II. GENERAL INSTRUCTIONS
The following are general instructions for the completion and submission of the BFCC-QIO Contract business proposal.
1. The CMS-provided electronic file titled BFCC-QIO Contract and the specified Excel 2007 and Word 2007 versions are the only acceptable formats for your cost proposal submission. Proposals received in any other format are unacceptable.
The business proposal spreadsheets are formatted to accept one decimal place for entries relating to hours, and whole dollars for entries relating to costs. However, where appropriate (e.g., hourly rates), the formatting will permit 2 decimal places.
2. Offerors proposing a BFCC-QIO Contract in more than one area must complete a separate business proposal for each area.
3. Offerors are to fill in only the unshaded cells. All blocks on the forms that are shaded auto-fill using a formula from within the form or from other forms and are for CMS’ use only. Thus, shaded cells shall not be filled out by the offeror.
