About this file
This is the FY2026 Distance Learning and Telemedicine (DLT) Application Checklist for the Rural Utilities Service (RUS). The checklist outlines all required submission components for applicants seeking federal grant funding to support distance learning and telemedicine infrastructure projects in rural areas. Applicants must complete and submit 13 major sections: a Standard Form 424, site worksheet, executive summary with publicly releasable project description, scoring criteria documentation addressing rurality and economic need, matching requirements, detailed scope of work with specific activities and timeframes, financial information and sustainability plan, statement of organizational experience, and evidence of legal existence with SAM.gov registration.
The telecommunications systems plan section requires comprehensive documentation including equipment capabilities and listings, carrier consultation records, service area maps with system overlays, and identification of all hub and end-user sites, with tribal consent required for non-tribal applicants with sites on tribal lands. Additional mandatory submissions include compliance certifications addressing flood hazards, relocation assistance policies, architectural barriers, and non-duplication of services, along with environmental impact and historic preservation narratives specifying project description, locations, and proposed NEPA class of action. State director consultation documentation is required for each state containing end-users. By signature, applicants certify agreement with all language in the required schedules and acknowledge that any flood insurance or other attachments required by checked boxes are included with the application submission.
