QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)

1. Contract or Task Order Title HC109120PL81002 : Weekly Shredding of paper contract for Scott DITCO

Building 3600.

2. Work Requirements: Shredding standards

Task 1 - Provide Weekly schedule

Task 2 - Shredding Task

3. Primary Method of Surveillance:

Periodic Inspection: These services are monitored weekly to monthly periodic to see that Shred bins are picked up and shredded weekly on time at least 90% of the time. Revisions that occur are minor and are resolved in a satisfactory manner.etc.

4. Scope of Performance: Proceed to award contract FY2020 with 4 option years

6. Performance Requirements. The contractor shall provide 2 shred bins 80-95 gallons each, 4 wheel role arounds containers, with slots on top to drop in paper and that the containers are lockable. Collect weekly and paper shredded on site at DITCO 2300 East Drive, Bldg 3600 Scott AFB, IL 62225. The contractor shall pro-vide a letter of certification of destruction weekly.

6.1 Task 1 - Shredding standards.

6.1.2 Subtask 2 – Shredding Task. Performance and schedule requirements will Provide a monthly status report monitoring the quality assurance, progress/status reporting and program reviews applied to see the shredding is being performed weekly to the performance requirements listed in this PWS.

The contract reference is Paragraph 6.

5. Performance Standards:

Performance Standard Acceptable Quality Level (AQL) Method of Surveillance

Subtask 6.1.1 Provide sched-ule for Weekly pick up of pa-per and shred on-site

Performance occurs with no re-quired re-performance or re-work at least 90% of the time. Problems that occur are minor and are resolved in a satisfactory manner.

Routine inspection of services.

Subtask 6.1.2 - Shredding material will be picked up on time.

Shred is picked up and shredded weekly on time at least 90% of the time. Revisions that occur are mi-nor and are resolved in a satisfac-tory manner.

Routine inspection of the shredding of materials in done properly on-site.

6. Mandatory or regulatory compliance items:

The contractor will provide initial proof of compliance with regulatory and compliance items contained in the contract. The Contracting Officer's Representative (COR) and the Contracting Officer (KO) will evaluate the initial submission and will conduct ad hoc reviews of the contractor's compliance throughout the remainder of the contract.

The contractor will monitor compliance as required by FAR 52.222-50, Combating Trafficking in Persons, and re-port any issues to the KO.

7. Acceptable Quality Level (AQL):

Shred is picked up and shredded weekly on time at least 90% of the time. Revisions that occur are mi-nor and are resolved in a satisfactory manner.

8. Evaluation Method:

The COR will document the time of verbal notification to the contractor. The COR will document the official time and date of notification on the Maintenance Call Record. The COR will review self-diagnostic systems logs, conduct a comparison with actual maintenance performance and otherwise verify and validate contractor performance. The contractor shall enter in the record the official time the system is restored to full operational status. The COR will confirm the date and time of problem resolution in the record.
