QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)
1. Contract or Task Order Title: EMSS Raised Flooring
2. Work Requirements:
· The contractor will ensure specified computer-room hard laminate floor tile will correctly fit into the existing understructure where old flooring material will be removed from. No new construction or changes to the floorplan is being made and therefore the new tile shall be directly installed back into the existing understructure without the need for reconfiguration. Adjustments will be required as necessary to ensure the tiles fit properly, level, and evenly. Some of the new tiles will need to be cut to fit in locations where obstructions exist. Examples of this include, but not limited to, walls, ducts, pillars, conduit, fire suppression system, communication cables, and electrical panels. In locations were communication racks or cabinets exist, there will need to have openings cut into the tiles so that electrical cables and communication wires can pass through it. Cut floor tiles edges shall have grommets, trim, or other means to protect the power cables and communication wires from being damaged.
· Phase one: the contractor will be responsible for uninstalling old flooring material from approximately half the room. The existing cove base and threshold will be reused and therefore removal of the old floor material will need to be done so without damaging them. The contractor will then install the new floor tile in the preexisting understructure. Adjustments to the understructure will need to be made as necessary. Custom cutting of the tiles will also be needed at various locations as indicated in step 1. Deliverables in this phase of the installation include approximately half of the floor panels.
· After the contractor has completed installing the first half of the room, 1-2 days will be required for EMSS personnel to move articles from old half to the new half of the room. In addition, EMSS personnel will coordinate moving power cables and communications wires from the old floor and relocating it and the cabinet to the new side, through access hole prepared by contractor. Once this work is complete, phase two can begin.
· Phase two: the contractor will return to the site to continue the installation. The contractor will continue uninstalling the flooring material from the second half of the room, avoiding damage to the cove base and threshold. Once clear of all old floor tiles, installation of the new tile will continue to be installed in the preexisting understructure in the same manner as in phase one. Custom cutting of the tiles will be needed at various locations as indicated in step 1. Deliverables in this phase includes installing the remaining floor tiles.
· The contractor shall furnish the final deliverables of 165 ea. Peel and Stick Static Smart carpet tile. Color: Livingstone.
3. Primary Method of Surveillance:
· 100 percent visual inspection will be the method of surveillance
4. Scope of Performance:
· EMSS staff will prepare the room by moving all server-racks, communications cabinet, computer tech stations, and other non-fixed items to half of the computer room exposing other half to be worked on in first phase of this project. The contractor will uninstall old flooring material from exposed part of the computer room and install new computer-grade hard laminate floor tile in the preexisting understructure. Upon completion of the first half, EMSS staff will return to move the items to the section with the new floor exposing the remaining old floor. At this point, the contractor can resume uninstalling the old flooring material and complete the installation.
· The installation of the new hard laminate floor tiles will require custom cutting at various locations so that the new tiles can fit in the old location where the old tile came out from. These include, but not limited to along the walls, around pillars, electrical and communications conduit, A/C ducting, fire suppression system, computer tech stations, server racks and communication cabinets. Factory grommets or other secure, durable, and visually appealing edge protection is required to be installed over cut portions of the tiles for esthetics as well as to protect the integrity of what is required to pass through it.
· The location of the work area is on the second floor. The building does not have an elevator or any type of equipment lift. There is no access to the inside of the building other than from the first floor and therefore all new/old material and tools will need to be carried to the location via the stairs.
· The contractor will be responsible for ensuring that new flooring material will be a direct fit into pre-existing understructure prior to purchasing material. A potential floor tile that may be compatible but will need to be verified by the contractor is TATE CONCORE 1250 panels measuring 24” x 24”. Factory applied 1/16” high pressure laminate surface with static dissipative properties and “Grey Starlite” color or similar lite grey or light blue finish is the desired choice of color.
· The contractor will be responsible for removing, disposing, and/or recycling of all waste material including packaging and shipping material, old flooring panels, scrap flooring waste, and old or unwanted hardware.
5. Performance Standards:
· Installation of the floor tile shall be uniform, level, and properly fitted with the rest of the adjoining tile
· Holes cut into floor tiles shall be installed with grommets, trim, or other means to protect the power cables and communication wires from being damaged
· Old floor tiles, packing material, and other waste shall be removed and disposed or recycled by contractor
6. Mandatory or regulatory compliance items:
· The contractor will provide initial proof of compliance with regulatory and compliance items contained in the contract. The Contracting Officer's Representative (COR) and the Contracting Officer (KO) will evaluate the initial submission and will conduct ad hoc reviews of the contractor's compliance throughout the remainder of the contract. The contractor will monitor compliance as required by FAR 52.222-50, Combating Trafficking in Persons, and report any issues to the KO.
7. Acceptable Quality Level (AQL):
· The AQL for this project is 100% due to the hazardous nature imposed if the installation is not done correctly and according to specifications.
8. Evaluation Method:
· Installation will be evaluated by visual inspections to ensure tiles are level, straight, and sturdy. In addition, areas around walls and other obstacles where custom modifications are required shall be esthetically appealing. Cut lines shall be straight and places where cables or other fragile items pass through, it must be free of jagged or sharp edges. Edging or grommets should be used to mitigate potential sharp or jagged edges.
9. Incentives (Positive and/or Negative):
The following negative incentives apply in event of incomplete or unsatisfactory work:
· Withhold final payment until work is complete to government satisfaction.
