PAST PERFORMANCE QUESTIONNAIRE

COMMERCIAL BROADBAND SATELLITE PROGRAM

1. Past Performance Assessment. The Defense Information Technology Contracting Organization (DITCO), Scott Field Of-fice, a component of the Defense Information Systems Agency (DISA), is conducting a source selection for indefinite deliv-ery/indefinite quantity (ID/IQ) Commercial Broadband Satellite Program contract(s). An Offeror has identified you as a point of contact concerning relevant past performance. As a result, please complete this questionnaire at your earliest convenience and return it to:

3. Contract Relevancy. For the purposes of determining contract relevancy, please identify which of these CBSP task areas that appear similar to the work performed under the contract identified in Section 2, above (check all that apply).

Task Area 1 - Space Segment: Defined as the commercial satellite resources needed to establish connectivity between the CBSP user terminal segment (shipboard) and the CBSP shore segment (Land Earth Station).

Task Area 2 - End-to-end Service: Defined as a full-period full-duplex connection or circuit between the Gov-ernment’s designated Point of Presence (PoP) ashore and the end user terminal (shipboard) through a means of satellite, Land Earth Station (LES) and terrestrial telecommunications connectivity. The end-to-end service includes items that span the three segments (user terminal, space, and shore).

Task Area 3 - Shore Segment: The CBSP shore segment consists of a number of geographically separated commercial land earth stations (LES) and the terrestrial telecommunications connectivity between them and desig-nated Navy points of presence (PoP). The CBSP shore segment supports the terminal segment by providing the necessary link between CBSP space segment and Navy communications networks ashore.

Task Area 4 - Operational Support: Defined as the professional services provided by the vendor to perform and/or monitor end-to-end services to ensure operation and integrity of the end to end system and interface with Navy operational staff.

Task Area 5 - Security and Information Assurance: Defined as a means to ensure that the vendor support will include all personnel, system, administrative, operational, and physical security requirements defined as appropriate to safeguard Navy information yet still execute all contract objectives.

Task Area 6 - Management Approach: The extent to which the vendor demonstrates the capability, organiza-tional structure, personnel and management plan to successfully execute the program, including management of subcontractors, if applicable. Also, the extent of participation of small business concerns in performance of the con-tract.

4. Performance Assessment Values. Use these definitions when completing the assessment elements in Section 5.

4 - Exceptional. Highly relevant past performance in all evaluated efforts. Performance met contractual requirements and exceeded many to the Government’s benefit. The contractual performance of the element or sub-element being as-sessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effec-tive.

3 - Very Good. Highly Relevant past performance in most evaluated efforts. Performance met contractual requirements and exceeded some to the Government’s benefit. The contractual performance of the element or sub-element being as-sessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

2 - Satisfactory. Highly relevant past performance in evaluated efforts. Performance met contractual requirements. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were satisfactory.

1 - Unsatisfactory. Few relevant past performance efforts evaluated. Performance did not meet most contractual re-quirements and recovery in a timely manner is not likely (applies to ongoing efforts). The contractual performance of the element or sub-element had serious problem(s) for which the contractor took no corrective actions or those taken were ineffective.

0 - Not Applicable. No performance record is identifiable. Completely lacks recent and relevant performance history or past performance is unavailable.

5. Assessment Elements. Please BOLD and underline (or if returning by fax or email, please circle or underline) the appropri-ate number for each item on the questionnaire and provide supporting narrative.

a. Quality of Product or Service. Rate the Offeror’s conformance to contract requirements, specifications, quality of services, and standards of good workmanship (e.g., commonly accepted technical, professional, environmental, or safety and health stan-dards.)

b. Schedule. Rate the Offeror’s timeliness against the completion of the contract, task orders, milestones, delivery schedules, and administrative requirements (e.g., efforts that contribute to or affect the schedule variance.

c. Business Relations. Rate the Offeror’s integration and coordination of all activity needed to execute the contract, specifically the timeliness, completeness, and quality of problem identification, corrective action plans, proposal submittals, the contractor’s history of reasonable and cooperative behavior, customer satisfaction, timely award and management of subcontracts, and whether the contractor met small business, small disadvantaged business, small business HUBZone, veteran-owned small busi-ness, service disabled veteran-owned small business, and women-owned small business participation and subcontracting goals.

d. Management of Key Personnel. Rate the offeror’s key personnel, to include adequacy of training and level of expertise, as well as the offeror’s ability to select, retain, and when necessary, replace, key personnel.

e. Socioeconomic Goals. Rate the offeror’s ability to meet small business subcontracting goals, if applicable.

f. Subcontracting Management. Rate the offeror’s ability to manage its subcontracting responsibilities, considering the offeror’s record relative to the timely award of subcontracts and payment of subcontractors along with any significant subcontracting is-sues that resulted in contract cost, schedule, and/or performance problems.

g. Recommendation. To what extent would you recommend this contractor for additional contracts? Rate on the following scale: 5 = Yes, definitely; 4 = Yes, likely; 3 = Maybe, 2 = No, not likely; 1 = No, not ever; 0 = N/A.
