About this file

This is a Statement of Work (SOW) budget template for FY2027 awards under the Department of Defense HIV/AIDS Prevention Program (DHAPP), administered by the Defense Health Agency through the USAMRAA execution office. The template is designed for Phase 1 new awards and uses a cooperative agreement funding instrument with the federal agency designated as DOD and partner name as DHA. The template establishes a standardized framework for budget planning and narrative documentation across up to 20 distinct interventions, with all monetary values currently showing $0 pending data population.

The template structure requires implementing partners to complete metadata fields including Award Number (TBD), Vendor ID, Mech ID, Prime DUNS Number (requiring 9 characters), and Operating Unit information. Data entry occurs in the "Work Plan Budget" tab and "DHAPP SOW" tab, with mandatory fields for Program Area selection from a dropdown list, Beneficiary designation, and cost category allocation across 18 line items including personnel salaries, fringe benefits, travel, equipment, supplies, contractual services, construction, training, and subrecipient costs. The DHAPP SOW tab requires population of MER Indicator Targets, Above Site (AS) activity outputs, and intervention narratives, with budgeted amounts in this column matching the Work Plan Budget total. Supporting reference data includes detailed Area of Cooperation categories (2.1 through 2.6), Sub-Categories spanning surveillance, laboratory systems, commodities, frontline health workers, data systems, and strategic assistance, and eligible Health Program Areas (HIV, TB, Malaria, GHS, and N/A for Partner Government Funding Only). The template accommodates fringe benefit rates and the number of subrecipients (0-100) for financial planning purposes.
