About this file
This document is a subcontractor consent form template for the Department of the Air Force solicitation FA330027R0002.
The form is designed to obtain written consent from major subcontractors and teaming partners to allow the Government to disclose their past and present performance information to the prime contractor during the source selection process. The document explains that because past performance information concerning subcontractors and teaming partners cannot be disclosed to private parties without consent, and prime contractors are considered private parties, the Government requires this consent before sharing subcontractor/teaming partner performance data with primes during proposal exchanges. Completed consent forms must be submitted by major subcontractors and teaming partners as part of the Past Performance Volume (or main Proposal), with the specific submission location detailed in Section M, Volume III of the solicitation. The sample letter form requires the subcontractor/teaming partner to identify their role, the prime contractor they are partnering with, and the specific Air Force solicitation and program they are responding to. The form must be signed by an individual with authority to legally bind the company and include the company name and address.

Attachment 4 – Solicitation # FA330027R0002
SUBCONTRACTOR CONSENT LETTER
SUBCONTRACTOR/TEAMING PARTNER CONSENT FORM FOR THE RELEASE OF
PAST AND PRESENT PERFORMANCE INFORMATION TO THE PRIME
CONTRACTOR
Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor's or teaming partner's consent. Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime during exchanges. In an effort to assist the Government's Past Performance Team in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the major subcontractors/teaming partners identified in your proposal. The completed consent forms should be submitted as part of your Past Performance Volume (or Proposal). (Note: Section M, Volume III, Past Performance should specify if past performance is to be submitted as a separate proposal volume).
