ATTACHMENT A
Performance Work Statement
Culture Self Assessment
1. INTRODUCTION: The Defense Language and National Security Education Office (DLNSEO) requires a culture assessment tool that will identify individual cultural capabilities as well as help recognize the effectiveness of education and training programs, across the Department of Defense, associated with interactions across cultures.
2. BACKGROUND:
DLNSEO is the policy office responsible for the Defense Language Regional Expertise and Culture Program (DLRECP) that supports global programs in over 100 regions and countries. In order to fulfill an expanding world-wide mission, the DoD allocates resources in support of language, region, and culture programs to develop and improve the skills and abilities of its personnel. The DoD must have an ability to determine if the current culture and region training programs are providing the required levels of learning. That ability will enable an accurate means to guide program development efforts, provide feedback that will help determine requirements, and ensure that there is an accurate return on investment.
3. SCOPE: The contractor shall provide maintenance support for the Cultural Intelligence (CQ) Self- Assessment Pro products.
4. REQUIREMENTS: The contractor shall:
4.1. Provide maintenance and support, date of award through twelve months, or final program post assessment, for the below listed products:

4.1.1. Provide personalized CQ Assessments to up to 500 individuals before and after completing a program requiring the individual to work across a varied cultural landscape (covering over 100 cultural regions). The programs could include:
4.1.1.1 Service training programs.
4.1.1.2 Deployment missions.
4.1.1.3 Study abroad programs.
4.1.1.4 Professional Exchange Programs.
4.1.1.5 Immersion programs.
4.1.2. Provide a common access website for individuals taking the assessment.
4.2. Provide a personalized CQ Feedback Report to each participant from the Pre-Assessment and the Post- Assessment. Feedback reports provide CQ ratings, worldwide norms, personal scores on specified cultural values, and an action plan for utilizing the feedback received.
4.3. Develop and provide mission specific, customized versions of the personalized CQ Assessments up to 300 designated personnel.
4.4 Provide a report, via MS Word, that details the data results of the following:
4.4.1 The aggregate results for the participants that indicate the effects of a training event or extended cultural interactions.
4.4.2 The comparison of results of individuals who were administered the regular CQ and those who were administered the customized report.

INSTRUCTIONS TO OFFERORS
OFFERORS SHOULD NOTE THE FOLLOWING IMPORTANT INFORMATION
REGARDING PROPOSAL SUBMISSION:
1. SAM: Any Offeror that submits a proposal in response to this solicitation must be registered in the System for Award Management (SAM), in accordance with FAR Clause 52.232-33 and DFARS Clauses 252.201-7000 and 252.204-7004 Alt A, prior to contract award. Registration information can be found at:
https://www.sam.gov/portal/public/SAM/.
2. Communication: Solicitation information and amendments will be posted to the Federal Business Opportunities website at www.fbo.gov. Offerors may email written questions requesting clarification of the RFP to the Contract Specialist,Shuna Ross at shuna.k.ross.civ@mail.mil. No information concerning this RFP or requests for clarification will be provided in response to telephone calls. Questions will be received up to 11:00 am Eastern on August 30, 2016. Questions received after the due date/time may not be answered.
3. Proposal Submissions: All proposal submissions in response to this solicitation must be received no later than the date and time specified on the SF1449. No electronic submissions will be accepted.
For hand delivery proposals, contact the Contract Specialist via email at least two business days prior to the due date for delivery instructions.
For mailed submissions, Offerors must submit notification to the Contract Specialist via email five days in advance of proposal delivery. This email notification shall also include a tracking number for the package. Submit proposals to the address below:

Proposal: Offerors shall submit one CD containing a complete copy of their proposal as well as hardcopies separated into the two volumes. Each volume shall be submitted in individual folders/binders as follows:
A. VOLUME I – Business: Offerors shall submit one hard copy of Tab I.
Tab 1:
i. Signed copy of the Standard Form 1449, with blocks 17 and 30 completed.
ii. Signed copy of the Standard Form 30 of any amendments issued against this RFP..
iii. Offeror’s DUNS number, Cage Code, and Tax ID number.
iv. Completed Representations and Certifications of Offeror (FAR Clause 52.212- 3 Alt I) and all fill-in clauses that require an offeror’s response.
Tab 2: Submit one hard copy of Tab 2.
Past Performance References: Offerors shall submit up to three relevant and recent past performance references for work completed as a prime contractor. The Government will utilize the references submitted to obtain additional performance information on the Offeror.
Subcontractors Past Performance will not be reviewed. Should the offeror possess no relevant past performance, a written statement shall be provided by the offeror so indicating. In this case, the Offeror will not be evaluated favorably or unfavorably on past performance and will be assigned a rating of Unknown Confidence.
B. VOLUME II – Technical: Offerors shall submit one hard copy of Volume II to address the items below. Page limitation - In total, the page count for this volume shall not exceed 30 single-sided pages (inclusive of any tables of contents, appendices, attachments, etc.) Margins shall not be less than ½ inch. Font may be Times New Roman or Arial with not less than eleven-pitch.
i. Technical Proposal: This section shall address the technical requirements of the Performance Work Statement (PWS) at Attachment A.
A. VOLUME III – Price Proposal: Offerors shall submit one hard copy and one soft copy (CD format) of Volume
III:
i. Proposed labor categories, and fully burdened labor rates and hours for the Base and all Option Periods.
ii. A brief description of each proposed labor category.
4. This acquisition will be a Firm-Fixed Price contract. Government will award with the lowest price, technically acceptable proposal based on all of the Requirements/SOW provided and that can meet the delivery schedule. In no event will the Government make an award against an offer that the Government does not find to be fair and reasonably priced. This is an all or none requirement. Partial quotes will not be accepted or evaluated and will be deemed nonresponsive to this requirement.

Acceptable All of the minimum acceptable criteria are clearly met by the offer.
The offeror’s offer meets all the performance and technical capability requirements defined in the SOW, Specifications, and Equipment List.Once the offeror’s offer has been determined to be “technically acceptable,” award will be based on cost only.
Unacceptable Not all of the minimum acceptable criteria are met by the offer. An unacceptable offer contains one or more deficiencies. Offer fails to meet specified minimum performance and technical capability requirements defined in the PWS, Specifications, and Equipment List.

52.219-1 SMALL BUSINESS PROGRAM REPRESENTATIONS (OCT 2014)
(a) Definitions. As used in this provision--
Economically disadvantaged women-owned small business (EDWOSB) concern means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States and who are economically disadvantaged in accordance with 13 CFR part 127. 252.204-7011 ALTERNATIVE LINE-ITEM STRUCTURE (SEP 2011)
(a) Line items are the basic structural elements in a solicitation or contract that provide for the organization of contract requirements to facilitate pricing, delivery, inspection, acceptance and payment. Line items are organized into contract line items, subline items, and exhibit line items. (b) If an alternative line item structure is proposed, the structure must be consistent with subpart 204.71 of the Defense Federal Acquisition Regulation Supplement and PGI 204.71. A sample solicitation line-item structure and a corresponding offer of a proposed alternative line-item structure follow.

252.232-7006 WIDE AREA WORKFLOW PAYMENT INSTRUCTIONS (MAY 2013)
(a) Definitions. As used in this clause--
Department of Defense Activity Address Code (DoDAAC) is a six position code that uniquely identifies a unit, activity, or organization.
Document type means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF).
Local processing office (LPO) is the office responsible for payment certification when payment certification is done external to the entitlement system.
(b) Electronic invoicing. The WAWF system is the method to electronically process vendor payment requests and receiving reports, as authorized by DFARS 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports.
(c) WAWF access. To access WAWF, the Contractor shall--
(1) Have a designated electronic business point of contact in the System for Award Management at https://www.acquisition.gov; and
(2) Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this Web site.
(d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training Course and use the Practice Training Site before submitting payment requests through WAWF. Both can be accessed by selecting the “Web Based Training” link on the WAWF home page at https://wawf.eb.mil/.
(e) WAWF methods of document submission. Document submissions may be via Web entry, Electronic Data Interchange, or File Transfer Protocol.
(f) WAWF payment instructions. The Contractor must use the following information when submitting payment requests and receiving reports in WAWF for this contract/order:
(1) Document type. The Contractor shall use the following document type(s).
Combo: Invoice/Receiving Report
(Contracting Officer: Insert applicable document type(s). Note: If a “Combo” document type is identified but not supportable by the Contractor's business systems, an “Invoice” (stand-alone) and “Receiving Report” (stand-alone) document type may be used instead.)
(2) Inspection/acceptance location. The Contractor shall select the following inspection/acceptance location(s) in WAWF, as specified by the contracting officer.

(3) Document routing. The Contractor shall use the information in the Routing Data Table below only to fill in applicable fields in WAWF when creating payment requests and receiving reports in the system.
