About this file
This is a General Contract for Services (Option II) template document used by the Columbia County School District in Georgia for staffing and labor services. The contract covers multiple service categories including Special Education Teachers (IRR, SLP, EBD, ASD, and CCETS positions), General Education Paraprofessionals, Special Education Paraprofessionals (ASD, EBD, SID/PID, MOID, IRR, Pre-K, and CCETS), American Sign Language (ASL) Paraprofessionals, and School Nutrition Assistants. The contract is structured for an initial one-year term from the date of award, with options to extend for up to four successive one-year periods based on mutual consent between the contractor and the district. The template includes fillable fields for specific service descriptions, dates of service commencement and completion, and payment terms.
The contract requires contractors to provide documentation including a completed W-9 Form, business licenses, certificates of liability insurance, and workers' compensation insurance (where applicable) prior to commencing work. Contractors must maintain general liability coverage of at least $1,000,000 aggregate liability and $1,000,000 per occurrence, with the Columbia County Board of Education listed as an additional named insured. Payment terms are customizable and specified in the contract. The agreement establishes the contractor as an independent contractor with no entitlement to district employee benefits, including unemployment compensation or workers' compensation. Contractors are solely responsible for all tax reporting and payments. The contract includes standard provisions for confidentiality, indemnification, facility protection, HIPAA and FERPA compliance, and includes a waiver and release of liability form that may be accepted only when all options to obtain insurance certificates have been exhausted. Either party may cancel the contract with 30 days' prior written notice.
