QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)

16 October 2015

1. Contract or Task Order Title: Defense Information Systems Network (DISN) Optical Network Enhancement Link 39

2. Work Requirements:

The Performance Work Statement (PWS) defines the requirements to provide dark fiber, equipment, and commercial facilities to support Department of Defense (DoD) users in the Continental United States (CONUS). The PWS further defines requirements for the Contractor to install, integrate, and test Optical Line Amplifier (OLA), Junction, and service delivery node (SDN) equipment and provide Operations and Maintenance (O&M) support in commercial fiber facilities. The Performance Task Areas from the PWS include, but are not limited to:

· Task 1 - Project Management

· Task 2 – Implementation of a new 100G Packet-Optical Transport Network (P-OTN) Segment

· Task 3 – Operation and Maintenance and Sustainment services for fiber and associated assets

3. Primary Method of Surveillance:

Periodic inspection is scheduled for Task 1. There will be 100% inspection for Tasks 2 and 3.

· 100 Percent Inspection. This is usually only the most appropriate method for infrequent tasks or tasks with stringent performance requirements, e.g., where safety or health is a concern. With this method, performance is inspected/evaluated at each occurrence. One hundred percent inspection is too expensive to be used in most cases.

· Periodic Inspection: These services are monitored weekly, monthly, quarterly, semiannually, annually, etc. Periodic types of activities are perfect for periodic inspection because not only are they infrequent, but there is normally a predetermined, specified time frame within which the tasks must be accomplished. Therefore, you know exactly when to conduct the evaluations. Periodic inspections automatically become 100 percent evaluations or "100 percent checks." Inspections should be divided and scheduled by frequency: annual, semiannual, quarterly, monthly, weekly and as required. Sometimes services are required for which the time or frequency cannot be predicted, such as accident investigations, one-time special tasking by higher headquarters, etc. These would be labeled "as required inspections." Others are known and predictable such as the quarterly status report or the monthly travel report currently included in some DISA service contracts.

4. Scope of Performance:

The Contractor shall Engineer, Furnish, Install, Secure, and Test (EFIST) and make operational a new 100G 6500 P-OTN consisting of fiber, amplifier and SDN locations. The Government, in conjunction with technology refreshing its current transport network to 100 Gbps (P-OTN), will be adding new sites with the same capabilities and monitoring systems.

5. Performance Standards:

The Contractor shall adhere to all prevailing telecommunications industry standards during the execution of the PWS.

Performance Standard
Acceptable Quality Level (AQL)
Method of Surveillance
Deliverables are submitted on time and the Contractor manages performance of the overall requirement in accordance with the Project Schedule. Contractor ensures performance of all tasks meet the requirements of the PWS and standard DISA requirements (e.g. TSRs, SCLs).
Deliverables contain required information and are delivered on time at least 90% of the time. Revisions that occur are minor and are resolved in a satisfactory manner.

Schedules and provided implementation plans are accurate 90% of the time. Issues that occur are minor and are resolved in a satisfactory manner.

Periodic inspection of deliverable products and services.

OLA installations will conform to accepted Telecom practice for other installations in contractor facilities
All installation will be of the same quality as similar equipment in the same facility
100% inspection of installations will be conducted.
Testing and test data will meet manufactures requirements to ensure the equipment functions as designed
All test data will be provided to the government engineering team and GNSC for review and historical records.
The Government will perform 100% inspection of the test data to ensure it complies with equipment manufactures requirements.
System integration will be acceptable when the link passes the 72 hour Bit Error Rate (BER) test with no errors
All test data will be provided to the government engineering team and GNSC for review prior to any service being activated.
100% inspection of and acceptance of all test data prior to any service activation.

Sustainment of fiber, colocation facilities, and equipment:

· System availability in accordance with PWS is 99.9%.

· Response times for cable failure should occur within 2 hours.

· Repair times for fiber outages are within 8 hours.

· Response time for equipment failure within 8 hours after GNSC notification.

· Restore time for equipment failures at co-location facilities within 16 hours (from arrival to restoration).

Due to the critical services to be provided, performance occurs with no required re-performance or re-work 99.9%
100% Inspection

6. Mandatory or Regulatory Compliance Items:

The Contractor will provide initial proof of compliance with regulatory and compliance items contained in the contract. The Contracting Officer's Representative (COR) and the Contracting Officer (KO) will evaluate the initial submission and will conduct ad hoc reviews of the Contractor's compliance throughout the remainder of the contract. The Contractor will monitor compliance as required by FAR 52.222-50, Combating Trafficking in Persons, and report any issues to the KO. The COR or KO reserve the right to perform oversight and surveillance of Contractor performance at any time, but not as a disruption or impedance to Contractor performance.

The Contractor shall be notified immediately (verbally if possible, but always in writing) of any discoveries requiring urgent attention/remedy/resolution. The COR will monitor outstanding issues until resolution. The COR shall document all notifications, discussions, and/or transmissions on contract matters.

7. Acceptable Quality Level (AQL):

The AQL’s for all tasks are defined in the table above in Section 5, Performance Standards.

8. Evaluation Method:

The Contracting Officer’s Representative (COR) will review outage reports and determine if credits are applicable. The COR will document the official time and date of notification of repair requirements and the official time and date when the system is restored.

9. Incentives:

Task Areas 1:

For any failure to meet the Acceptable Quality Level in submitting deliverables and any failure to meet the delivery date in accordance with the implementation schedule, the Government may seek consideration, which may include, but is not limited to, reductions in contract price.

Task Area 2:

If system availability is less than required by the PWS and/or Fiber Requirements Specification (FRS - Attachment J3) the Contractor shall grant the Government an availability credit in accordance with Section H of the contract. The availability credit shall be a percentage amount of the total monthly charge, which is determined by subtracting the actual percentage of availability attained during the month from the required percentage of availability. The actual link availability percentage will be calculated monthly by dividing the hours of actual operational use time attained during the month by the maximum operational use time for the month.

Task Area 3:

If response and/or repair times for fiber/equipment failure are longer than required by the PWS and FRS, the Contractor shall grant the Government a maintenance credit as specified in Section H.

Both positive and negative performance will be documented in past performance reports/database, (Contractor Performance Assessment Reporting System (CPARS), as appropriate and will be taken into consideration for follow-on work.
