The purpose of this questionnaire is to obtain an evaluation of past performance on a contract that has been completed or is currently being performed by the Contractor identified in Area 1 below. You have received this questionnaire because the Contractor selected you to provide this evaluation in response to a request by the United States Special Operations Command (USSOCOM) for past and present performance information. Note: For prime contractors, relevancy will be assessed for contracts that are most similar to the effort in terms of the general scope of work. Efforts such as Government owned, contractor operated (GOCO) and contractor owned contractor operated (COCO) ISR services, unmanned air vehicle performance experience, integration and management of Government payload(s) experience, overseas logistics support network experience and similar magnitude of efforts. These previous efforts listed which are related to

United States Department of Defense contracts, will be considered highly relevant. The Government will also be looking at percent of work performed by the prime, management required, training required, subcontracts utilized, and customer supported.

b. Was the contractor able to complete their responsibilities relating to the contract with minimal layers of ineffective management and oversight?

c. What was the contractor's record of timeliness, completeness and quality of problem identification, corrective action plans, proposal submittals (especially responses to change orders, engineering change proposals, or other undefinitized contract actions), and the contractor's history of reasonable and cooperative behavior, effective business relations, and customer satisfaction?

5. PROGRAM MANAGEMENT AND OTHER MANAGEMENT

a. To what extent does the contractor discharge their responsibility for integration and coordination of all activity needed to execute the contract; identify and apply resources required to meet schedule requirements; assign responsibilities for tasks and actions required by contract; and communicate appropriate information to affected program elements in a timely manner?

b. What were the contractor's risk management practices, especially the ability to identify risks and formulate and implement risk mitigation plans?

c. Was the contractor flexible in responding to changing needs, including surge capacity and reach back capability for subject matter experts and / or key personnel?
