DISA ON-SITE MAINT
QASP
QUALITY ASSURANCE PLAN (QASP)
DISA On-Site Hardware Maintenance
As of 25 January 2012
1. Title. DISA On-Site Hardware Maintenance
2. Work Requirements.
· Item 1 – Equipment Readiness/OEM Specifications
· Item 2, 3 – Response and Repair Time
· Item 4 - Response and Repair Outside of Principle Period of Maintenance (OPPM)
· Item 5, 7 – Provide Diagnostics and Call Home
· Item 6 - Engineering Change Proposals (ECPs)
· Item 8 - Predictive Maintenance
· Item 9 - Preventative Maintenance
· Item 10 - Other Services
· Item 11 – Best Effort T&M
· Item 12 – Emergency Maintenance
· Item 13 - Inspections and Certificate of Maintainability (COM)
· Item 14 - Monthly Reports
3. Primary Method of Surveillance.
Periodic Inspection, as required, is scheduled for Task 1 - 14. There will be 100% inspection for Task 1 – 14 when hardware maintenance support is required.
4. Scope of Performance.
The Contractor shall provide hardware maintenance support on the products identified within the contract. All terms shall be in accordance with the requirements of the contract.
5. Performance Standards.
Performance standards are identified below.

6. Remedies.
· During inspection, if performance thresholds are not met, equipment will be removed from contract and placed on a different contract IAW termination clauses found elsewhere in the contract.
· In the event the contractor is unable to repair the equipment within the performance thresholds and the Government must obtain service elsewhere (elevation to the OEM or other vendor), the contractor invoice will be reduced by the invoice amount of the Government obtained repair service.
7. Acceptable Quality Level (AQL):
The ACL for this project is 100% due to the critical support required for hardware maintenance for DISA processing...
8. Evaluation Method:
The COR will review the “ADPE Maintenance Record/FORM 7” to document the time elapsed between the initial call for support of a hardware maintenance/repair requirement, voice and physical response by the contractor and resolution to verify and validate contractor performance.
The “ADPE Maintenance Record/FORM 7” is defined as the official documentation from the
Government that hardware maintenance/repair is required. The “ADPE Maintenance Record/Form
7” captures the time the hardware error occurred, when the Government requests support from the contractor, and the time that the hardware error is resolved. The “ADPE Maintenance Record/Form
7” is the official measure of response and resolution time.
