be accompanied by an original delivery ticket or rental slip with the number of hours on the piece of equipment listed. All equipment replacements and changes throughout the life of the contact will be updated and annotated on new delivery tickets. Contractor shall provide a copy of said ticket or rental slip to the POC on site at the time of delivery. It shall contain the following:

2.1. On/ About Timeline of use is as follows: Late February - April: All equipment shall be in place 15 March 2021, picked up 28 April 2021.

2.2. All equipment shall operate off of DSS (construction grade) Diesel fuel.

2.3. The projected usage included above are estimates per individual piece of heavy equipment.

2.4. At the time of delivery, all equipment shall be in good, working order in accordance with all applicable manufacturers’, OSHA, and DOT requirements.

3. Field Service (Repair & Preventative Maintenance).

3.1. Contractor shall schedule the delivery and all maintenance of heavy equipment between the hours of 7:00 A.M. – 7:00 P.M. Monday-Saturday, excluding federal holidays and

Sundays. In the event a repair is required, the contractor shall repair within 48 hours from the time the repair call is placed. Contractor shall provide a letter containing a point of contact’s name and phone number for a maintenance contact.

3.2. Contractor shall provide the name of a qualified mechanic, licensed by the State of New

Mexico to complete field service repairs, as required, in accordance with the timelines specified above.

3.3. If field service is determined by the POC and the Contractor to be impractical, or the rented equipment remains inoperable, Contractor shall provide a suitable substitute, acceptable to the POC, within 24 hours.

3.4. All repairs due to normal wear-and-tear in operation of heavy equipment will be the responsibility of the Contractor. Normal wear and tear is defined as tire tread wear, body panels coming loose, rock chips, leaks and other issues occurring during operation of the vehicle in accordance with the manufacturer’s published operation’s manual. If it is deemed that damage is caused by negligence or operator error, the Contractor shall provide a minimum of three (3) repair quotes to the contracting officer for reimbursement.

3.5. Contractor is responsible for all routine preventative maintenance associated with the equipment rentals and shall provide a detailed maintenance schedule for each individual piece of equipment at the time of delivery. If the preventative maintenance requires a piece of equipment to be down for more than eight (8) hours the Contractor shall provide a suitable substitute, acceptable to the contracting officer’s designated representative during the maintenance period.

3.6. Contractor shall remove all product waste associated with the field services provided.

3.7. Contractor shall provide a daily inspection checklist for each piece of equipment.

3.8. Contractor shall provide all items for operator maintenance (i.e. grease, grease guns, etc.) and be responsible for disposal of these items.
