STATEMENT OF WORK - REVISED
Attachment 1
STATEMENT OF WORK
FOR
BLADES, GRINDER PLATES AND KNIVES, AND SHARPENING
1. SCOPE OF WORK: The contractor shall provide all personnel, supervision, supplies, tools, parts, transportation, mailing expenses, and other items and services necessary to provide sharp blades, plates and knives. The grinder plates and knives will be provided on a rental/exchange basis and will remain the property of the contractor. The saw blades, once accepted, will become the property of the Government.
2. DEFINITION: For purposes of this SOW, the acronym “SD” means the Store Director or designated representative.
3. NOTIFICATION: Within ten (10) days of contract award, the contractor shall provide the SD or their designated representative, at each store location, with a designated point of contact. The SD is hereby designated to participate in the administration of this contract to ensure the contractor’s compliance with its rental/exchange services.
4. PERSONNEL & SECURITY: Contractor personnel must be recognizable as such while in the commissary by means of uniform or identification badge or patch. The contractor shall be responsible for obtaining installation access, if required, for vehicles and personnel as needed. Contractor personnel shall observe all personnel security directives in place at the commissary, such as entrance/exit doors, sign-in/sign-out procedures, etc.
5. CONDITIONS OF EXCHANGE: If the rental/exchange service is performed in person at the commissary, such service shall be performed Monday through Friday between the hours of 8:00 a.m. through 5:00 p.m., excluding Federal Government Holidays as explained in paragraph 6, and other days throughout the year when the commissary is closed, i.e. inventory days, etc. Commissary hours can be located at www.commissaries.com. If the rental/exchange is completed by mail, the contractor shall provide all packing materials necessary to protect items being shipped, pre-printed mailing labels, and postage (i.e. the government will incur no cost related to monthly/quarterly exchange by mail). Payments will be made using the store’s Government Purchase Card (GPC).
In addition, if the contractor arrives at the store (the same applies to an exchange taking place via mail) to make the exchange, and rental items are missing, the store is responsible for the costs to replace the missing item(s). The delivery ticket should be marked accordingly (see Section 9 for more information).
6. LEGAL PUBLIC HOLIDAYS: Reference Title 5, United States Code (U.S.C.) 6103(A).
New Year’s Day Labor Day Martin Luther King Jr.’s Birthday Columbus Day President’s Day Veteran’s Day Memorial Day Thanksgiving Day Independence Day Christmas Day
When such holidays fall on Saturday, the preceding Friday will be considered a holiday.
When such holidays fall on a Sunday, the following Monday will be considered a holiday.
7. REQUIRED SERVICE: The Contractor shall provide:
Grinder Knives and Plate Rentals: on a rental/exchange basis, sharpened grinder plates and knives of the type, in the quantities, and at the frequency listed in the schedule of supplies/service of the contract unless changed by the commissary two weeks prior to their scheduled quarterly service. The contractor will coordinate the initial delivery with the SD or their designated representative 14 days prior to the effective date of this contract.
Blade Purchase and Blade Sharpening: The purchasing of meat cutting saw blades and sharpening of already owned Biro and Treif blades will be on an as needed basis. Sharpening should be accomplished in accordance with industry standards.
*All products provided including sharpened Biro and Treif blades must be safe for the intended use of processing commercial grocery meat operations.
Delivery for Grinder Knives and Plate Rentals: The initial quarterly DELIVERY shall be performed not more than twenty-one (21) days, after contract start date (no later than October 21, 2017) and thereafter not less than 70 days, nor more than 100 days, from the last quarterly service. This does not change the date in which the quarter begins or ends. The 70 to 100 days is a grace period to allow for the contractor to make all of their quarterly deliveries.
Quarters are not subject to change due to when deliveries are actually made (Quarterly deliveries are expected to be delivered within 7 days of beginning a new quarter - October, January, April, and July). Date and time of scheduled service should be coordinated with the SD to minimize disruption of commissary operations. The purchasing of the Saw Blades and the Sharpening of the Biro and Treif Blades will be on an as needed basis and maybe coordinated to coincide with the rental exchange if this meets the needs of the commissary.
8. INSPECTION/ACCEPTANCE: Monitoring deliveries will be accomplished on a 100% inspection basis immediately after delivery (this will normally be within one day of use). When the SD determines the blades, plates or knives do not meet the requirement of this contract, the contractor will be notified. The contractor shall deliver the correct items within twenty-four hours from the time the commissary notifies the contractor of the problem if an urgent requirement where the store does not have the immediate supplies needed for operations. If the need is not immediate, the corrective action will take place within three business days.
9. DELIVERY TICKET: The contractor shall furnish a delivery ticket at time of each purchase and/or exchange of plates and knives. Delivery tickets shall contain but is not limited to the contract number, bill to, and ship to address, quantity and description of product, unit price, and the amount. In order to expedite payment, the delivery ticket MUST be validated by the signature of the contractor. One copy of the validated delivery ticket shall be left with the SD or designated representative. SD or designated representative must also sign delivery ticket. If delivery ticket is validated with both signatures (Contractor and SD or designated representative), delivery ticket can be used as the receiving report and payment can be authorized. If delivery ticket does not reflect both signatures, a receiving report is required before authorizing payment.
10. VARIATION IN QUANTITY: Quantities of rental items may increase or decrease during the term of the contract. There is a possibility of a 20% overall variation in quantity over the course of the contract. The first period of performance will be funded at time of award. The total amount will be computed by the contracting office and added to the Listing attachments for each location. The contractor will not charge for an increase in quantity unless quantities are actually ordered, delivered, and accepted. Prices for any increases or decreases in quantity shall be in accordance with the contractor’s awarded unit prices on the supply listing attachments.
11. ADDITIONS/DELETIONS:
The Government may issue a bilateral modification to incorporate the additional items. Only the Contracting Officer has the authority to grant increases and decreases in quantities beyond the 20% variation. All additions and deletions involving quantities shall be in accordance with the contractor’s awarded prices on the Supply Listing Attachments.
(1) In the event that a commissary’s requirement exceeds the 20% overall variation in quantity,
(2) OR in the event the commissary requires additional items other than those originally shown on the supply listing attachments.
12. INVOICES: Invoices must be submitted to the address for each commissary is identified within Exhibit A.
